- 우리 몸의 세포, 조직, 그리고 기관은 여러 종류의 chemical messenger system에 의해서 유지된다.
⑥ Cytokine: 세포에서 extracellular fluid로 secrete되는 peptide로써, autocrine/paracrine/
endocrine으로 모두 작용가능하다. Helper cell에서 유리되는 interleukin과 lymphokine이 대
표적인 예로 다른 세포들과 면역세포에 작용한다. Adipocyte에서 유리되는 cytokine hormone
(e.g. leptin)의 경우 adipokine으로 불리운다.
- Neuroendocrine cell들은 hypothalamus에 존재하며 axon이 posterior pituitary gland 및
median eminence 에서 terminate하며, ADH (antidiuretic hormone), oxytoxin,
hypohysiotropic hormone 등을 분비하여 anterior pituitary hormone의 secretion을 조절한다.

endocrine hormone들은 각기 다른 역할을 수행한다.

① Polypeptide and protein hormones are stored in secretory vesicles until needed
- protein/polypeptide hormone은 ER에서 생성
되어서 preprohormone (biologically inactive)
형태로 생성된다. 이후 ER에서 더 작은 형태의
prohormone으로 cleaved되고, golgi appara
tus로 이동하며, secretory vesicle에 packing
되며, 여기에서 더 작고 biologically active한
hormone의 형태로 보관된다.
- vesicle은 cytoplasm에 있으며 대부분 cytoplasm
에 bound된 형태로 존재한다. 외부 자극에 의해
(대부분 Ca2+/cAMP의 증가를 매개로 하여서)
secretory vesicle과 membrane의 fusion이
일어나면서 extracellular fluid로 배출된다
(exocytosis).
- Ca2+의 증가는 depolarization of membrane
→ exocytosis. cAMP의 증가 → protein
kinase activation → secretion

② Steroid hormones are usually synthesized from cholesterol and not stored
- adrenal cortex (cortisol, aldosterone), ovaries (estrogen, progesterone), testes
(testosterone), placenta (estrogen, progesterone)
- Steroid hormone의 구조는 cholesterol과 매우 흡사하며, 대게는 cholesterol을 기반으로 만
들어진다.
- lipid soluble하고 세개의 cyclohexyl ring과 하나의 cyclopentyl ring이 하나의 구조에 합쳐진
모양을 하고 있다.
- steroid-producing endocrine cell에는 매우 적은 양만이 보관되어 있고, 자극에 의해 세포내
의 cytoplasm vacuole에 보관되어 있던 cholesterol ester에서 synthesis가 시작된다 (de
novo synthesis).
- Lipid soluble하기 때문에 만들어진 후, cell
membrane을 투과한다. 대부분의 경우 핵막을
통과하여 작용한다.

③ Amine hormones are derived from tyrosine
- Tyrosine에서 유래한 호르몬은 크게 thyroid hormone과 adrenal medullary hormone
(epinephrine, norepinephrine)
- Thyroid hormone; thyroid에서 만들어져서 thyroglobulin이라는 protein과 결합하여
thyroid gland의 follicle에 보관된다. Thyroglobulin과 분리되면서 free hormone이 blood
stream으로 release된다. 혈중에서 대부분의 thyroid hormone은 plasma protein인
thyroxine-binding globulin과 결합하고, 이 결합에 의해 thyroid hormone이 target tissue
로 서서히 유리되게 해준다.

③ Amine hormones are derived from tyrosine
- Epinephrine과 Norepinephrine은 adrenal medulla에서 만들어져서 유리된다.
- Catecholamine은 preformed vesicle에서 secretion전까지 store되어 있다가 exocytosis에
의해서 유리된다.
- 혈중으로 나간 이후에는 plasma에서 free form이나 conjugation form으로 존재한다.

① Onset of hormone secretion after a stimulus, and duration of action of hormones
- NE나 Epi와 같은 호르몬은 자극 후 수 초 이내에 secretion되고, 다시 수초-수분 동안 full action
을 취하게 된다.
- Thyroxine, growth hormone 등은 full effect를 보이기 위해 수 개월이 소요된다.
- 따라서 각기 다른 호르몬의 작용은 각기 다른 onset과 duration을 지닌다.
② Concentration of hormones in the circulating blood, and hormonal secretion rate
- 작용을 위해 필요한 각 호르몬의 양은 굉장히 낮다.
- 예를 들어 1 picogram/1 ml blood로도 작용을 나타낼 수 있다.
- 비슷하게 hormone의 secretion rate 역시 μg-mg/day 정도로 낮은 편이다.

