- Adrenal gland; 4g, kidney의 superior pole에 위치
- 각각의 gland는 adrenal medulla와 adrenal cortex로 구성되어 있다.
- Adrenal medulla
- gland의 20%정도
- sympathetic stimulation에 대해서 epinephrine과 norepinephrine을 분비 (ch 60).
- Adrenal cortex
- corticosteroid를 분비한다. steroid cholesterol에서 만들어지며 모두 비슷한 화학구조를 지니고
있다.

① Corticosteroid: Mineralocorticoids, glucocorticoids, androgens
- Adrenal cortex에서는 mineralocorticoid와 glucocorticoid와 함께 적은 양의 androgenic
hormone을 분비한다 (하지만 adrenal gland에서 분비되는 testosterone의 경우는
masculinizing effect 정도만을 가진다.)
ⓐ Mineralocorticoid
- electrolytes (mineral), 특히 sodium과 potassium의 이동에 관여
ⓑ Glucocorticoids
- blood glucose concentration에 영향을 미치기 때문에 glucocorticoid라는 이름
- protein과 fat metabolism에도 영향을 미친다.
- 30 종류 이상의 steroid가 adrenal cortex에서 분리되지만, 그 중 가장 중요한 두 가지 호르몬은
aldosterone (mineralocorticoid)와 cortisol (glucocorticoid)이다.

• Zona glomerulosa
- capsule 바로 아래에 위치한 세포층으로 adrenal cortex의 15%
- aldosterone synthase를 포함하고 있어서, aldosterone을 주로 분비
- 이 호르몬들의 분비는 extracellular fluid concentration of angiotensin II와 potassium에 의
해서 조절된다.
• Zona fasciculata
- middle and widest layer로 cortex의 75% 정도
- cortisol과 corticosterone을 분비, 적은 양의 adrenal
androgen을 분비한다.
- hypothalamic-pituitary axis의 ACTH에 의해 조절
• Zona reticularis
- cortex의 deep layer
- adrenal androgen인 dehydroepiandrosterone
(DHEA)와 androstenedione, 적은 양의 estrogen과

- ACTH는 이 세포들의 분비를 조절하지만, 다른
cortical androgen-stimulating hormone도 분비를
조절한다.

- 모든 종류의 steroid hormone은 cholesterol로 부터 생성된다.
- Adrenal cortex는 acetate로부터 적은 양의 cholesterol을 만들어 쓸 수 있기는 하지만 (de
novo), 대부분의 steroid 생성에 관여하는 cholesterol의 80% 이상은 plasma의 low-
density lipoprotein (LDL)에서 유래한다.
- LDLs은 cholesterol을 다량으로 가지고 있으며, adrenocortical cell membrane에서 coated
pits이라 불리우는 구조에 의해서 세포내로 endocytosis된다. 이후 endocytosis된 LDL-
containing vesicle은 lysosome과 fusion하여서 세포 내로 cholesterol을 유리하게 된다.
- Adrenal cell로 cholesterol의 이동은 feedback mechanism에 의한다;

① Adrenal cortex has three distinct layer
ⓔ Adrenocortical hormones are bound to plasma proteins
- cortisol의 90-95%는 cortisol-binding globulin/transcortin/albumin에 binding되어있다
→ plasma protein에 binding되어 있으므로 plasma에서 elimination되는 속도를 줄여준다
(cortisol의 경우는 60-90 시간 정도의 긴 half-life를 가진다.)
- aldosterone의 60%는 plasma protein에 binding되어서, 나머지 40%는 free form으로 존
재한다 (aldosterone의 half-life는 20분 정도로 짧다.)
- 이러한 plasma protein binding은 adrenal hormone의 reservoir 역할을 함으로써 조직에
균일하게 distribution할 수 있게 해준다.
ⓕ Adrenocortical hormones are metabolized in the liver
- Adrenal steroid는 liver에서 degradation되고, glucuronic acid/sulfate와 conjugate된다
→ hormone이 inactive해진다
- 이렇게 conjugation된 후, 25% 정도는 bile로 excreted된다. 나머지는 다시 circulation으로
들어가지만 plasma protein에 bound되지 않고, soluble해져서 kidney에서 filtration된 이
후에 urine으로 excreted된다.
- Liver 질환은 adrenocortical hormone의 inactivation rate를 줄이고, kidney 질환은
inactive conjugate의 excretion을 줄인다.

