conducting both air and noise monitoring surveys. Air and noise
monitoring sampling procedures are conducted in accordance with

Our professional staff has a significant level of experience
conducting personal protective equipment evaluations. We will
conduct evaluations of equipment and processes to determine
required PPE for employees working in and around specific
equipment and/or processes. An extensive evaluation is conducted
to determine potential hazards associated with a particular piece of
equipment or a particular process. If engineering controls cannot
eliminate the hazard, we will determine appropriates PPE for
affected personnel. The implementation of specific types of PPE,
for example respirators and hearing protection, require the

client’s specific needs. An internal audit program consists of
development of audit protocols used internally to verify
environmental, safety and health compliance. Internal audits are

buildings, land, or other structure to a third party. The lessee

Dominic L. Martin
With over 24 years of experience in this field, Mr. Dominic Martin have performed a
variety of projectsspecialties in Industrial Hygiene, Fire, Environmental, Occupational
Health, and Safety; this has afforded me the opportunity to enhance my skill set overall in
various aspects within my profession. Mr. Martin have coordinated and managed the
above programs and implemented Industrial Hygiene Programs, In Door Air Quality
Programs and Hazardous Waste Disposal Programs. Also, Mr. Martin has prepared
compliance recommendations regarding these functions for all levels of management at
both Northrop and Hughes Aircraft Companies and the military sector.
Mr. Martin’s military background consists of serving in support of OPERATION
DESERT STORM as an Industrial Hygienist at the Naval North Island facility. During
that time, Mr. Martin performed over sixty industrial hygiene surveys involving industrial
chemicals, total dust, noise and ventilation monitoring. Frequently, he inspected hospital
facilities for safety/ JCAHO compliance. In addition, he served as an Environmental
Health Officer for the 4th Marines Civil Affairs Group during OPERATION IRAQ
FREEDOM in Al Arbor Providence, Iraq. During this time, Mr. Martin evaluated
medical facilities and addressed public health related issues to help rebuild the intra-
structure within the country of Iraq. As part of his military career, he was awarded the

16. Naval and Marine Corp Achievement Medal, Global War on Terror Medal and The Iraq
Freedom Campaign Medal.
During his employment at Onyx Waste Services, he was in charge of Ford’s
Total Waste Management Program at their Dearborn Assembly facility. With this
responsibility, he supervised all Industrial Services and Environmental Waste disposal
programs. These programs were implemented and maintained as part of Ford’s Total
Waste Management Program designed to reduce all aspects of waste and produced a
surplus for the company by cost saving measures. Mr. Martin evaluated all aspects of
waste including water, electricity, solid waste, hazardous waste and heavy metals. He
recommended cost cutting measures including recalibrating water, electricity meters. In
addition, Mr. Martin implemented recycling programs for all solid waste, hazardous
waste, heavy metals and cardboards. As a result of these programs, he reduced disposal
cost and producing a surplus at Ford from vendor who purchased these products
At Team One Staffing, Mr. Martin evaluated hazardous waste involving
commercial and government contracts for proper disposal. In addition, he was involved in
the complete manifesting and documentation of these materials. Also, working for
several Environmental Staffing Agencies, he received experience working as an
Industrial Hygienist /Environmental Tech. His duties involved chemical and noise
exposure monitoring, hazardous waste lab packing, hazardous waste manifesting and
hazardous waste cleanups. Occasionally, he was assigned to perform phase one and two
evaluations at various EPA superfund and remediation sites for closure purposes.
In addition to his Master of Science Degree in Safety and Environmental
Management from West Virginia University, he has enhanced my professional
development by completing various continuing education courses at the University of
Southern California, Institute of Safety Management. Mr. Martin is a Register
Environmental Assessors Certification from the State Of California and a Hazwoper
Certification.

Jeffrey Dee Holler, CEP, CLM
Jeffrey Dee Holler, CEP, CLM is a senior level environmental, health, and safety (EHS)
professional with over 20 years of progressively responsible and successful experience
serving the private consulting, industrial, commercial, governmental regulatory, and
educational sectors. He holds an M.S. degree in Interdisciplinary Studies in Applied
Sciences from the University of North Texas, a B.S. degree in Natural Resource
Development from Michigan State University, and two nationally-recognized
professional certifications namely, Certified Environmental Professional (CEP) and
Certified Lake Manager (CLM). He has outstanding performance and pride in
accomplishment in the EHS policies and procedures, training objectives, and cost
reductions have always taken place under his direction and leadership.

Cliff Cooper
Clifford A Cooper CIH, has earned a High-Performance Building Design
Professional certification from the American Society of Heating, Refrigerating and
Air-Conditioning Engineers.
ASHRAE’s certification program identifies individuals who have demonstrated
they have the necessary training and tools for the design of high-performance
buildings that live up to their performance capability.
“An ASHRAE certification benefits building owners, employers and individuals,” says
ASHRAE President Bill Harrison. “Firms who employ ASHRAE-certified engineers
are better able to promote their services, and individuals who are certified approach
their design responsibilities with greater confidence. ASHRAE certification helps
designers demonstrate that they have mastered a certain body of knowledge in order to
stand out from their competition.” Currently ASHRAE has certified just 50 High
Performance Building Design Professionals in the U.S. and Canada.

Of
Garret Salomon
Garret Salomon is a health and safety associate for Martin & Associates International. As
a health and safety associate, Mr. Salomon was contracted out to the industrial hygiene
department of the United States Steel Corporation’s Great Lakes Works division located
outside of Detroit, MI. Mr. Salomon’s responsibilities include conducting facility surveys
for asbestos and lead based materials, collecting air samples thru out the plant and

20. assisting in the management of asbestos, lead, radiation and chemical hygiene programs.
Mr. Salomon is an accredited asbestos inspector in the State of Michigan and also has
completed a NIOSH 582 equivalency course. Prior to joining AM Health and Safety on
May 27, 2008, Mr. Salomon spent eight years with Nova Environmental of Ann Arbor,
MI. serving as an environmental consultant. During his time at Nova Environmental, Mr.
Salomon conducted asbestos, lead and hazardous material surveys and hazardous
material abatement project management for a variety of educational, commercial and
industrial clients. Mr. Salomon graduated from the University of Michigan-Dearborn in
2002 with a degree in liberal studies with focuses in environmental science, geology and
political science.

initiating or expanding existing P2 practices, you serve as a role model for others by demonstrating

Following the end of each year (December 31), you will have until April 1st to submit an annual
report, which summarizes progress toward your stated goals and also supplies the MBP3 with new
goals for the coming year. A web-based annual reporting form has been developed and can be
accessed on the MBP3 web site located at www.michigan.gov/deq, then Pollution Prevention and
Business Partnerships. This annual report form is available in hard copy or you can also take
advantage of our online reporting form that can be quickly filled out and submitted via e-mail.