ⓐ Negative feedback prevents overactivity of hormone systems
- 호르몬의 secretion은 대부분 negative feedback mechanism에 의해서 조절된다.
- 어떠한 자극이 호르몬을 release하게 되면, 그 호르몬 자체가 further release를 막는 역할을
하게 된다 → oversecretion을 막는 역할
- 혹은 target tissue의 activity가 어떠한 정도를 넘어가게 되면 endocrine gland에 feedback
signal을 주어서 further secretion을 막게 한다.
- 이처럼 호르몬은 negative feedback을 통해 일정 수준의 호르몬 양을 유지

ⓑ Surges of hormones can occur with positive feedback
- 가끔씩은 positive feedback이 일어나기도 한다.
- 즉, hormone의 biological action이 그 호르몬의 추가적인 release를 유도
- LH (luteinzing hormone) surge; ovulation 이전에 estrogen이 ant. pituitary gland를 자
극을 하면서 LH가 유리되고, 유리된 LH는 ovary를 자극해서 더 많은 양의 estrogen의 형성을
유도하고 이에 의해서 다시 더 많은 양의 LH가 생성 → 결국 LH가 ovulation에 필요한 농도에
도달할때까지 계속 증가

③ Feedback control of Hormone secretion
ⓒ Cyclical variations occur in hormone release
- 계절의 변화에 따라서, 노화의 정도에 따라서, daily cycle에 따라서, sleep에 따라서
hormone secretion이 변화할 수 있다.
- 예를 들어 growth hormone의 경우는 later stage of sleep에서 많이 분비된다.
④ Transport of hormones in the blood
- water-soluble hormone (peptide, catecholamine)의 경우 plasma에 녹아서 target tissue로
이동되고, capillary에서 diffuse하여서 interstitial fluid → target cell로 이동
- steroid/thyroid hormone의 경우는 plasma protein에 bound해서 이동된다. 전체의 10% 이
하만이 free form으로 돌아다닌다. 하지만 protein에 bound 된 경우 capillaries를 통해 확산이
잘 되지 않으며, plasma protein에 bound된 상태는 대게 biologically inactive한 상태이다. 또
한 대부분 protein에 붙어있는 호르몬들의 경우는 reservoir로 작용

⑤ Clearance of hormones from the blood
- 혈중 호르몬의 농도는 ⓐ rate of hormone secretion ⓑ removal of hormone from the
blood에 의해서 유지된다.
- 이러한 호르몬이 제거되는 rate를 측정하기 위해서는 ⓐ plasma에서 호르몬이 제가되는 rate
(ng/min) ⓑ plasma concentration of hormone (ng/ml plasma)를 알아야 한다.
- 즉, metabolic clearance rate = rate of disappearance hormone from the plasma /
concentration of hormone으로 구할 수 있다.
- 대부분은 radioactive substance가 tagging되어 있는 호르몬을 주사하고, 이 radioactive
hormone의 제거 속도를 측정함으로써 확인할 수 잇다.
- 호르몬의 제거는 다음과 같이 제거된다.
ⓐ metabolic destruction by the tissue
ⓑ binding with the tissue
ⓒ excretion by the liver into the bile
ⓓ excretion by kidney into the urine
21. 4. Mechanisms of action of hormones
① Hormone receptors and their activation
- 호르몬이 작용하기 위해서는 target 세포의 specific receptor에 binding해야한다.
- 호르몬 수용체는 큰 단백질으로 각각의 세포는 2000개에서 100,000개 정도의 receptor를 가진
다.
- 또한, 각각의 수용체는 하나의 호르몬에 대해서 굉장히 특이적이다.
- 호르몬에 대한 각기 다른 수용체들은 다음과 같이 분포한다.
ⓐ In or on the surface of the cell membrane; 대게 protein, peptide, catecholamine 호르몬
에 대한 수용체들
ⓑ In the cell cytoplasm; steroid 호르몬에 대한 수용체들
ⓒ In the cell nucleus; thyroid hormone에 대한 수용체는 핵내에 존재하며 chromosome과 직
접적인 association을 하는 것으로 추정된다.

