Abstract
A patient fall is defined as an unplanned descent to the floor with or without injury to the
patient. A fall may result in fractures, lacerations, or internal bleeding, leading to increased
health care utilization. According to agency for Healthcare and Quality (2013), research shows
that close to one-third of falls can be prevented. Fall reduction in the inpatient is important in
healthcare, and finding articles to conduct a research on this topic can prove challenging.
Nevertheless, learning how to conduct an effective library database search and website search
enhances the chance of finding information that is current, peer reviewed, accurate and with
detailed information. With this in mind, two search engines on the World Wide Web (www);
Google and Yahoo and two library databases; CINAHL and ProQuest were used to discover
valid information pertaining to fall reduction in the inpatient. In the end, Google and Yahoo
provided information mainly by directing to websites that provided the information that was not
always reliable while CINAHL and ProQuest provided mainly scholarly documents.

Website Search and Evaluation
Fall reduction in the inpatient is a major concern in healthcare. Google and Yahoo were used as
the search engines of choice in the search for valid information pertaining to this topic. Each site
provided similar results. Initially, the two keywords used were: Fall reduction. Fall reduction was
searched in both Google and Yahoo, a great deal of blogs and articles were discovered but
nothing that gave detailed information on the topic. However, after advancing the search by
adding more words to make it: Fall reduction in hospitals, a great deal of articles from peer
reviewed sites and nursing journals were found. One difference between Google and Yahoo was
that the similar articles would be found with each search but they would appear under different
headings until that particular article or website was accessed. For example; the same article from
American Nurses Today appears in Yahoo’s search as ‘’the average hospital stay for patients
who fall is….’’, while on Google’s search, it is shown as ‘’Best Practices for Falls Reduction: A
Practical Guide-American Nurse…’’ (see Appendix C). In order to assess validity of the
resources found on the websites, the Kent and Cornell website evaluation tool was used. Google
was evaluated by Kent website evaluation tool and Yahoo was evaluated by Cornell website
evaluation tool.
Firstly, the Kent website evaluation tool is derived from Kent State University and aids in
assessing and evaluating the reliability of resources found on websites. In this case, it was used
to evaluate the information found on American Nurse Today via Google search. With focus on
fall reduction in hospitals and the article ‘’Special Supplement to American Nurse Today – Best
Practices for Falls Reduction: A Practical Guide’’. The article was written by multiple authors
from various areas of healthcare in March, 2011. Providing that, American Nurse Today was
created by HealthCom Media, which is a larger website. American Nurses Today is one of three

websites governed by HealthCom Media; Women’s Healthcare, Wound Care Advisor.
Moreover, it is affiliated with the American Nurses Association (ANA). The credentials of the
owner of the Website were not found but the document being analyzed was written by healthcare
professionals such as doctors and nurses and with references attached. Organizational
information is readily available and the site has copyrights dated 2016. Furthermore, the purpose
and intention of the Website is clear; peer reviewed clinical information is provided to healthcare
workers and patients. Affiliations were found but sponsorship was not disclosed. The
information on the website was not current and some of the links on the website and in the article
itself were outdated. In addition, the pages were not dated and stamped, the site is not well
designed but it was easy to maneuver. The content is written at a readable level for healthcare
workers but not for patients. Information was presented without grammatical errors. American
Nurse Today is easily accessed and the link to the homepage is accessed by simply selecting
home at the top left of the page (See Appendix A).
Secondly, the Cornell University Website evaluation tool was used to critique the Agency for
Healthcare and Quality (AHRQ) via a Yahoo search of fall reductions in hospitals. AHRQ is a
product of the United States Department of Health and Human Services. The document was
written by multiple medical professionals such as nurses, practitioners and doctors with their
credentials listed. Indeed, the purpose of the website is to provide information on current
practices and research in healthcare. Also, the contact information for the author or cooperation
that is responsible for the site is provided. The URL is from the government (.gov). The
information is cited as bibliography and listed correctly. Moreover, AHRQ is current with latest
reviewed date stamped at the bottom of the page and the links were functioning. In addition, the
Website also provided information on grants, various funding and sponsorships in English and

Spanish. The Website is easy to navigate with detailed, current information provided (see
Appendix B).
In the end, both Kent and Cornell Website evaluation tool are effective in analyzing Websites for
their authenticity. Both tools utilized similar questions to evaluate the sites with minor
differences in sentence structure and wording.
Monmouth University Library Database Search
A library database search was conducted on the topic; fall reduction in hospitals using CINAHL
and ProQuest (See table of searches). The full text was chosen from CINAHL and carefully
evaluated; while a second citation was chosen from ProQuest that did not have full text.
Nonetheless, there are two ways to retrieve the full text of an article. First attempt would be to
find the article within the database by selecting the link to full text tab at the top left corner, and
follow the prompts. The next prompt is to select the tab ‘’Request this item’’, then the library
Webpage will ask that a New Clio Interlibrary Web Account be created. After creating an
account, the Website will request the information needed to locate the article in full text. Such as;
title, author, publication date, edition, publisher, ISBN number, citation source, notes and the
date of which you will need the article. After submitting the information, the librarian will
contact you via email. Second attempt would be o search directly from the library’s homepage
by typing information such as title, topic or author.

