3. • Occurs most often in elderly women
• The fracture is usually caused by a fall on the
shoulder.
• Often the symptoms are minimal.
• Impacted fracture in most of cases.
4. • Impacted fracture :-
An impacted fracture occurs
when the broken ends of the
bone are jammed together by
the force of the injury
6. Classification
• Neer has classified these fractures into 4 types
• 4 parts :- Shaft, Head, Greater tuberosity and
Lesser tuberosity.
• Depending upon in how many parts the bone is
fractured ,he divided them into two to four part
fracture.
• For example, a fracture where the head ,the
greater tuberosity , the lesser tuberosity and the
shaft ,all have separated, it will be called a four
part fracture.

8. Treatment
• With moderate displacement
– Immobilization in triangular sling
• With wider displacement
1. Reduction under anesthesia and percutaneous
K-wire fixation
2. Open reduction and internal fixation
3. Arthroplasty

16. • Common in adults
• Caused by fall on shoulder
• Usually undisplaced and comminuted
• Sometime displaced due to muscle
attachment
17. Diagnosis
• History of fall
• Pain, swelling and
tenderness
• Pain during
movement
• AP view of x-ray
18. Treatment
• For undisplaced
– Rest in triangular sling
• For displaced and comminuted
– Shoulder immobilization in abduction
– Open reduction and internal fixation