① Hormone receptors and their activation
- target 세포에 존재하는 수용체의 숫자는 항상 일정하게 유지되지 않는다.
- 수용체 자신은 기능을 수행중에 inactivation되거나 destroyed되며, reactivation되거나 새로운
수용체로 대체되기도 한다.
ⓐ Down-regulation of receptor
- 호르몬의 농도가 증가하고, 이로 인해 target cell의 수용체에 binding되면 세포의 active
receptor의 숫자가 감소하게 된다
- 이는 receptor molecule의 inactivation이나 intracellular signaling molecule의
inactivation, 잠시 수용체를 세포내로 이동시킴으로써 호르몬이 작용하는 것을 억제하거나,
세포내로 들어간 수용체가 lysosome에 의해서 분해되거나, 수용체의 생산이 억제되기 때문
이다. → 어떠한 경우든 receptor down-regulation은 target 세포의 hormone에 대한 반응
을 감소시킨다.
ⓑ Up-regulation of receptor
- 일부 호르몬의 경우는 receptor와 intracellular
signaling protein의 up-regulation을 가져오기도
한다. 이 경우에는 target tissue가 hormone에
대해서 보다 sensitive해진다.

다는 역할을 수행한다.
- Ion channel은 대게 sodium, potassium, calcium에 대해서 permeable하다.

ⓑ G protein-liked hormone receptor
- 많은 호르몬들은 heterotrimeric GTP-binding protein (G-protein)과의 coupling을 통해서
target protein (enzyme, ion channel)의 기능을 조절하게 된다.
- 이러한 G-protein couple receptor (GPCR)은 7개의 transmembrane segment를 가지고
있다. GPCR의 구조 중 세포내부 부분이 G protein의 trimeric part (α, β, γ)와 결합한다.
- ligand가 extracellular part에 binding하면 conformational change → G-protein과
binding → cell membrane ion channel를 open/close 하거나 세포내의 enzyme의
activity를 change한다.

ⓑ G protein-liked hormone receptor
- Trimeric G protein은 guanosine nucleotide과 binding할 수 있는 기능에 따라서 이름이 명
명되었다.
- inactive state에서는 α,β,γ subunit은 GDP (guanosine diphosphate)와 α subsut과 bind
해서 존재한다.
- activation되면, conformational change가 일어나면서 GTP (guanosine triphosphate)
exchange가 일어난다. GDP가 GTP로 대체되는 과정은 α subunit이 β/γ subunit에서 떨어
져나와서 다른 intracellular signaling protein과 association하게 해준다; 이 protein은 ion
channel이나 intracellular enzyme (e.g. adenylyl cyclase, phospholipase)의 활성을 유도
한다.

ⓑ G protein-liked hormone receptor
- 이러한 signaling은 호르몬이 제거되고, α subunit에서 GTP가 GDP로 converting되면서
inactivation되면서 정지된다; 그러면 α subunit은 다시 β/γ subunit과 binding하여서
inactive membrane-bound trimeric G protein이 된다.
- 기능에 따라서 G protein은 stimulating G protein (Gs protein), inhibitory G protein (Gi
protein)으로 나뉘기도 한다.

- 다른 예는 호르몬에 binding한 receptor가 adenylyl cyclase로 작용하는 경우가 있다. 이 경
우에는 cAMP의 생성을 유도해서 여러 활성을 일으키는데, 호르몬 자체가 세포내 변화를 일
으키지는 않고 cAMP와 같은 second messenger에 의해 변화를 일으킨다.
- 예를 들어 atrial natriuretic peptide (ANP), cyclic guanosine monophosphate (GNP) 등
역시 second messenger로 작용한다.