① Mineralocorticoid deficiency causes several renal NaCl wasting and hyperkalemia
- Mineralocorticoids의 주된 역할은 H2O retention, Na+/Cl- reabsorption, K+ excretion 증가
- Mineralocorticoid 부족의 경우 ECF의 K+ 농도의 급격한 증가, Na/Cl의 소실, ECF volume의 감
소, blood volume의 감소 → cardiac output 감소 → shock

② Aldosterone is the major mineralocorticoid secreted by the adrenals
- Aldosterone은 mineralocorticoid activity의 90% 정도를 담당한다. (cortisol은 나머지 10%
정도를 담당하지만, aldosterone의 mineralocorticoid activity가 3000배 정도 potent)
- Mineralocorticoid는 mineralocorticoid receptor에
binding해서 역할을 수행한다.
- cortisol 역시 mineralocorticoid receptor에
binding하지만, renal epithelial cell은 11β-
hydroxysteroid dehydrogenase type 2를
가지고 있어서 cortisol을 cortisone으로 전환.
변화된 cortisone은 mineralocorticoid
receptor에 binding하지 않으므로, 일반적으로
cortisol은 mineralocorticoid effect를 많이
가지지 않는다.
- 11β-hydroxysteroid dehydrogenase가 선천적
으로 모자란 경우 cortisol → cortisone이 일어나지
않기 때문에, cortisol에 의한 mineralocorticoid
effect를 보이게 된다; apparent mineralocorticoid
excess syndrome (AME); 혈중 aldosterone의
양이 적음에도 excessive aldosterone secretion에
의한 증상을 보인다.

ⓑ Excess Aldosterone increase ECF volume and arterial pressure but has only a small
effect on plasma sodium concentration
- Aldosterone은 kidney에서 sodium 분비를 감소시키지만, 정작 ECF에서 sodium 농도는 소
량 증가한다.
- 이는 sodium이 tubule에서 재흡수되면서 H2O 역시 osmotic absorption이 일어나며, 또한
ECF에서 sodium의 증가는 thirst를 유발하여서 water intake를 높이게 된다 → 따라서
sodium concentration의 소량 상승과 함께 ECF volume의 증가
- 또한, aldosterone이 powerful sodium-retaining
hormone이기는 하지만, 실제 retention되는 sodium의
양은 생각처럼 크지는 않다
- 이는 ECF volume 증가 → arterial pressure ↑ →
pressure natriuresis/pressure diuresis (신장에서
pressure에 의한 salt/water의 excretion) ↑;
aldosterone escape이라 불리운다.
- 하지만 aldosterone 증가에 의한 hypertension은 계속 지속
- Aldosterone이 zero → urine의 sodium loss →
ECF volume의 감소 → dehydration and low blood
volume → circulatory shock

③ Renal and Circulatory effects of aldosterone
ⓒ Excess aldosterone causes hypokalemia and muscle weakness; too little aldosterone
causes hyperkalemia and cardiac toxicity
- Aldosterone ↑ → excretion of K+ in urine, K+의 ECF에서 세포내 이동을 stimulate →
plasma potassium concentration의 감소
- Adrenal tumor의 경우 K+ 농도가 정상치인 4.5 mEq/ℓ에서 2 mEq/ℓ 까지도 감소한다;
hypokalemia
- Potassium concentration이 정상치의 1/2로 감소할 경우 severe muscle weakness가 일어
난다 (nerve와 muscle fiber membrane에서 electrical excitability의 alteration에 의해서)
- 반대로 aldosterone이 deficient한 경우에는 ECF의 potassium concentration은 증가한다.
정상치의 60-100% 이상 증가될 경우 cardiac toxicity가 일어난다 (weakness of heart
contraction, arrhythmia의 development)

