talk to them, Myles would say that he found clues that his
college was just a front for an organized crime operation. His
sister said that she has often seen him mumbling quietly to
himself and at times, he seems to be talking to people who are
not there. He would emerge from his room and ask his family to
be quiet even when they are not making any noise.
Myles began talking about organized crime so often that his
father and sister brought him to the emergency room. On
examination, Myles is found to be a poorly groomed young man
who seems inattentive and preoccupied. His family said that
they had never known him to use drugs or alcohol, and his drug
screening results are negative.
Myles is now on the behavioral unit under voluntary admission.
This is his first psychiatric hospitalization. He is diagnosed
with paranoid schizophrenia.

Case Study
Ms. Tyra Brown is a 23-year-old African American female
who presents herself in the emergency department (ED) due to
“feeling depressed” for the past six months. Ms. Brown reports
that she no longer feels like doing any of the things she used to
enjoy, stating, “It all seems so pointless.” She can’t even bother
to eat most days and has lost weight recently. Although she has
no energy, she finds it difficult to both fall asleep and stay
asleep. She reports getting only 4-5 hours of sleep every night.
Even when client does manage to fall asleep, she never feels
rested when she wakes up in the morning. She reports having
difficulty at work as a computer support person because it is so
hard for her to concentrate. In the ED, Ms. Brown reports that
she finds herself crying almost every day and that she has
thought about suicide. Client further states, “I don’t know why I
have been feeling this way.”
Ms. Brown is admitted to the behavioral unit with the
diagnosis of major depressive disorder (MDD). This is her first
psychiatric hospitalization. Vital signs on admission are T:
98.2°F orally, P: 78, R: 20, and BP: 106/68. Client is 5’6” tall
with a weight of 110 lbs. Ms. Brown is started by her
psychiatrist on paroxetine (Paxil) PO once daily.

Case Study
A 22-year-old girl is admitted to the National Institute of
Mental Health accompanied by her mother with complaints of
history of repeated self-injurious behavior, aggressive behavior
6. toward others, feeling of emptiness and lack of interest in all
activities. These symptoms started six months ago after she
completed her higher secondary certificate (HSC) exam. Family
environment is described by the client as very chaotic, hostile &
distressful. She stated that growing up, her father enforced rules
that were harsh and rigid. Client now lives with her boyfriend
whom she always gets into a fight with.
On mental state examination, client is cooperative, coherent,
and kempt. She is generally dysphoric with a constricted affect
but denies current suicidal ideations. Client has multiple
superficial abrasions on her arms and admits to cutting herself
the day before when her boyfriend threatens to leave her. This
is the third psychiatric admission for the client. The last one
was last year after she hit a coworker over a dispute in their

borderline personality disorder.