- Adrenal/gonadal steroid hormone, thyroid hormone, retinoid hormone, vitamin D 등

- 위의 호르몬들의 경우는 lipid soluble해서 cell membrane을 통과하여 cytoplasm, nucleus
의 receptor와 interact한다.
- activated hormone-receptor complex는 DNA의 hormone response element 라 불리우
는 specific regulatory (promoter)와 결합하여 mRNA를 만드는 특정한 gene의
transcription을 activate
하거나 repress한다.
- 이에 따라 수분, 수시간, 혹은

- Target tissue를 자극하는데 adenylyl cyclase-cAMP를 사용하는 호르몬들 (Table 74-2).
- 호르몬이 수용체에 결합하면, G protein (Gs protein)이 수용체에 결합해서 adenylyl
cyclase-cAMP system을 작동시킨다 → ATP를 cAMP로 catalyze → cAMP-dependent
protein kinase의 활성화 → phosphorylation of specific protein reaction

ⓐ Adenylyl cyclase - cAMP second messenger system
- cAMP가 생성되면 여러가지 종류의 enzyme cascade를 활성화시킨다. 이러한 enzyme들의
활성화는 다른 enzyme을 또 활성화를 시키고, 이 enzyme이 또 다른 enzyme을 활성화시킨
다; 이러한 기전은 초기에 아주 적은 양의 cAMP로도 세포내의 반응을 증대시킬 수 있는 기전
으로 작용한다.
- 만일 호르몬이 Gi protein (inhibitory G protein)에 coupled receptor와 결합할 경우는 이
러한 cAMP의 생성을 억제하게 되어, 세포내의 활성을 억제하게 된다.

ⓑ Cell membrane phospholipid second messenger system
- 몇 호르몬들은 phospholipase C를 활성화시킬 수 있는 transmembrane receptor와 결합한
다 (Table 74-3). Phospholipase C는 cell membrane의 phospholipid (특히
phosphatidylinositol biphosphate (PIP2)를 catalyze해서 inositol triphosphate (IP3)와
diacylglycerol (DAG)으로 분해한다.
- IP3는 ER에서 calcium을 mobilize하여서 calcium이 second messenger로 작동할 수 있게
해준다.
- DAG은 protein kinase C (PKC)를 activate한다.또한 lipid portion of DAG은 arachidonic
acid여서 prostaglandin의 precursor가 되기도 한다.

ⓒ Calcium-Calmodulin second messenger system
- 또 다른 second messenger인 calcium은 세포내로 calcium이 들어오면서 작동하게 된다.
calcium의 세포내 이동은 calcium channel을 open시키는 membrane potential이 변화나
membrane receptor에 의해 진행된다.
- 세포내로 들어온 calcium은 calmodulin과 결합을 한다. calmodulin은 4개의 calcium
binding site를 가지며, 여러 protein kinase의 활성을 유도하거나 억제하는 역할을 한다.
calmodulin- dependent protein kinase는 여러 protein의 phosphorylation을 통해
활성을 조절한다.

④ Hormones that act mainly on the genetic machinery of the cell
ⓐ Steroid hormone increase protein synthesis
- adrenal cortex, ovaries, testes 등에서 유리되는 또다른 호르몬들의 주된 역할 중 하나는
target cell의 protein synthesis를 조절하는 것이다.
- 새롭게 생성된 protein들은 enzyme으로 작용하거나, protein transport에 관여하거나,
structural protein을 작용하게 된다.
- steroid의 기능은
㉠ 스테로이드 호르몬이 cell membrane을 투과하려 cytoplasm으로 이동한 뒤 specific
receptor protein과 결합
㉡ Combined receptor protein-hormone은 nucleus로 이동
㉢ Combination은 chromosome의 DNA strands의 특정부위에 결합하여서 transcription
process를 유도하여서 mRNA를 생성
㉣ mRNA는 cytoplasm으로 diffuse되어서 translation process를 통해 ribosome에 의한
새로운 protein의 생성

ⓑ Thyroid hormones increase gene transcription in the cell nucleus
- Thyroid hormone인 thryoxine과 triidothyronin은 핵내의 특정 유전자의 transcription을
유도한다. 이를 위해서는 thyroid hormone은 nucleus의 receptor protein과 직접적으로 결
합하고; 이 receptor가 activated transcription factor로 작용하게 된다.
- 이러한 thyroid hormone의 특징은
㉠ 많은 intracellular protein을 생성하게 해준다. 이 들 중 대부분은 enzyme으로써
intracellular metabolic activity를 증강시켜주는 역할을 한다.
㉡ intranuclear receptor에 결합한 후, thyroid hormone은 수일-수주까지 작동하게 된다.

