Generic Janumet Tablets (Zita-Met) are used along
with diet and exercise to improve glycemic control
(reduce blood sugar levels) in adults with type 2

2. Generic Janumet Tablets
Active Ingredient
The active pharmaceutical ingredients contained
in Generic Janumet (Zita-Met Tablets) are
Sitagliptin Phosphate and Metformin
Hydrochloride.
Zita-Met Tablets contain 50 mg of Sitagliptin
Phosphate and 500 mg of Metformin HCl .

5. Generic Janumet Tablets
Recommended Dosage
Generic Janumet tablets should be given
twice daily with meals, with gradual dose
escalation, to reduce the gastrointestinal (GI)
side effects due to Metformin.
Starting Adult Dose :
The usual starting dose of Generic Janumet tablets
is 50 mg Sitagliptin/500 mg Metformin Hydrochloride
twice daily .

Contraindications
Generic Janumet Tablets (Zita-Met) is
contraindicated in patients with the
following conditions:
➔Hypersensitivity to Sitagliptin Phosphate or
Metformin Hydrochloride or to any inactive
ingredient present in the medication.
➔Contraindicated in cases of Renal dysfunction,
e.g., serum creatinine 1.5 mg/dL or more in
males, 1.4 mg/dL or more in females or abnormal
creatinine clearance

Side Effects of Generic Janumet
The most commonly observed side
effects associated with the use of
Generic Janumet (Zita-Met Tablets)
were Diarrhoea, Headache, Nausea,
Upper respiratory tract infections,
Dyspepsia, Flatulence, Vomiting,
Gastrointestinal adverse reactions,
Hypoglycaemia on concomitant use
with other anti-diabetic drugs and
Allergic reactions.

Warnings And Precautions
➔Lactic acidosis is a rare, but serious metabolic
complication that can occur due to Metformin
accumulation during treatment with Zita-Met.
➔Temporarily discontinue Generic Janumet in patients
undergoing radiologic studies with intravascular
administration of iodinated contrast materials or any
surgical procedures necessitating restricted intake of food
or fluids.
➔There have been postmarketing reports of acute
pancreatitis, including fatal and non-fatal hemorrhagic or
necrotizing pancreatitis. If pancreatitis is suspected,
promptly discontinue Generic Janumet.
➔Vitamin B12 deficiency: Metformin may lower Vitamin
B12 levels. Measure hematologic parameters annually.

Warnings And Precautions
➔There have been postmarketing reports of acute renal failure,
sometimes requiring dialysis. Before initiating Generic Janumet
tablets (Zita-Met) and at least annually thereafter, assess renal
function and verify as normal.
➔When used with an insulin secretagogue (e.g., sulfonylurea) or
with insulin, a lower dose of the insulin secretagogue or insulin
may be required to reduce the risk of hypoglycemia.
➔There have been postmarketing reports of serious allergic and
hypersensitivity reactions in patients treated with sitagliptin (one
of the components of Generic Janumet), such as anaphylaxis,
angioedema, and exfoliative skin conditions including Stevens-
Johnson syndrome. In such cases, promptly stop Generic
Janumet, and initiate alternative treatment for diabetes.

Overdose Symptoms
In case of Sitagliptin Phosphate overdose, there
were no dose-related clinical adverse reactions
observed with Sitagliptin with doses of up to 600
mg per day for periods of up to 10 days and 400
mg per day for periods of up to 28 days.
Overdose of Metformin Hydrochloride has
occurred, including ingestion of amounts greater
than 50 grams. Hypoglycaemia was reported in
approximately 10% of cases, but no causal
association with Metformin Hydrochloride has been
established. Lactic acidosis has been reported in
approximately 32% of Metformin overdose cases.

Overdose Treatment
Sitagliptin is modestly dialysable. In clinical
studies, approximately 13.5% of the dose was
removed over a 3- to 4-hour haemodialysis
session. Prolonged haemodialysis may be
considered if clinically appropriate.
Metformin is dialysable with a clearance of up to
170 mL/min under good haemodynamic
conditions . Therefore, haemodialysis may be
useful for removal of accumulated medicine from
patients in whom Metformin overdosage
is suspected.
