This case report describes a 50-year-old man who presented with sudden loss of consciousness due to a chronic subdural hematoma, which caused Kernohan-Woltman notch phenomenon (KWNP). KWNP occurs when a supratentorial brain lesion displaces brain tissue, compressing the contralateral cerebral peduncle against the tentorial edge and causing ipsilateral hemiparesis. The patient underwent burr hole drainage surgery and regained consciousness, but had residual left-sided hemiparesis and mydriasis, confirming KWNP. This represents the first reported case of chronic subdural hematoma causing KWNP in Ghana.
