CONTEXT:
We at Healthcare self-insurance and consulting Group offer our customers variety of services to our clients. We
encourage many wellness activities, promote self-management through healthcare Apps and increased patient clinician
interaction. Providing such services requires building an IT infrastructure and systems which can support huge volume of
customers. In this document we have designed a swim lane diagram and data dependency chart. The purpose of the
swim lane diagram in this document is to present the flow of all the processes in the wellness activity tracking system. It
also shows how the wellness activities are tracked. Apart from that it describes how the customer feedback is captured
and analysed to evaluate the success of wellness activities.
This document also includes a Data dependency chart for the wellness activity tracking system. The data dependency
describes how the wellness activity tracking system is integrated with other existing system, various departments
accessing the system’s database, the flow of system inside and outside the system.
Our company is the only consulting group which uses various technologies so it becomes necessary to remain
competitive in the market. In short the process modelling diagrams gives the overview of the system at a higher level.

Swim lane Diagram:
The flow of this swim lane starts with the WAPC which plans the wellness activity after speaking to theirs key
partners (Yoga centres, Nutritionists, Marathon Sponsors).
These plans are sent to the enrolment department. As soon as these plans are received by the enrolment
department they access their database of the customers. These data of the customers is sent to the Notification and
Fetch component (NFC) of the wellness tracking system.
The system 4: Medical Information tracking system also sends the data of the potential customers which will attend
the related activities. This data is also sent to the Notification and Fetch component.
The NFC fetches data from DB1 and DB3. This data is used to notify all the customers via Apps or web portal.
Customers login into the system with their Apps or web portal in order to RSVP for the wellness activity
Whether the correct user is logging into the system and whether a particular customer is allowed for a particular
activity is decided by the Verification and Fetch Component. If there is any error or illegibility then the system will
notify the user accordingly.
The information of the customers who have already RSVP the wellness activity is stored in the wellness tracking
system’s database. The customers who actually attended the wellness activity would swipe their cards at the
registration desk. This information is updated in the database of the wellness tracking system. After the wellness
activity feedbacks are taken from the customers.
This data along with the data of the customers who actually attended is sent to the WAPC. WAPC analyses the
feedback and accordingly generates the success reports of the activity.

1) Enrollment Department sends the data of all the students, staff and non-teaching staff in the university to the
Notification & Fetch component of our system
2) Medical Information of patients can be fed into the system. For example: If this month the wellness activities
planned are related to the healthy food and nutrition then all the customers having diabetes, heart diseases can
be notified. Therefore, the medical information of the customers can be useful for notifying customers.
3) The Notification & Fetch firstly fetches data of all the customers and all the potential customers for the event
from Enrollment Department and System 4: Medical Information respectively. Based on the data fetched,
Notification & Fetch will send the notification regarding the activities planned in near future. The notification
will be sent via Healthcare App and the web portal.
4) As soon as the Customers get notification regarding the wellness activities planned, they will try to login into the
system either via Apps or web portal. These login credentials are sent to the Verification and Validation
component of our system.
5) Once the verification is done, the Verification and Validation component will send the result stating whether
login credentials were right or wrong
6) Customers can RSVP for the wellness activities (yoga, marathon, nutrition seminars etc.) that match their
interests. If they RSVP a particular activity it is sent to the Validation and Verification component to check
whether the customer is eligible to attend a particular activity. For example: A patient with asthma/breathing
elements should preferably not attend a marathon event.
7) The Validation and Verification component will evaluate and then the customers will be allowed to RSVP or they
will be given a pop on the screen that they cannot enroll for that particular activity.
8) The Verification and Validation component will also send the data to the DB of our system. The data comprises
of how many customers have already RSVP the wellness activity and how many customers actually attended the
activity. Also, it sends the feedback of the activities of all the customers who attended the wellness activities.
9) The system’s database is fetched by two departments. One of the departments is Finance. They use this data to
get an estimate of how many customers will be attending the wellness activity. This data is useful to make a
rough estimate for the budget of wellness activity.

