Detailed Job Description:
Assess and evaluate patient’s physical, functional and cognitive level and capacity. Activities
of daily living.
2. Carry out rehab programs given by the physiatrist to their home as a Home Health Care
provider.
Providing home service for patients who cannot come to the hospital and prefer treatment at
home.
Provides patient and family education regarding home exercise program, goal directed
treatments and outcomes. Training the family to become independent in home exercise
program.
Assess patient’s home situation/ set up in order to adopt patient’s capacity in his
environment.
Monitor and document patient’s progress in the patient files.
Adjust treatment regimens as to patient’s response.
Supervises interns in the department regarding clinical performance and patient care.
Have handled patients with mixed case diagnosis i.e Neurologic, Orthopedic, Pediatric, and
other musculoskeletal conditions.
Modalities being used in the department but not limited to:
