May 1979-August 1984 McKean County EMS, Emergency Medical Technician
Duties: Provide basic life support in the pre-hospital setting at the Emergency Medical
Technician level and aboard a ground ambulance. Patient assessment skills, CPR,
spinal & fracture immobilization, airway control and management, emergency childbirth,
overdose/poisoning management, vehicle extrication, etc.
September 1984-January 1989 Suncoast Medevac Helicopter Service Tampa, Fl.,
Flight Paramedic (part-time)
Duties: Provide rapid assessment, treatment, and prolonged air transportation to

medical and trauma patients, to both pediatric and adults/medical and trauma. Duties
included medication infusion management, advanced airway management, transport
ventilator management, chest decompression, pericardiocentesis Advanced life support
level of care aboard a Bell-Jet Long Ranger. Also responsible for assuring safe landing
zone operations. Completed over 700 patient flights

November 1984-August 1996 Hillsborough County EMS, Tampa, Fl.,
Senior Paramedic/Field Instructor (full-time)
Duties: Provided initial and continuing education and in-service training, as well as
supervision to paramedics in a busy (75,000 calls/year) professional and advanced
EMS system. Standard medications administered, as well as: dopamine, aminophylline,
dexamethasone, RSI medications. 12-lead EKG interpretation. Procedures authorized
included: surgical cricothyroidotomy, central lines, McSwain dart chest tube, intra-
cardiac injections, and pericardiocentesis.
September 1985-August 1996 Hillsborough Community College, Tampa, Fl.,
EMT & Paramedic Instructor (part-time)
Duties: Lead instructor for Paramedic & EMT courses. This EMS program is a two-
year paramedic program, which is competency based training. Program graduates
approximately 300 EMT’s a year, and 65 paramedics a year. I also served as EMS
Instructor for the Department of Defense-United States Central Command-MacDill Air
Force Base, Tampa, Florida.
January 1989-August 1996 Tampa General Hospital/Aeromed I
Aeromedical Specialist/Preceptor (part-time)
Duties: Served as an Aeromedical Specialist/Preceptor & Shift Supervisor for a 1,000
bed tertiary care, Level One adult & pediatric trauma center/teaching hospital.
Transporting patients from both scene & outlying hospitals/prolonged transport times
common. Patient population consisted of: adult & pediatric major trauma and major
medical cases, burns, myocardial infarctions, strokes, poisoning, spinal cord injuries,
submersion/diving incidents, and snake bites. Procedures/skills performed included:
Intra-Aortic Balloon Pump transport, surgical cricothyroidotomy, escharotomy, central
lines, emergency cesarean section, rapid sequence intubation, cut downs, & chest
tubes. Duties/responsibilities also included functioning as shift supervisor for total of
three hospital helicopters, as well as training newly hired paramedics & registered
nurses to function aboard an aircraft (BK-117/EC-145). Completed over 2,000 patient
flights.

January, 1991 EMS Commander: Superbowl XXII Tampa, Florida.
Duties: Responsible for all EMS operations, command and direction for Superbowl
XXII at Tampa Stadium which included over 100,000 in attendance. This Superbowl
was conducted during the first Gulf War. As such, worked directly with the Unites
States Department of Defense, CIA, Secret Service, FBI, CDC, State of Florida EMS
Office, Florida National Guard, and all area EMS agencies, hospitals, EMS helicopter
services.
May 1997- May 2000 Registered Nurse/House Supervisor Grafton City Hospital
Emergency Department
Duties: RN in a hospital Emergency Department. Functioned as the evening
charge nurse, supervising other emergency department RN’s and clinical support staff.
Patients seen where a wide variety as usually seen a hospital emergency department,
including adult and pediatric patients, medical and trauma. Also assisted with quality
assurance in the emergency department by chart reviews as well as staff education as
related to emergency medicine.
May 1999-May 2000 Logan General Hospital/Logan, WV. Physician Assistant
Medical Internship
Duties: Served as a physician assistant student at this community hospital for a year.
Completed rotations in the following areas: emergency medicine, pediatrics, surgery,
psychiatry, Ob/Gyn, internal medicine, & family practice.
June 2000- July 2004 West Virginia University Hospital, Department of Emergency
Medicine Chief, Physician Assistant (full-time)
Duties: Evaluate, diagnose and treat adult and pediatric patients in a 550-bed
teaching hospital Emergency Department. Complexity of patients varied from walk-in
patients, to critical medical and trauma patients. Emergency Department is an American
College of Surgeons verified Level One Trauma Center, Stroke Center, and Regional
Cardiac Care referral center. The facility also has an emergency medicine residency.
Other duties include establishing a Fast-Track from the ground/up at this tertiary care

