- I’m presenting a project related to the development of a PHR with a diabetes self-management tool for Portuguese speaking patients. This project is being funded by a grant from the Harvard Medical School Portugal program.
- Additionally, patients’ health information is scattered through the EHRs of several institutions and health care providers.
- is currently inexistent, which can have a negative impact on the provision of health care, and can also hamper the promotion of health literacy and patient empowerment.
- Therefore, there is currently the need to put patients at the centre of healthcare and in control of their health information, which they sould be able to share with whoever they wanted.
- And we thought about diabetes because this is currently a considerable health problem in Portugal, with a rapidly increasing prevalence that reached 12.4% in 2011. Additionaly, about half of the Portuguese diabetics do not reach metabolic control.
- Several web-based interventions have already been used in DM self-management, and they tend to show the potential to improve various outcomes, like glycemic control, diabetes knowledge, quality of life, among others.
- and, we also decided to create a module for type 2 diabetes self-management.
- And, finally, a design team.
- The development of the PHR was patient-centered and we followed the “Research-based web design and usability guidelines” throughout the project. Furthermore, we analyzed the feedback messages we got from the users of the platform to guide its improvement.
- And so the PHR was launched in May 2010. and This is the homepage. For now, data has to be self-entered by patients although in the future the plan is to integrate the PHR with the several EHRs available throughout the country.
- An important feature of the PHR is the ability to share all or just part of the health information with anyone patients choose to do so.
- And, finally, there is also a diabetes module to keep all the information related to the disease and to access relevant content to help its self-management
- One of the features of the diabetes module is the cardiovascular risk calculator that apart from calculating the cardiovascular risk, provides a link to health advices to reduce or maintain that risk, depending on the situation and based on that particular patient.
- Furthermore we developed guiding sliders to help patients throughout their use of the PHR.
- The evaluation of the platform is now taking place. In terms of awareness, despite not having been yet advertised, the PHR has now close to 5000 users.
- And finally, we want to evaluate its cost-effectiveness.
