 Provide safety support including, but not limited to: construction safety, shutdown safety, project safety,
pre-job safety reviews, safety meetings, and tailgates.
 Perform substance abuse tests and investigations in adherence with the company’s substance abuse
policies and procedures.
 The Safety Supervisor has authority implement, and execute the duties and responsibilities described
above.
 Assists with any task required by management with approval from direct supervisor.
 Task:
 Training new employees, either personally or with the assistance of site management, until he/she
demonstrates the ability to work in a safe and efficient manner;
 Monitor employees’ performance to ensure the use of safe work practices;
 Analyze work activities to identify potential hazards;
 Implement work procedures that incorporate control methods for identified hazards;
 Consult with the Vice President of EHS on injury management issues;
 Conduct scheduled and unscheduled inspections of the work areas;
 Responsibly manage on-site injury treatment and follow up as necessary with appropriate first aid;
 Investigate and report all incidents that occur in their respective areas of responsibility;
 Apply appropriate disciplinary procedures to address breaches of the EHS Policy and procedures;
 Conduct toolbox meetings;

For my fire department, I was assigned to the shift that was given the responsibility to design, develop and
implement rescue training to the entire department in the areas of trench, confined space, high angle and
collapse structure stabilization. We were to train the entire 80 member department through awareness,
operator and technician level training in order for our department to provide a regional response to a 16
county area surrounding the DFW Metroplex in the areas of trench, confined space and heavy rescue
building collapse. It was also our responsibility to provide ongoing continuing education in those rescue
disciplines to help maintain a constant state of readiness and to monitor skills while keeping the department
up to date on current rescue techniques. As an instructor, I also worked part-time for a private company that
provided training and stand-by rescue services in both the municipal and industrial setting. The training
provided included high angle, confined space and trench rescue as specialty training and other classes such
as industrial fall protection and safety in the industrial market.

After my time in the fire service, I started working full time for Nocona General Hospital in the EMS
Division. We provided hospital based 911 EMS services to one half of Montague County. Being that we
are hospital based and run out of the hospital, we also provide continuing care in the emergency room and
emergency and non emergency transfers to higher level of care facilities. Although NGH is a Level 4
Trauma Center, we have the capabilities of starting initial dosing for TPA Therapy for strokes and MIs. We
also have the capability of starting Antivenom for poisonous snake bites in the form of Cro-Fab. As an
added bonus, we are also trained as lab techs to assist the Lab in drawing all labs as well as performing
basic lab work in the form of CBC, BMP, CMP, PT, PTT/INR and cardiac enzymes.
In February of 2013, I took a job with Sendero Drilling Company and L and C Safety working as a Safety
Supervisor and Educator for a land based drilling company. We provide on-site Safety Inspections on the
rigs to insure that they meet current OSHA and Industry Safety Standards for operation. We also provide
training to new hires and employees in all areas of safety to meet OSHA Compliance Standards on Safety.
We performed Post Incident Investigations, RCA’s, and injury case management. At the end of 2014 going
into 2015,with a 2 person safety group, we managed and trained all field personnel, completely restructured
our New Hire Orientation and took it from a 8 hour course and turned it into a 16 hour course. All current
employees, managers and new hires were required to pass the orientation. We likewise reconfigured all
Safety Reporting forms with strong emphasis on JRA, STOP and Observation based behaviors. We were
able to take the company’s 2014 OSHA Recordable Rate of 2.52 (11 OSHA Recordables) down to a 2015
OSHA Recordable Rate of Zero. We went from 137 total incidents in 2014 to only 19 incidents in 2015.