Emergency Medicine Clinical Assistant Professor (full-time)
Duties: Provide education & training to emergency medicine residents and medical
students in an academic environment by way of Grand-Rounds and Core Content
lectures, as well as during clinical rotations in the emergency department. Lectures
follow the American Board of Emergency Medicine core content recommendations.
July 2002- June 2004 Alderson-Broaddus College Physician Assistant Master’s in
Emergency Medicine Program Clinical Assistant Professor (part-time)
Duties: Educate currently trained Physician Assistants in emergency medicine to the
post- graduate level by way of lectures and clinical experience. This Master’s program
is a two-year academic training program for Physician Assistants who desire to obtain
further advanced academic training and education in emergency medicine.

Duties: Solo practitioner at this critical access hospital. Responsible for evaluating,
diagnosing and treating adult & pediatric patients in this acute care/community hospital
Emergency Department. Complexity of patients varied from cardiac arrest, acute
myocardial infarction, blunt & penetrating trauma, overdoses/poisoning, emergency
childbirth, altered mental status, CVA’s, etc. Also, proficient at arranging transfer of the
above patients to specialty care centers. Emergency Department volume: 15,000/year.
February 2005-April 2011 Physician Assistant Cass Lake Hospital: Cass Lake, MN
(full-time)
Duties: Solo practitioner at this critical access hospital. Responsible for evaluating,
diagnosing and treating adult & pediatric patients in a community hospital Emergency
Department. Complexity of patients varied from minor medical disorders to critical
medical & trauma patients, blunt/penetrating trauma, myocardial infarctions, CVA’s,
poisoning/overdoses, spinal cord injuries, suturing, emergency child birth, shock
management, etc. Procedures and treatments include: RSI, oral intubation, central line
insertion, I/O insertion, X-Ray interpretation, chest tube insertion, resuscitation of adults

and pediatrics from shock, thrombolytic administration, surgical cric’s, fracture
management, Also, proficient at arranging transfer of the above patients to specialty
care centers. Urgent Care & Emergency Department volume: 24,000/year.

