ENTRY refers to the age of child at time of completion of initial IFSP. If the answer to the question is YES , then ENTRY data should be completed for the child. The ENTRY child outcomes process should be initiated for the child. If the answer to the question is NO, then ENTRY data does NOT have to be completed for the child. The ENTRY child outcomes process should NOT be initiated for the child.
The Service Coordinator is the key personnel in the ENTRY data process. Initiation of the child outcomes ENTRY process should come from the Service Coordinator. They are responsible for helping the family understand the child outcomes process and making sure they understand their level of input and involvement in the process. Service Coordinators should use the ENTRY process as a time to remind families that the Part C Outcome Measurement process is to evaluate the BabyNet system, without focusing on any one child, family, or provider. EXIT refers to the time at the completion of the child’s final IFSP or discharge/transition out of the system. At EXIT there are two question that should be asked: Has child received at least 6 months of service? If the answer to the question is YES, then EXIT data should be completed for the child. The EXIT child outcomes process should be initiated for the child. If the answer to the question is NO, then EXIT data does NOT have to be completed for the child. The EXIT child outcomes process should NOT be initiated for the child. If a child has received at least 6 months of service, then the child had to have entered the system at 30 or less months of age. EXIT data should include those children who entered the system at 30 months or less and have received at least 6 continuous months of service.
Service Coordinators are the responsible for the child outcomes process at EXIT. COSFs should be disseminated only to service providers who are currently working with a child or have worked with a child in the past 90 days. ENTRY data should be re-inputted at EXIT along with new EXIT data at EXIT. The process should follow the same timeframe for closure as the general EXIT or transition process.
