4. PURPOSE
ā¢ To prevent disease.

ā¢ To promote the health.
ā¢ To give as symptomatic treatment.
ā¢ To supply missing components.
5. CONTRAINDICATIONS
ā¢ Alteration in the gastro intestinal tract.
ā¢ Immediately after surgical procedure.
ā¢ Patient who are unable to swallow i.e. with
neuromuscular disorders.
ā¢ Patient has gastric suction in situ and
before some test or surgery.

ā¢ Patient who are unconscious, confused and
unwilling to swallow etc.
ā¢ When drugs are destroyed in stomach by
the action of digestive juices e.g. insulin.
7. ARTICLES REQUIRED
ā¢ Medication cardex.
ā¢ Medication tray.
ā¢ Medicine
ā¢ Measuring cup/dropper, plastic cup, and
spoon.
ā¢ Kidney tray/ paper bag
ā¢ Glass of water
ā¢ Towel or sponge cloth, cotton to dry
measuring cup.
8. PROCEDURE
ā¢ Identify patient by asking the patient to
state his name.
ā¢ Explain the purpose of medication.
ā¢ Assess for any contraindications.
ā¢ Allow patient to clarify doubts.
ā¢ Wash hands.

ā¢ Prepare one medication at a time.
ā¢ Verify medications instructions in cardex.
ā¢ Proceed from top to bottom of the cardex.

ā¢ Take appropriate medication.
ā¢ Assist patient to sitting position or side lying
position.
ā¢ For tablet and capsules pour the required
medication into cup. Do not touch with
fingers.

Preparation of Oral Medication
ļ¶Shake the bottle well before
administration.
ļ¶Remove the cap and place the upside
down.
ļ¶Hold the bottle with the label against the
palm.
ļ¶Hold the medication cup at the eye level to
fill it to desire level ,using the bottom of the
meniscus to align with the container scale

ā¢ Administer one drug at a time.
ā¢ Offer a glass of water with a drug.
ā¢ Ask if the patient wishes to hold medication
cup in hand before placing in mouth.
ā¢ If the patient is unable to hold the
medication cup in hand place cup to lip
and introduce each drug one at a time with
a spoon.

ā¢In case of buccal or sublingual
medicine instruct patient not to
swallow it.
ā¢Stay with the patient until each tablet is
swallowed.
ā¢Assist the patient in comfortable
position.
ā¢Dispose all the soiled supplies.
ā¢Replace articles and wash hands.

ā¢ Document the administration of drug.
ā¢ If a tablet falls to the floor, discard it and
repeat the preperation.
ā¢ ( all three checks and ten rights should
be performed)