September 2009-12/2012 Physician Assistant Sanford Health: Bagley, MN (per
diem & then full-time: Full-time effective date= 4/2011)
Duties: Solo practitioner responsible for evaluating, diagnosing and treating adult &
pediatric patients in a community hospital Emergency Department, which is also a State
of Minnesota designated Level IV Trauma Facility. I was directly responsible for the
initial evaluation of all patients presenting to the Emergency Department, as well as for
their care during their entire stay in the ED. Complexity of patients varied from minor
injuries and aliments, to major medical and major trauma cases. Procedures and
treatments include: rapid sequence intubation, central line insertion, intraosseous line
placement, chest tube insertion, thrombolytic’s, suturing, fracture management, X-Ray &
CT initial interpretation, 12-Lead EKG interpretation, etc.
March 2011- Current Instructor/Educator Emergency Medical Staffing & Education
Duties: Instructor/Educator teaching ACLS, PALS, CALS (Comprehensive Advanced
Life Support), CPR, EKG interpretation, Airway control, IV techniques, and
pharmacology to physicians, nurses, paramedics. Courses conducted all throughout the
state of Minnesota, through the American Heart Association-Emergency Cardiovascular
Care Division, as well as the University of Minnesota.
January 2013- 2016 Instructor/Educator for University of Minnesota- Medical
School/Duluth, MN
Duties: Instructed medical students on the following: Orthoglass splint application,
fracture and sprain management. Also, instructed 4th
year medical students on
techniques of suturing, Dermabond application, wound irrigation, wound care.
November 2013- Current EMS Provider: Bemidji Youth Sports League-Bemidji, MN
Duties: Provide emergency medical assessment and care to injured or acutely ill sports
participants in youth sports activity. Also provided emergency medical assessment and
care to injured or acutely ill spectators at these events.
April 2016- Current Clinical Educator: Teleflex & Arrow
Duties: Conduct training and educational events in the classroom, as well as a health-
care setting environment for paramedics, RN’s, physicians, PA’s, etc on the use of
various advanced airway, chest decompression, and vascular access devices. Devices
such as: EZ intraosseous (I.O) device, central line access, chest decompression,
surgical airways, etc. I am engaged in teaching and guiding learners, evaluating

Duties included medication infusion management, advanced airway management, transport ventilator management, chest decompression, pericardiocentesis Advanced life support level of care aboard a Bell-Jet Long Ranger. Also responsible for assuring safe landing zone operations. Completed over 700 patient flights EXPERIENCE- CONTINUED November 1984-August 1996 Hillsborough County EMS, Tampa, Fl., Senior Paramedic/Field Instructor (full-time) Duties: Provided initial and continuing education and in-service training, as well as supervision to paramedics in a busy (75,000 calls/year) professional and advanced EMS system. Standard medications administered, as well as: dopamine, aminophylline, dexamethasone, RSI medications. 12-lead EKG interpretation. Procedures authorized included: surgical cricothyroidotomy, central lines, McSwain dart chest tube, intra- cardiac injections, and pericardiocentesis. September 1985-August 1996 Hillsborough Community College, Tampa, Fl., EMT & Paramedic Instructor (part-time) Duties: Lead instructor for Paramedic & EMT courses. This EMS program is a two- year paramedic program, which is competency based training. Program graduates approximately 300 EMT’s a year, and 65 paramedics a year. I also served as EMS Instructor for the Department of Defense-United States Central Command-MacDill Air Force Base, Tampa, Florida. January 1989-August 1996 Tampa General Hospital/Aeromed I Aeromedical Specialist/Preceptor (part-time) Duties: Served as an Aeromedical Specialist/Preceptor & Shift Supervisor for a 1,000 bed tertiary care, Level One adult & pediatric trauma center/teaching hospital. Transporting patients from both scene & outlying hospitals/prolonged transport times common. Patient population consisted of: adult & pediatric major trauma and major medical cases, burns, myocardial infarctions, strokes, poisoning, spinal cord injuries, submersion/diving incidents, and snake bites. Procedures/skills performed included: Intra-Aortic Balloon Pump transport, surgical cricothyroidotomy, escharotomy, central lines, emergency cesarean section, rapid sequence intubation, cut downs, & chest tubes. Duties/responsibilities also included functioning as shift supervisor for total of three hospital helicopters, as well as training newly hired paramedics & registered nurses to function aboard an aircraft (BK-117/EC-145).   • 6. EXPERIENCE- CONTINUED January, 1991 EMS Commander: Superbowl XXII Tampa, Florida. Duties: Responsible for all EMS operations, command and direction for Superbowl XXII at Tampa Stadium which included over 100,000 in attendance. This Superbowl was conducted during the first Gulf War. As such, worked directly with the Unites States Department of Defense, CIA, Secret Service, FBI, CDC, State of Florida EMS Office, Florida National Guard, and all area EMS agencies, hospitals, EMS helicopter services. May 1997- May 2000 Registered Nurse/House Supervisor Grafton City Hospital Emergency Department Duties: RN in a hospital Emergency Department. Functioned as the evening charge nurse, supervising other emergency department RN’s and clinical support staff. Patients seen where a wide variety as usually seen a hospital emergency department, including adult and pediatric patients, medical and trauma. Also assisted with quality assurance in the emergency department by chart reviews as well as staff education as related to emergency medicine. May 1999-May 2000 Logan General Hospital/Logan, WV. Physician Assistant Medical Internship Duties: Served as a physician assistant student at this community hospital for a year. Completed rotations in the following areas: emergency medicine, pediatrics, surgery, psychiatry, Ob/Gyn, internal medicine, & family practice. Lectures follow the American Board of Emergency Medicine core content recommendations. July 2002- June 2004 Alderson-Broaddus College Physician Assistant Master’s in Emergency Medicine Program Clinical Assistant Professor (part-time) Duties: Educate currently trained Physician Assistants in emergency medicine to the post- graduate level by way of lectures and clinical experience. This Master’s program is a two-year academic training program for Physician Assistants who desire to obtain further advanced academic training and education in emergency medicine. December 2002- 2005 Physician Assistant Eagle Butte Indian Health Service Hospital: Eagle Butte, SD (per diem) Duties: Solo practitioner at this critical access hospital. Responsible for evaluating, diagnosing and treating adult & pediatric patients in this acute care/community hospital Emergency Department. Complexity of patients varied from cardiac arrest, acute myocardial infarction, blunt & penetrating trauma, overdoses/poisoning, emergency childbirth, altered mental status, CVA’s, etc. Also, proficient at arranging transfer of the above patients to specialty care centers. Emergency Department volume: 15,000/year. February 2005-April 2011 Physician Assistant Cass Lake Hospital: Cass Lake, MN (full-time) Duties: Solo practitioner at this critical access hospital. Responsible for evaluating, diagnosing and treating adult & pediatric patients in a community hospital Emergency Department. Complexity of patients varied from minor medical disorders to critical medical & trauma patients, blunt/penetrating trauma, myocardial infarctions, CVA’s, poisoning/overdoses, spinal cord injuries, suturing, emergency child birth, shock management, etc. Procedures and treatments include: RSI, oral intubation, central line insertion, I/O insertion, X-Ray interpretation, chest tube insertion, resuscitation of adults David Pecora7