Falls are often the cause of additional unplanned injury in acute care facilities. Therefore, this
topic is of great importance to healthcare workers, especially nurses as fall reduction is a mainly
nurse driven measure. In order for falls to be decreased, there needs to be more enhanced policies
to aid in fall prevention. In a study published by the Journal of Advanced Nursing, reducing
serious fall-related injuries in acute hospitals with low-low beds was evaluated. Low-low beds
are hospital beds that can be lowered all the way to the floor and helps eliminate harm to the
patient if they fall (Barker, Hill, Kamar, & Tyndall, 2013). The fall reduction team set this
project in motion and continued to monitor the outcome by reminding staff to lower beds to the
floor and increased rounding.
Further, use of the low-low beds were carefully considered in patient type such as patients with
cognitive impairment and patients who may attempt to get out of bed unassisted. In addition,
staff was encouraged to perform thorough assessments in order to identify patients that would
benefit from low-low beds (Barker, Hill, Kamar, & Tyndall, 2013). In the end, results of the
study showed a decrease in falls with injury but no significant change in the number of falls. A
low-low bed will not stop a fall from occurring. Patients with cognitive impairment were still
counted as a fall if they rolled out of bed, even if no injury was sustained (Barker, Hill, Kamar,
& Tyndall, 2013)

Introduction to Supplement – Meeting the
challenge of falls reduction
Preventing patient falls is at the top of mind for every caregiver in every clinical setting.
Multidisciplinary teams across the continuum of care convene each day as falls
“champions” to determine the best ways to identify patients at highest risk for falls and
to develop falls-prevention strategies.
Despite heightened national attention to this issue, threats to reimbursement, and the
best possible intentions, patient fall rates across the United States continue to escalate,
putting patients and caregivers at increasing risk. The facts are undeniable: Up to 50%
of hospitalized patients are at risk for falls, and almost half of those who fall suffer an
injury.

Each year, somewhere between 700,000 and 1,000,000 people in the United States fall in the hospital. A
fall may result in fractures, lacerations, or internal bleeding, leading to increased health care utilization.
Research shows that close to one-third of falls can be prevented. Fall prevention involves managing a
patient's underlying fall risk factors and optimizing the hospital's physical design and environment. This
toolkit focuses on overcoming the challenges associated with developing, implementing, and sustaining a
fall prevention program.