ⓓ Excess Aldosterone increase tubular hydrogen ion secretion and cause alkalosis
- aldosterone은 cortical collecting tubule의 intercalated cell에서 sodium exchange과정
에서 hydrogen ion secretion을 유도해서 metabolic alkalosis를 유도한다.
④ Aldosterone stimulates Na/K transport in sweat gl., salivary gl., intestinal epith.
- aldosterone은 renal tubule에서와 똑같이 여러 exocrine gland에서 Na의 reabsorption, K의
excretion을 유도한다 → 이는 더운 환경에서 salt를 conserve하는데 있어서 중요
- 또한 aldosterone은 intestine에서 sodium absorption을 증가시켜서 stool로 sodium loss를
억제한다; 만일 aldosterone이 없을 경우 sodium absorption이 감소하고, 이로 인해
chloride, other anions, water의 absorption이 감소한다 → diarrhea와 salt loss

⑤ Cellular mechanism of aldosterone action
ⓐ Lipid solubility에 의해서 cellular membrane을 통과해서 tubular epithelial cell내로 이동
ⓑ Tubular cell의 cytoplasm에서 mineralocorticoid receptor (MR)과 binding; cortisol에 대해
서도 high affinity를 가지지만, 11β-hydroxysteroid dehydrogenase type 2가 cortisol을
cortisone으로 convert해서 MR에 binding하는 cortisol의 양은 적다.
ⓒ Aldosterone-receptor complex 혹은 complex의 product가 nucleus로 diffuse →
transcription factor로 작용 → mRNA ↑
ⓓ mRNA가 cytoplasm으로 diffuse back해서 translated → Na+/K+/hydrogen transport에 작
용하는 enzyme 및 membrane protein을 만든다. (e.g., Na/K triphosphatase, ENaC)
- ENaC (Epithelial Sodium channel)의 경우는
luminal membrane에 insertion해서 sodium
ion을 tubular lumen에서 cell로 빠르게 diffuse
하게 해준다.

⑥ Regulation of Aldosterone secretion
- Aldosterone secretion을 조절하는 인자들은 extracellular fluid electrolytes, extracellular
fluid volume, blood volume, arterial pressure (chapter 28, 29).
- Aldosterone의 regulation에서 중요한 인자들은
ⓐ Increased K+ concentration in ECF → aldosterone secretion ↑
ⓑ Increased angiotensin II in ECF → aldosterone secretion ↑
ⓒ Increased Na+ concentration in ECF → slightly aldosterone secretion ↓
ⓓ ACTH는 aldosterone secretion에서 필요하기는 하지만, secretion의 rate를 조절하지는 않
는다.
- Potassium ion concentration과 renin-angiotensin system이 가장 중요한 두 가지 조절기전

- Adrenalectomized animal은 mineralocorticoid가 보충되지 않으면 금새 죽게 된다; life-saving
hormone
- Adrenalectomized animal에 mineralocorticoid를 보충해주더라도 metabolism의 장애가 많아져
서, physical/mental stress를 극복하지 못하며,
호흡기의 약간 감염으로도 죽을 수 있다.
- Glucocorticoid 기능의 95% 정도는 cortisol
(hydrocortisone)에 의해서 이루어지지만, 다른
중요한 5%의 기능은 corticosterone에 의해서
이루어진다.
- 이러한 glucocorticoids의 주된 function은
•스트레스 상황에서 glucose를 주요기관 (뇌 등)에
공급하는 것

- 대부분의 inflammation의 시작은 면역세포의 intracellular lysosome이 rupture하면서
histamine을 비롯한 여러 inflammatory mediator들의 release하면서 시작된다. cortisol
은 이러한 lysosomal membrane의 rupture를 막아준다

- Inflammation이 진행이 된 상황이라고 하더라도 cortisol의 투여는 inflammation을 reduce
해준다.
- cortisol은 inflammation 촉진 인자들을 block하며, rate of healing을 촉진한다.
- Rate of healing의 증가는 mobilized된 amino acid에 의해서 damaged tissue의 repair가
빠르게 일어나는 것과 증가된 gluconeogenesis에 의한 glucose와 fatty acid mobilization
에 의해 증가한 fatty acid가 energy source로 사용되기 때문이다.

