I hereby give consent to Webb Productions for my appearance within the film trailer
‘Dilemma ‘and/or advertising through the use of magazine or poster coverage. I understand
that I am to receive no compensation for my part within the production or post production
and I give Webb Productions the right to the ownership of my appearance. I give the
producer the right to use my name, likeness and biographical material to publicize the film
trailer ‘Dilemma.’
The producer may:
1. Photograph me, record me, my voice or likeness for the purpose of the production –
whether by videotape, digitally or otherwise;
2. Gain the right of the recordings and make copies or edit those in a way that is
appropriate towards the film trailer.
3. Use my name and likeness for the purpose of promotion through the use of
recordings or photographs.
Furthermore, I understand that the final cut of the film trailer ‘Dilemma’ will remain the
property of the producer and there are no restrictions as to how many copies are produced
nor are there restrictions on the amount of times my name and likeness are used.
