Several issues were identified at a weekly contract meeting regarding incident reporting at Sydney Water sites. Permit to Work were not being signed by subcontractors, incidents were not being reported to site managers, and poor work practices were not being raised. The bulletin reminds all managers to ensure employees and subcontractors personally report all incidents to the helpdesk and relevant site manager before leaving site. Actions include raising the alert at monthly toolbox talks and making subcontractor companies aware of reporting requirements.

Incident Description
At the weekly contract meeting held on the 21st June 2017, the following issues were raised from feedback
across Sydney Water:
 Permit to Work (PTW) are not being signed by sub-contractors,
 Incidents are not being reported to the relevant site managers; Plant Manager/Production Officers
and Delivery Team Leaders &
 People are not raising concerns regarding poor work practices.
Preliminary Findings/ Underlying Causes
Plant Managers/Production Officers and Delivery Team Leaders have a duty of care to ensure all
incidents are reviewed, injured persons are managed, damage is assessed and, the security of assets is
maintained.
The current process of relying solely on a call to the helpdesk needs to be supported by additional
contact with the relevant site managers before leaving the site.
Reminder
All managers are to ensure every employee and sub-contractor are made aware of the requirement to
individually and personally report incidents to the Helpdesk AND site Plant Manager/Production Officer &
Delivery Team Leader before leaving site, this includes unmanned sites.
In addition, where poor work practices are identified, such issues are required to be raised with the
responsible workforce members and/or Sydney Water/Ventia site managers.
Actions
 Internal staff: Alert to be raised at the monthly TBT’s.
 Sub-contractor management: Managers of sub-contracting staff are to ensure companies are aware
of the additional reporting processes.
