 General Description
The Nurse Home Visitor/Advocate providesprogramguidedpreventive healthandsocial
servicestoat riskpregnantwomenandchildrenbirthto2 yearsof age. The Nurse Home
Visitor/Advocate willutilize advancedskillstoassessindividual andfamilyneeds;screening
clientsforeligibility,providingcase management, offeringprogramoutlinededucational
models, andmakingreferralsforcommunityinterventionservicestopromote positivefamily
functioningandchildoutcomes.The Nurse Home Visitorsupervisorwillreview referralsfrom
communityagenciesandthe HealthDepartmentandassigncase managementinterventions
basedon level of serviceneeded.The Nurse Home Visitor/AdvocateandSupervisorwillwork
closelywithprivate providers,andothersprovidingfamilysupport/casemanagementprograms,
inthe healthdepartmentandothercommunityagencies.Servicescanbe providedinhospitals,
private physicians’officesandhome settings;designedtoenhance the continuumof services
available tothispopulation.The Nurse Home Visitor/Advocate will maintainasafe environment
and performjobresponsibilitiesinasafe manner.The goalsof the projectare to achieve family
outcomes asgood as or betterthanthe outcomeshave thathave beendemonstratedthrough
research,andalignswiththe goalsof the SurgeonGeneral andthe Office of PopulationAffairs.
 Essential DutiesandResponsibilities
 Demonstrate knowledgeof the principlesandpracticesof publichealth,maternal-child
nursing,andthe fundamentalsof childgrowthanddevelopment;
 Promote linkageswithotherhealthandhumanservicesonbehalfof the programor
client;
 Serve asliaisonwithothercommunityhealthandhumanservice providers;
 Maintaina positive workingrelationshipwithotherorganizationstoenhance the
referral processandthe clientassesstoservice;
 Demonstrate knowledgeof communityresourcesandassure NFPHome Visiting
Program islinkedtoa largersystemof communitybasedfamilyfocusedpreventionand
interventionservices;
 Participate inthe developmentandimplementationof acommunitywide triage system
for all familiesinneedresources;
 Assistindevelo0pingprojectguidelinesandpoliciesthatreflectproven,evaluatedand
effectivecommunitystrategies;
 Complete programforms,forbothNFPand EHS, as outlinedinpolicy andprocedure
manualskeepingclientrecordsup-to-date andaccurate;
 FollowNSOguidelinestoeducate andempowerhighriskfamiliestorequest
interventionservicesthatproduce positive familyoutcomesandbuildontheircapacity
to problemsolve,meetgoals,becomeself-sufficient,health,andpositive parents;
 Participate inongoingprojectmonitoring/evaluationandqualityimprovementactivities
to maintainprogrameffectiveness;
 Teach andpromote clientwellnessthroughearlyscreenings,healthcounselingand
education;
 Complete anddocumentappropriateinterventionsbasedonclientassessments;
 Provide projectinformation,educationandcounselingasneededinprivatehomes,
hospitals, physicianoffices,orotherarrangedlocationsthatmeetthe client’sneeds;
 Assistclientstoobtainneededservicesbylinkingthemwithresourcesformedical, drug
treatment, financial,developmental,orotherservices;
 Conductperiodicfamilyinterviewstoevaluate effectivenessof programinterventions;
 Participate inQA/PIactivities
 Performotherjobrelatedtasksasassigned.
3. Essential Qualifications
 Masters inNursingandone year of PublicHealthexperience preferred;orgraduation
froman accreditedschool of professionalnursingwithatleasttwoyearsof professional
nursingexperience,includingone yearof publichealthexperience.Currentlicenseor
temporary license topractice asa RegisteredNurse inNorthCarolinabythe NCBoard of
Nursing.
 Current,validNorthCarolinadriver’slicense isrequired.
 Maternal ChildHealthnursingexperience ispreferred.
 Spanishasan additional language ispreferred.
 Participationonagencycommitteesandincommunityactivitiesasaprogram
representative.
 CertificationinbasicCPR
 Conductparenttrainingas qualifiedandwhenneeded.
 Provide learningexperiencesfornursingstudentsand othercommunityagencies.

4. AmericanwithDisabilityActandWorkers’CompensationSpecifications
While performingthe dutiesof thisjob,the followingactsare requiredbythe employee:
 Work flexiblehourswithinagencyestablishedworkdaypoliciestomeetrequiredclient
visitsschedules;
 Home visitinat risksneighborhoods;
 Walk,stand,sit,lift,reach,push,andpull as needed;
 Liftand supportthe weightof 50 poundsinhandlingof patients,equipmentand
supplies;
 Conductgroup presentations;
 Possessintactsense of sight, hearing,touch,andfingerdexterity;
 Able toconcentrate,prioritize,setgoals,negotiate,andinteractwithindividualsand
familiesof differingethnicandsocioeconomicbackgrounds;
 Able torespondquicklytochanges inclientand/or home conditions,
 Have reliable transportation;
 Demonstrate nursingknowledge of the principlesof growthanddevelopment;
 Possessthe abilitytorespondtoage specificissuesanddatareflectiveof the client’s
status;
 Demonstrate knowledge andskillsnecessaryto provide care forthe followingage
groups:neonate,infant,child,adolescent,andolderadult;
 Able tooperate andmaintainthe integrityof medical equipmentandsupplies;
 Computerskillsincludingoffice,wordandexcel;
 Manage weatherexposureasconditionspresent;and
 Manage exposure tomoderate noiselevel.

