Permit to Work were not being signed by subcontractors, incidents were not being reported to site managers, and poor work practices were not being raised. The bulletin reminds all managers to ensure employees and subcontractors personally report all incidents to the helpdesk and relevant site manager before leaving site. Actions include raising the alert at monthly toolbox talks and making subcontractor companies aware of reporting requirements.
