- 6. The individual problem •Too busy to track their health •Often forget to track simple changes in their condition. •Leads them to having an ineffective conversation with their HCP
- 7. The individual problem • People living with long term diseases in the UK struggle to improve their condition and quality of life.
- 8. Root cause • Too many health apps available in the market • Older people struggle to use visual user interfaces • Inconvenient to regularly have to track your own physical and mental symptoms

- 12. The opportunity Check up will allow patients with long term conditions to input data using Alexa, which will then provide actionable content to improve the users condition. The data is then accessible by their healthcare practitioners

Solution • We present the patient with a series of questions to get them to start thinking about their health holistically • We use their answers to our questions to serve the right actionable content to them • We store their data to allow them to track it over time and share it with their HCPS