Urgent Care & Emergency Department volume: 24,000/year. EXPERIENCE- CONTINUED September 2009-12/2012 Physician Assistant Sanford Health: Bagley, MN (per diem & then full-time: Full-time effective date= 4/2011) Duties: Solo practitioner responsible for evaluating, diagnosing and treating adult & pediatric patients in a community hospital Emergency Department, which is also a State of Minnesota designated Level IV Trauma Facility. I was directly responsible for the initial evaluation of all patients presenting to the Emergency Department, as well as for their care during their entire stay in the ED. Complexity of patients varied from minor injuries and aliments, to major medical and major trauma cases. Procedures and treatments include: rapid sequence intubation, central line insertion, intraosseous line placement, chest tube insertion, thrombolytic’s, suturing, fracture management, X-Ray & CT initial interpretation, 12-Lead EKG interpretation, etc. March 2011- Current Instructor/Educator Emergency Medical Staffing & Education Duties: Instructor/Educator teaching ACLS, PALS, CALS (Comprehensive Advanced Life Support), CPR, EKG interpretation, Airway control, IV techniques, and pharmacology to physicians, nurses, paramedics. Courses conducted all throughout the state of Minnesota, through the American Heart Association-Emergency Cardiovascular Care Division, as well as the University of Minnesota. January 2013- 2016 Instructor/Educator for University of Minnesota- Medical School/Duluth, MN Duties: Instructed medical students on the following: Orthoglass splint application, fracture and sprain management. Also, instructed 4th year medical students on techniques of suturing, Dermabond application, wound irrigation, wound care.
