1. The Evolution of the
Modern Hair Transplant
Presentation prepared by Paul Green
DHI webinars 2018-19
2.
3. TECHNIQUE OVERVIEW
• There are now 3 MAIN TECHNIQUES
• 1993 - FUT is a highly surgical process where a strip of skin is cut from the back of head, to harvest
hairs.
• 2002 - FUE is a newer technology which was pioneered by DHI Group, in which follicles are extracted
one-by-one. This method has now regressed into the Motorised extraction technique now referred as
FUE
• 2010 - DHI is an EVOLVED version of FUE, all done by hand, utilising smaller high precision
instruments, minimal graft handling, higher survival rates and tighter protocols.
4. THE NEED TO EVOLVE
• To ENHANCE the patient journey
• To INCREASE THE DENSITY achieved to near full in a single session
• To REDUCE SCARING to undetectable levels
• To ACCELERATE THE HEALING process to less than a week
• To offer a transplant that is 100% NATURAL in appearance
5. WHO PERFORMS YOUR PROCEDURE
DHIFUE
Sessions are generally performed
by assistants or nurses with little
or no oversight from a Doctor.
Sessions performed end-to-end by
Medically trained and certified
Doctor only.
Doctors are present for only 30-
40% of the procedure.
Only the Doctors touch the patient
to perform 100% of the surgery.
6. HOW TRANSPLANTS HAVE EVOLVED
Extraction:
Follicles extracted from the strip of skin
removed from the scalp.
Harvesting:
Follicles are divided using a microscope
Recipient Holes:
Implantation area is prepared by making
holes a with a scalpel
Implantation:
Follicles are implanted in the slits and
holes by using tweezers
Extraction:
Follicles are extracted one by one with a
motorized extractor/drill.
Harvesting:
Follicles are then divided using a
microscope

Extraction:
Follicles are extracted one by one from
the donor
Harvesting:
The extracted follicles stored at a
specific temperature, in Hypothermosol
solution
Implantation:
With the exclusive patented tool “The
DHI Implanter”, each follicle is implanted
with right direction, depth and angle

7. EXTRACTION
DHIFUE
Motorized extraction using blunt punches.
High Trauma in Donor area. Non-uniform
extraction.
Uniform extraction using sharp, titanium
coated punches of 0.75 mm to 1.0 mm
diameter.
8. FOLLICLE CARE
DHIFUE
Follicles are kept in Chilled trays as
shown above, with a simple saline
solution.
DHI has Strict protocols on follicle care:
• Temp - <4 deg C.
• Exposure - <4 hrs
• Bio-Tissue Preservation:
Hypothermosol Solution
• Platelet Rich Plasma
12. FUE vs DHI HAIR TRANSPLANT
DHIFUE
1 Hour Post FUE
Frontal
correction
Low cost surgery
1/2 Hour Post
3500 Hairs FUE
Modern Clinic
½ hours post Surgery, 10500
hairs, 20 Hours of Surgery,
spread over two days
13. HAIRLINE RESULT DENSITY
COMPARISON
SINGLE SESSION DHISINGLE SESSION FUE
Angle, Direction and Density of implanted
hair is improper resulting in a very
unnatural hairline.
Total control over Angle, Direction and
Density of implanted hair helps get a
beautiful hairline.
25. COMPARISON OF DHI & FUE
• 100% of the procedure from start to finish is
performed by DHI doctors.
• Extraction of follicles is performed using
titanium-coated punches (up to 1mm
diameter)
• Each follicle is inserted directly with the DHI
patented implanter.
• Angle, direction and depth are totally
controlled, thus providing high graft survival
rate and a 100% natural and safe result.
• The implantation is a one-step process
Editor's Notes
There are 2 main techniques – FUT (Follicular Unit Transplant) and FUE (Follicular Unit Extraction), then DHI invented Micro FUE in 2010
FUT is a highly surgical process where a strip of skin is cut from the back of head, to harvest hairs. This was the start of the Modern Transplant Era Developed in 1993
FUE is a newer technology which was pioneered by DHI Group in 2002, in which follicles are extracted one-by-one without a lot of scarring.
DHI is an evolved micro version of FUE with high-precision instruments, least graft handling and tight protocols.
44% of all doctors still perform FUT, with most FUT surgeries being conducted in developing markets such as Asia.
Only 20% of all FUE surgeries are done by hand as 80% doctors prefer using motorized punches or robotics for extraction, this can lead to damaged over harvested donor areas and poor results.
Just 58% of patients globally were able to achieve their desired results.

FUE
Motorized Extractor and Punches
*Positives:
Lower fatigue levels for Doctors
*Negatives:
Heat generation
Extremely inflexible
Low ratio (Graft to Hair)
Blunt punch causing high transection loss
DHI
*Positives
Patented Titanium coated Punches and Implanters
Size of 0.8 to 1 mm
Super sharp edges and accurate wall thickness
Made in USA under world class quality control protocols
*Negatives
High cost of instruments
*Adherence to the strict protocol of single use instruments.
1. Extraction PhaseHair follicles are extracted one by one from the donor area using proprietary tools with diameter of 0.8mm to 1mm following strict DHI protocols
2. Follicle CareHair follicles are segregated based on number of hairs per graft and kept in Hypothermosol at 4 degrees temperature under strict protocols to ensure high graft survival rate.
3. Implantation PhaseHair follicles are implanted directly using a patented tool, the DHI Implanter, in a single one-step process which ensures the right angle, direction and depth at the same time.