- antigen + antibody에 의한 allergic reaction 반응 자체는 cortisol에 영향을 받지 않지만,
cortisol에 의한 anti-inflammatory effect (특히 lysosomal membrane stabilization)은
histamine을 비롯한 inflammatory mediator 유리를 억제 → allergic reaction을 억제
㉡ Effects of blood cells and immunity in infectious diseases
- Cortisol은 eosinophil과 lymphocyte의 숫자를 줄인다; 반대로 감소된 eosinophil과
lymphocytes는 cortisol 분비 과다의 진단에 유용하게 쓰이기도 한다.
- 또한, cortisol의 과량 투여는 lymphoid tissue의 significant atrophy를 유도해서 T cell과
antibody 생성량이 줄어들게 된다 → pathogen에 대한 immunity의 감소 → 반복된 감염
에 의해서 사망에 이르기도 한다.
- 반대로 이러한 면역억제능을 이용해서 transplantation시 rejection을 줄이기 위해
cortisol이 사용되기도 한다.

- ACTH가 ant. pituitary gland에서 분비되는 과정에서 다른 호르몬들이 생성된다.
- 이는 ACTH synthesis를 일으키는 RNA를 만들기 위한 전사된 유전자가 초기에 POMC
(proopiomelanocortin)이라 불리우는 large protein의 형성을 일으키기 때문이다.
- POMC는 ACTH 뿐만 아니라 MSH (melanocyte-stimulating hormone), β-lipotropin, β-
endorphin의 전구물질이다.
- 정상적인 상황에서는 이러한 호르몬들은 신체에 큰 영향을 미치지 않지만, ACTH의 secretion이
높아진 상황 (addison’s disease등)에서는 POMC-derived hormone의 양이 많이 증가되게 된
다.

- POMC gene은 ant.pituitary gland의 corticotroph cell뿐만 아니라 hypothalamus의 arcuate
nucleus의 POMC neuron, dermis의 cell, lymphoid tissue 등에서도 transcribed된다.
- 이러한 세포들에서 POMC는 다양한 작은 peptide로 processed되며, 이는 tissue에 따라
expressing하는 enzyme에 따라 달라지게 된다.
- 예를 들어 pituitary corticotroph cell은 prophormone convertase 1 (PC1)만을 발현하여서,
N-terminal protein, joining protein, ACTH, β-lipotropin을 분비한다.
- Hypothalamus의 경우는 PC2 expression에 의해서 α-, β-, μ-MSH와 β-endorphin이 형성되
며 ACTH는 형성되지 않는다 (α-MSH+CLIP으로 분해); α-MSH는 식욕에 관여
- MSH는 melanocyte에서 melanin (black pigment)의 형성을 일으킨다. 일부 하등
동물의 경우 pituitary gl.의 intermediate lobe
(pars intermedia)를 가져서 여기에서 많은 양의
MSH를 생성 → 계절에 따른 털색깔의 변화
- ACTH의 경우는 MSH sequence를 가지고
있지만, melanocyte stimulating effect는
매우 약하지만, 인간의 경우 분비되는 MSH의
양이 매우 적어서 ACTH가 skin에서의 melanin의
양을 결정하는 요소가 된다.

- Adrenal androgen (대부분 dehydroepiandrosterone) 역시 adrenal cortex에서 분비된다. 특히

- Adrenal androgen의 경우 사람에서는
childhood에서 생성되어 male sex organ의

- Central obesity - lower part of body에서 upper abdominal region으로 fat의
deposition → buffalo torso, Edematous appearance of face “moon face”
- Increase blood glucose. protein catabolism에 의해서 liver를 제외하고 tissue protein의
감소 → muscle weakness, immune system suppression, osteoporosis, weakness of
bone
- Collagen의 감소로 인해서 subcutaneous tissue가 쉽게 tear → purplish striae
- Acne and hirsutism

① Cushing’s syndrome (Hyperadrenalism)
ⓑ Clinical manifestations
- 특히, fragility of skin (쉽게 멍이들거나 purplish striae), proximal myopathy (환자에게
손을 쓰지 않고 의자에서 일어나거나 계단을 오르도록 시킨다.)
- Ectopic ACTH syndrome의 경우 knuckle, scar, 관절부위 등에서 hyperpigmentation이
나타난다 (POMC-cleavage products, ACTH에 의해).