- 혈중의 대부분의 호르몬들은 굉장히 적은 양으로 존재한다; 1 picogram/ml까지도 존재
- 일반적인 화학적 방법으로 이들의 농도를 측정하는 것은 어렵다.
- 따라서 민감도가 굉장히 높은 방사선동위원소를 이용한 측정법 (radioimmunoassay)이 많이 사용
된다.

① Enzyme-linked immunosorbent assay
- 호르몬에 대해 서로 다른 특이적인 부위에 결합하는 항체인 AB1과 AB2를 만든다.
+ AB2에 특이적으로 결합하는 항체 AB3에 substrate를 catalyze하여서 colored product로 만
드는 enzyme을 결합한다
→ 측정하려는 체액을 넣으면 AB1/AB2가 호르몬에 결합하고 여기에 AB3가 AB2에 결합한다.
이후 substrate를 넣어주면 enzyme에 의해 색깔을 띄는 product로 catalyze된다.
→ 체액 내의 호르몬의 양을 색을 측정하는 흡광계를 통해 측정한다.

① 호르몬 과잉의 이유
- 내분비 세포의 종양, 자가면역질환, 혹은 외부에서 많은 호르몬의 주입
- 종양의 경우 well differentiated 종양의 경우 호르몬을 분비할 수 있다.
- MEN (multiple endocrine neoplasia) syndrome; MEN1은 parathyroid, pancreatic,
pituitary tumor가 동시다발적으로 일어나며, MEN2의 경우 medullary thyroid carciona,
pheochromocytoma, hyperparathyroidism을 보인다.
- Autoimmune Grave’s disease는 TSH receptor을 자극하는 자가항체의 형성으로 인해 TSH와
비슷한 역할을 수행하여 호르몬의 과잉분비를 유도한다.
- Subacute thyroiditis의 경우 염증반응에 의해 갑상선 호르몬의 과도 분비를 유도한다.
- 이와 비슷하게 activating receptor mutation 역시 과도한 호르몬의 분비를 가져온다.

② 호르몬 부족의 이유
- 호르몬 부족의 이유는 내분비세포 및 조직의 파괴; 자가면역반응, 수술, 염증, 감염, infarction,
hemorrhage, tumor infiltration
- 예를 들어 자가면역반응에 의한 thyroid gland의 파괴 (Hashimoto’s thyroiditis)나 pancreatic
islet β cell의 파괴 (type 1 diabetes mellitus)
- 이외에도 enzyme defect나 developmental defects에 의해 제대로 호르몬 분비가 안 되는 경우
거나 호르몬 자체의 mutation에 의해 생성된 호르몬이 제 역할을 하지 않는 경우, 혹은 호르몬의
원료의 부족 (nutritional/vitamin deficiency)등에 의해 호르몬의 부족이 일어난다.

③ Hormone resistance
- 호르몬에 대한 저항성은 세포막 수용체, 핵수용체, receptor signal pathway의 defect에 의
해 호르몬에 대해서 target 세포가 반응을 잘 하지 않는 경우 → 따라서, 높은 호르몬의 농도
에도 불구하고 호르몬의 기능이 나타나지 않는다.
- 가장 유명한 예는 insulin resistance에 의한 type II diabetes나, 일부 비만 환자들에서 보이
는 leptin resistance 등이 있다.

④ Clinical evaluation of endocrine disorder
- 대부분의 gland들은 inaccessible하므로 physical examination은 호르몬의 excess나
deficiency와 palpable gland의 direct examination에 의한다 (e.g. thyroid gland).
- 따라서 환자의 presenting symptom과 review of system, family/social history, exposure to
medication이 중요하다.
- Laboratory finding은 quantification/assessment of hormone level과 dynamic를 파악하는
데 굉장히 중요하며, radiologic imaging (CT/MRI/thyroid scan/ultrasound) 등 역시 진단에
필요할 경우 시행한다.
