2. • Section 38 of the Work Health and Safety Act 2012 (SA) refers.
• As a responsible line supervisor / manager of our company you must
notify SafeWork SA of fatalities, serious injuries and illnesses, and
dangerous incidents as soon as you become aware of the incident.
• You may render emergency first aid, evacuate and take any precautions
to reduce any further risk before reporting.
• Discussion – penalties under the regulations can be severe for both the
company and the individual.ie Failure to notify fine of $50000 for
company $10000 for individual 2015 penalties

• Serious injury or illness of a person requiring immediate treatment as an
in-patient in a hospital; or immediate treatment.

medical treatment within 48 hours of exposure to a substance.
What is a notifiable incident?
4. Dangerous incident - any incident that exposes a worker or any other
person to a serious risk to a person's health or safety
Examples
an uncontrolled escape, spill or leak
an uncontrolled implosion, explosion or fire
uncontrolled escape of gas ,steam or pressure
electric shock
fall or release from a height
the collapse, overturning, failure or malfunction any plant
the collapse or partial collapse of a structure or excavation
the collapse or partial collapse of shoring
the inrush of water, mud or gas in workings
interruption of the main system of ventilation underground

5. • As supervisor / manager you must notify SafeWork SA of any work-
related injury that requires treatment as an inpatient in a hospital
immediately after the injury (disregarding any time taken for emergency
treatment or to get the person to hospital).

How to notify
7. • Inform your immediate manager as soon as possible after notifying
Safework SA. – any further notifications as required.
• Secure the scene – exclude people -make safe
• Ensure an accurate incident log is kept.
• Follow all incident reporting requirements in the company policy.