① Cushing’s syndrome (Hyperadrenalism)
ⓑ Clinical manifestations
- Upregulation of gluconeogenesis, lipolysis, protein catabolism과 함께 cortisol,
corticosterone의 mineralocorticoid effect에 의해서 hypokalemia, diastolic
hypertension, edema등이 생겨날 수 있다.
- 또한 glucocorticoid는 gonadotropin을 억제하여서 hypogonadism, amenorrhea를 일으
키며, hypothalamic-pituitary-
thyroid axis를 억제하여서 TSH 감소를
일으키기도 한다.
- 대부분의 sign and symptom은 non-
specific한 경우가 많다. obesity,
diabetes, diastolic hypertension,
hirsutism, depression 등
- 따라서, 주의깊게 환자의 증상들을
살펴봐야 한다.

- sufficient adrenocortical hormone을 produce할 수 없는 경우 (mineralocorticoids,
glucocorticoids 모두 감소하는 경향)
ⓐ Etiology
- 가장 많은 예는 adrenal cortex의 primary atrophy/injury에 의해서.
•Primary adrenal insufficiency를 Addison’s disease라고 부른다.
- Autoimmune adrenalitis
- Congenital adrenal hyperplasia (non-functioning adrenal tissue의 hyperplasia)
- Adrenal infection (tuberculosis)
- Adrenal infiltration (metastasis, lymphoma, amyloidosis)
- Adrenal hemorrhage
- Bilateral adrenalectomy
- Drug-induced
•Secondary adrenal insufficiency
- Pituitary tumor
- Mass lesion affecting hypothalamic-pituitary region
- Autoimmune hypophysitis
- Pituitary hemorrhage/irradiation/Pituitary infiltration

ⓑ Clinical manifestation
•Mineralocorticoid deficiency
Lack of aldosterone → tubular reabsorption of Na+/Cl-/Water ↓ K+ excretion ↓ →
extracellular volume ↓ hyponatremia, hyperkalemia, mild acidosis → plasma volume
↓ red blood cell concentration ↑ cardiac output ↓ blood pressure ↓ → shock
•Glucocorticoid deficiency
Lack of glucocorticoids → gluconeogenesis ↓ → blood glucose 유지를 하지 못함
Protein과 fat의 mobilization ↓ → metabolic function ↓ (sluggish energy mobilization)
→ muscle weakness, metabolic disturbance
면역세포의 정상 대사에 문제가 생기므로, 여러 stress에 대해 highly susceptible해진다;
mild respiration infection에도 생명이 위험할 수 있다.
•Melanin pigmentation
mucous membrane과 skin에서 melanin pigmentation. 특히 thin skin area (lips,
nipple)에서; 이는 cortisol 감소에 따른 negative feedback에 의해 ACTH 생성이 많아지고
이 과정에서 MSH도 증가하면서, MSH/ACTH에 의한 pigmentation

- Confirmation
Saline infusion test (2 ℓ의 saline injection), oral sodium loading
→ plasma aldosterone concentration 측정 → aldosterone 농도가 낮아지면 secondary
aldosteronism, 농도가 계속 높은 채면 primary aldosteronism
ⓓ Treatment
- Primary aldosteronism의 치료는 hyperplasia를 일으키는 adrenal 조직의 제거 및 tumor
의 surgical removal
- 또한 HTN를 조절하기 위해 mineralocorticoid receptor 억제제인 spironolactone을 투여

④ Adrenogenital syndrome
- adrenocortical tumor에 의한 androgen의 과다생성
- 여성의 경우 남성적인 특징을 보이게 되어서, beard의 growth, deeper voice, baldness,
masculine distribution of hair on body and pubis, growth of clitoris, deposition of
protein in skin, muscle
- Prepubertal male의 경우는 male sexual organ의 빠른 성장을 보인다 (Fig. 77-11).
성장한 어른의 경우는 testes에서 생성되는 testosterone의 효과에 masking된다.
- 치료는 tumor의 적출