  • 3. LIBRARY DATABASE AND WEBSITE SEARCH 3 Abstract A patient fall is defined as an unplanned descent to the floor with or without injury to the patient. A fall may result in fractures, lacerations, or internal bleeding, leading to increased health care utilization. According to agency for Healthcare and Quality (2013), research shows that close to one-third of falls can be prevented. Fall reduction in the inpatient is important in healthcare, and finding articles to conduct a research on this topic can prove challenging. Nevertheless, learning how to conduct an effective library database search and website search enhances the chance of finding information that is current, peer reviewed, accurate and with detailed information. With this in mind, two search engines on the World Wide Web (www); Google and Yahoo and two library databases; CINAHL and ProQuest were used to discover valid information pertaining to fall reduction in the inpatient. In the end, Google and Yahoo provided information mainly by directing to websites that provided the information that was not always reliable while CINAHL and ProQuest provided mainly scholarly documents.   • 4. LIBRARY DATABASE AND WEBSITE SEARCH 4 Website Search and Evaluation Fall reduction in the inpatient is a major concern in healthcare. Google and Yahoo were used as the search engines of choice in the search for valid information pertaining to this topic. Each site provided similar results. Initially, the two keywords used were: Fall reduction. Fall reduction was searched in both Google and Yahoo, a great deal of blogs and articles were discovered but nothing that gave detailed information on the topic. However, after advancing the search by adding more words to make it: Fall reduction in hospitals, a great deal of articles from peer reviewed sites and nursing journals were found. One difference between Google and Yahoo was that the similar articles would be found with each search but they would appear under different headings until that particular article or website was accessed. For example; the same article from American Nurses Today appears in Yahoo’s search as ‘’the average hospital stay for patients who fall is….’’, while on Google’s search, it is shown as ‘’Best Practices for Falls Reduction: A Practical Guide-American Nurse…’’ (see Appendix C). In order to assess validity of the resources found on the websites, the Kent and Cornell website evaluation tool was used. Google was evaluated by Kent website evaluation tool and Yahoo was evaluated by Cornell website evaluation tool. Firstly, the Kent website evaluation tool is derived from Kent State University and aids in assessing and evaluating the reliability of resources found on websites. In this case, it was used to evaluate the information found on American Nurse Today via Google search. With focus on fall reduction in hospitals and the article ‘’Special Supplement to American Nurse Today – Best Practices for Falls Reduction: A Practical Guide’’. The article was written by multiple authors from various areas of healthcare in March, 2011. Providing that, American Nurse Today was created by HealthCom Media, which is a larger website. American Nurses Today is one of three
  • 5. LIBRARY DATABASE AND WEBSITE SEARCH 5 websites governed by HealthCom Media; Women’s Healthcare, Wound Care Advisor. Moreover, it is affiliated with the American Nurses Association (ANA). The credentials of the owner of the Website were not found but the document being analyzed was written by healthcare professionals such as doctors and nurses and with references attached. Organizational information is readily available and the site has copyrights dated 2016. Furthermore, the purpose and intention of the Website is clear; peer reviewed clinical information is provided to healthcare workers and patients. Affiliations were found but sponsorship was not disclosed. The information on the website was not current and some of the links on the website and in the article itself were outdated. In addition, the pages were not dated and stamped, the site is not well designed but it was easy to maneuver. The content is written at a readable level for healthcare workers but not for patients. Information was presented without grammatical errors. American Nurse Today is easily accessed and the link to the homepage is accessed by simply selecting home at the top left of the page (See Appendix A). Secondly, the Cornell University Website evaluation tool was used to critique the Agency for Healthcare and Quality (AHRQ) via a Yahoo search of fall reductions in hospitals. AHRQ is a product of the United States Department of Health and Human Services. The document was written by multiple medical professionals such as nurses, practitioners and doctors with their credentials listed. Indeed, the purpose of the website is to provide information on current practices and research in healthcare. Also, the contact information for the author or cooperation that is responsible for the site is provided. The URL is from the government (.gov). The information is cited as bibliography and listed correctly. Moreover, AHRQ is current with latest reviewed date stamped at the bottom of the page and the links were functioning. In addition, the Website also provided information on grants, various funding and sponsorships in English and
  • 6. LIBRARY DATABASE AND WEBSITE SEARCH 6 Spanish. The Website is easy to navigate with detailed, current information provided (see Appendix B). In the end, both Kent and Cornell Website evaluation tool are effective in analyzing Websites for their authenticity. Both tools utilized similar questions to evaluate the sites with minor differences in sentence structure and wording. Monmouth University Library Database Search A library database search was conducted on the topic; fall reduction in hospitals using CINAHL and ProQuest (See table of searches). The full text was chosen from CINAHL and carefully evaluated; while a second citation was chosen from ProQuest that did not have full text. Nonetheless, there are two ways to retrieve the full text of an article. First attempt would be to find the article within the database by selecting the link to full text tab at the top left corner, and follow the prompts. The next prompt is to select the tab ‘’Request this item’’, then the library Webpage will ask that a New Clio Interlibrary Web Account be created. After creating an account, the Website will request the information needed to locate the article in full text. Such as; title, author, publication date, edition, publisher, ISBN number, citation source, notes and the date of which you will need the article. After submitting the information, the librarian will contact you via email. Second attempt would be o search directly from the library’s homepage by typing information such as title, topic or author.   • 7. LIBRARY DATABASE AND WEBSITE SEARCH 7 Falls are often the cause of additional unplanned injury in acute care facilities. Therefore, this topic is of great importance to healthcare workers, especially nurses as fall reduction is a mainly nurse driven measure. In order for falls to be decreased, there needs to be more enhanced policies to aid in fall prevention. In a study published by the Journal of Advanced Nursing, reducing serious fall-related injuries in acute hospitals with low-low beds was evaluated. Low-low beds are hospital beds that can be lowered all the way to the floor and helps eliminate harm to the patient if they fall (Barker, Hill, Kamar, & Tyndall, 2013). The fall reduction team set this project in motion and continued to monitor the outcome by reminding staff to lower beds to the floor and increased rounding. Further, use of the low-low beds were carefully considered in patient type such as patients with cognitive impairment and patients who may attempt to get out of bed unassisted. In addition, staff was encouraged to perform thorough assessments in order to identify patients that would benefit from low-low beds (Barker, Hill, Kamar, & Tyndall, 2013). In the end, results of the study showed a decrease in falls with injury but no significant change in the number of falls. A low-low bed will not stop a fall from occurring.