10)The other department which accesses the database is Wellness Activity Planning Center (WAPC). They fetch two
kinds of data:
i) Data of customers who have already RSVP for the wellness activity for planning venue, time etc.
ii) Data and feedback of customers who actually attended the wellness activity to analyze feedback and
generate success reports

September 26, 2015 HEALTH CARE SELF INSURANCE AND CONSULTING GROUP 2 Jash Mehta | HSICG CONTEXT: We at Healthcare self-insurance and consulting Group offer our customers variety of services to our clients. We encourage many wellness activities, promote self-management through healthcare Apps and increased patient clinician interaction. Providing such services requires building an IT infrastructure and systems which can support huge volume of customers. In this document we have designed a swim lane diagram and data dependency chart. The purpose of the swim lane diagram in this document is to present the flow of all the processes in the wellness activity tracking system. It also shows how the wellness activities are tracked. Apart from that it describes how the customer feedback is captured and analysed to evaluate the success of wellness activities. This document also includes a Data dependency chart for the wellness activity tracking system. The data dependency describes how the wellness activity tracking system is integrated with other existing system, various departments accessing the system’s database, the flow of system inside and outside the system. Our company is the only consulting group which uses various technologies so it becomes necessary to remain competitive in the market. In short the process modelling diagrams gives the overview of the system at a higher level.

September 26, 2015 HEALTH CARE SELF INSURANCE AND CONSULTING GROUP 4 Jash Mehta | HSICG Swim lane Diagram: The flow of this swim lane starts with the WAPC which plans the wellness activity after speaking to theirs key partners (Yoga centres, Nutritionists, Marathon Sponsors). These plans are sent to the enrolment department. As soon as these plans are received by the enrolment department they access their database of the customers. These data of the customers is sent to the Notification and Fetch component (NFC) of the wellness tracking system. The system 4: Medical Information tracking system also sends the data of the potential customers which will attend the related activities. This data is also sent to the Notification and Fetch component. The NFC fetches data from DB1 and DB3. This data is used to notify all the customers via Apps or web portal. Customers login into the system with their Apps or web portal in order to RSVP for the wellness activity Whether the correct user is logging into the system and whether a particular customer is allowed for a particular activity is decided by the Verification and Fetch Component. If there is any error or illegibility then the system will notify the user accordingly. The information of the customers who have already RSVP the wellness activity is stored in the wellness tracking system’s database. The customers who actually attended the wellness activity would swipe their cards at the registration desk. This information is updated in the database of the wellness tracking system. After the wellness activity feedbacks are taken from the customers. This data along with the data of the customers who actually attended is sent to the WAPC. WAPC analyses the feedback and accordingly generates the success reports of the activity.

September 26, 2015 HEALTH CARE SELF INSURANCE AND CONSULTING GROUP 5 Jash Mehta | HSICG 1) Enrollment Department sends the data of all the students, staff and non-teaching staff in the university to the Notification & Fetch component of our system 2) Medical Information of patients can be fed into the system. For example: If this month the wellness activities planned are related to the healthy food and nutrition then all the customers having diabetes, heart diseases can be notified. Therefore, the medical information of the customers can be useful for notifying customers. 3) The Notification & Fetch firstly fetches data of all the customers and all the potential customers for the event from Enrollment Department and System 4: Medical Information respectively. Based on the data fetched, Notification & Fetch will send the notification regarding the activities planned in near future. The notification will be sent via Healthcare App and the web portal. 4) As soon as the Customers get notification regarding the wellness activities planned, they will try to login into the system either via Apps or web portal. These login credentials are sent to the Verification and Validation component of our system. 5) Once the verification is done, the Verification and Validation component will send the result stating whether login credentials were right or wrong 6) Customers can RSVP for the wellness activities (yoga, marathon, nutrition seminars etc.) that match their interests. If they RSVP a particular activity it is sent to the Validation and Verification component to check whether the customer is eligible to attend a particular activity. For example: A patient with asthma/breathing elements should preferably not attend a marathon event. 7) The Validation and Verification component will evaluate and then the customers will be allowed to RSVP or they will be given a pop on the screen that they cannot enroll for that particular activity. 8) The Verification and Validation component will also send the data to the DB of our system. The data comprises of how many customers have already RSVP the wellness activity and how many customers actually attended the activity. Also, it sends the feedback of the activities of all the customers who attended the wellness activities. 9) The system’s database is fetched by two departments. One of the departments is Finance. They use this data to get an estimate of how many customers will be attending the wellness activity. This data is useful to make a rough estimate for the budget of wellness activity.

They fetch two kinds of data: i) Data of customers who have already RSVP for the wellness activity for planning venue, time etc. ii) Data and feedback of customers who actually attended the wellness activity to analyze feedback and generate success reports