Guilford Child Development Nurse-Family Partnership Title:Nurse Home Visitor Supervisor:Nurse-FamilyPartnershipDirector  General Description The Nurse Home Visitor/Advocate providesprogramguidedpreventive healthandsocial servicestoat riskpregnantwomenandchildrenbirthto2 yearsof age. The Nurse Home Visitor/Advocate willutilize advancedskillstoassessindividual andfamilyneeds;screening clientsforeligibility,providingcase management, offeringprogramoutlinededucational models, andmakingreferralsforcommunityinterventionservicestopromote positivefamily functioningandchildoutcomes.The Nurse Home Visitorsupervisorwillreview referralsfrom communityagenciesandthe HealthDepartmentandassigncase managementinterventions basedon level of serviceneeded.The Nurse Home Visitor/AdvocateandSupervisorwillwork closelywithprivate providers,andothersprovidingfamilysupport/casemanagementprograms, inthe healthdepartmentandothercommunityagencies.Servicescanbe providedinhospitals, private physicians’officesandhome settings;designedtoenhance the continuumof services available tothispopulation.The Nurse Home Visitor/Advocate will maintainasafe environment and performjobresponsibilitiesinasafe manner.The goalsof the projectare to achieve family outcomes asgood as or betterthanthe outcomeshave thathave beendemonstratedthrough research,andalignswiththe goalsof the SurgeonGeneral andthe Office of PopulationAffairs.  Essential DutiesandResponsibilities  Demonstrate knowledgeof the principlesandpracticesof publichealth,maternal-child nursing,andthe fundamentalsof childgrowthanddevelopment;  Promote linkageswithotherhealthandhumanservicesonbehalfof the programor client;  Serve asliaisonwithothercommunityhealthandhumanservice providers;  Maintaina positive workingrelationshipwithotherorganizationstoenhance the referral processandthe clientassesstoservice;  Demonstrate knowledgeof communityresourcesandassure NFPHome Visiting Program islinkedtoa largersystemof communitybasedfamilyfocusedpreventionand interventionservices;  Participate inthe developmentandimplementationof acommunitywide triage system for all familiesinneedresources;  Assistindevelo0pingprojectguidelinesandpoliciesthatreflectproven,evaluatedand effectivecommunitystrategies;  Complete programforms,forbothNFPand EHS, as outlinedinpolicy andprocedure manualskeepingclientrecordsup-to-date andaccurate;
  • 2.  FollowNSOguidelinestoeducate andempowerhighriskfamiliestorequest interventionservicesthatproduce positive familyoutcomesandbuildontheircapacity to problemsolve,meetgoals,becomeself-sufficient,health,andpositive parents;  Participate inongoingprojectmonitoring/evaluationandqualityimprovementactivities to maintainprogrameffectiveness;  Teach andpromote clientwellnessthroughearlyscreenings,healthcounselingand education;  Complete anddocumentappropriateinterventionsbasedonclientassessments;  Provide projectinformation,educationandcounselingasneededinprivatehomes, hospitals, physicianoffices,orotherarrangedlocationsthatmeetthe client’sneeds;  Assistclientstoobtainneededservicesbylinkingthemwithresourcesformedical, drug treatment, financial,developmental,orotherservices;  Conductperiodicfamilyinterviewstoevaluate effectivenessof programinterventions;  Participate inQA/PIactivities  Performotherjobrelatedtasksasassigned. 3. Essential Qualifications  Masters inNursingandone year of PublicHealthexperience preferred;orgraduation froman accreditedschool of professionalnursingwithatleasttwoyearsof professional nursingexperience,includingone yearof publichealthexperience.Currentlicenseor temporary license topractice asa RegisteredNurse inNorthCarolinabythe NCBoard of Nursing.  Current,validNorthCarolinadriver’slicense isrequired.  Maternal ChildHealthnursingexperience ispreferred.  Spanishasan additional language ispreferred.  Participationonagencycommitteesandincommunityactivitiesasaprogram representative.  CertificationinbasicCPR  Conductparenttrainingas qualifiedandwhenneeded.  Provide learningexperiencesfornursingstudentsand othercommunityagencies.  4. AmericanwithDisabilityActandWorkers’CompensationSpecifications While performingthe dutiesof thisjob,the followingactsare requiredbythe employee:  Work flexiblehourswithinagencyestablishedworkdaypoliciestomeetrequiredclient visitsschedules;  Home visitinat risksneighborhoods;  Walk,stand,sit,lift,reach,push,andpull as needed;  Liftand supportthe weightof 50 poundsinhandlingof patients,equipmentand supplies;
  • 3.  Conductgroup presentations;  Possessintactsense of sight, hearing,touch,andfingerdexterity;  Able toconcentrate,prioritize,setgoals,negotiate,andinteractwithindividualsand familiesof differingethnicandsocioeconomicbackgrounds;  Able torespondquicklytochanges inclientand/or home conditions,  Have reliable transportation;  Demonstrate nursingknowledge of the principlesof growthanddevelopment;  Possessthe abilitytorespondtoage specificissuesanddatareflectiveof the client’s status;  Demonstrate knowledge andskillsnecessaryto provide care forthe followingage groups:neonate,infant,child,adolescent,andolderadult;  Able tooperate andmaintainthe integrityof medical equipmentandsupplies;  Computerskillsincludingoffice,wordandexcel;  Manage weatherexposureasconditionspresent;and  Manage exposure tomoderate noiselevel.
