1) The document describes a proposed health monitoring system that would centralize patient information and allow patients to access their medical records, prescriptions, reports and more through a mobile app and smart card.
2) Key features of the system include reduced wait times, guidance through the hospital process, online appointments, electronic prescriptions that can be filled directly by pharmacists, and medication reminders.
3) The system aims to improve efficiency for patients, doctors and hospitals by streamlining processes and improving information access, which could help increase standards of healthcare overall.

3. Introduction
• Today patient needs to carry all the documents related to their medical hist
ory and previously prescribed medications all along with him.
• Workflow from the front desk to physicians to billing office staff is seamle
ss.
• Patients have to carry health card.
• Health card is accessible at the Hospitals, Doctor, Pharmacy store, Patholo
gy labs.
• Where the machine learning and deep learning concepts extends more func
tionality.
• Health assistance and medication management.
4. Problem Statement
• Despite of taking an appointment at the reception, the patient needs to wait for his t
urn to come in long queues.
• Patients don’t get proper details of the whereabouts of the next department as
stated by the doctor.
• Patients don’t get timely reminders for their regarding treatment follow-ups.
• Patients need to go to distant places for collecting their prescribed medicines from t
he chemist and their pathology reports from the pathology labs.
• It always has been a issue to find a match immediately for required blood type when
emergency occurs.
5. Objectives
• centralized Information of patients.
• link up with Aadhar Card.
• Patient will easily be able to access all information regarding the prescribed
medications, reports, billing services options on the app itself.
• The suggestions for the chemist by the availability of medicine in nearest
area.
• To build a next generation cognitive assistant with advanced multimodal
analytics, clinical knowledge and reasoning capabilities that is qualified to
assist in clinical decision making.
• Medicine Remainder
• Doctor can prescribe medicine online, chemist can give medicine using that
prescription and pathologist also upload the report of patient.
8. • Web Portal Application
• Frontend
– HTML5/CSS3/Bootstrap/AngularJS
• Backend
– Spring maven
– Hibernate
– RESTful web services
• Database
– MySQL
9. Features
• All the information regarding the patient will be centralized and would be accessible
via his smart card once he has registered for the same.
• Patient will easily be able to access all information regarding the prescribed medicati
ons, reports, follow-ups, blood type availability including payment options on the ap
p itself.
• The patient will be guided through all the departments inside the hospital from the ve
ry start of his treatment process till the end.
• The suggestions for the chemist in the app will be prioritized according to the nearest
locations and their availability.
• The required blood donor will immediately be sent alert messages whenever there is
a emergency case for a particular blood type.
19. Expected outcome
• The patients will no longer have to wait in long queues and thereby will
reduce the registration and process.
• The doctors and researchers will be able to scan and research through past
details of the patients which would help them in curing certain diseases.
• The hospitals will able to increase their management efficiencies.
• The society will get an overall benefit thereby increasing the health standard
s.
• The hospitals will able to increase their management efficiencies.
• Patients/User can collect or know more information from home using assista
nce.
20. Advantages
• Health card users (physicians, physician assistants, pharmacists, nurse practit
ioners, nurses) are provided with a simple and more efficient logging on the
process with no need to remember complex passwords.
• Healthcare providers have easy access to all necessary patient health informa
tion; therefore, health information is not obtained from the patient’s memory,
which improves the process of gathering health information.
• The data of patient will be centralized.
• Online appointment to save the time and don’t need to stand in the queue.
• E-prescription by the doctor that can be helped to patient and pharmacist.
• Medication reminder, Chabot, Virtual nursing assistance, health prediction.
• Paperwork will be reduced.
21. Limitation
• Internet connection is required
• Admin has to issue and maintain the card and all record of the patient’s.
• Hospital, personal clinic, pharmacist have this system on their computers.
22. Future work
• We cannot consider android based information system and website as a com
plete system. The System continues to evolve throughout the life-cycle.
• Due to the creative nature of design, there remains some gapes/lapse mistake
n communication between users and developers.
• So, some aspects of the system must be modified as experience is developed
with it. As users work with the system, they develop ideas for change and up
-gradation in the system.
• In future, we are planning for better user interface and interactive environme
nt so the user can easily get the data and visit our website.
