• * Bij aanwijzingen voor maligniteit (onregelmatige of slecht afgrensbare tumor, een tumor vast aan de huid en/of onderlaag, schilfering of
eczeem van de tepel, huid- en/of tepelintrekking, regionale lymfeklierzwelling, non-puerperale mastitis):
• o directe verwijzing naar een chirurg of mammateam/-poli.
• * Bij lokale, palpabele afwijking en geen aanwijzing voor maligniteit:
• o mammografie (tenzij de afwijking verdwenen is in een andere fase van de cyclus);
• + indien verdacht: verwijzing chirurg of mammateam/-poli;
• + indien gunstig: controle na drie maanden; indien nog aanwezig of groter: alsnog verwijzing.
* Borsten voelen diffuus knobbelig aan: indien bij palpatie onzekerheid bij vrouwen > 30 jaar:
• o mammografie.
* De vrouw voelt een knobbeltje, de huisarts niet:
• o controle na twee weken; als de vrouw iets blijft voelen: alsnog mammografie; bij aanhoudende klachten verwijzing naar een
chirurg of mammateam/-poli.
• * Er is gelokaliseerde pijn/gevoeligheid in een borst:
• o controle na twee weken en bij aanhoudende klachten na drie maanden; indien de klachten persisteren: mammografie.
• * Bij diffuus pijnlijke/gevoelige borsten:
• o geen indicatie voor mammografie.
• * Bij éénzijdige tepeluitvloed, bruin of bloederig:
• o verwijzing naar chirurg of mammateam/-poli.
• * Bij één- of dubbelzijdige tepeluitvloed, helder of melkachtig:
• o geen indicatie voor mammografie of verwijzing.

8. Epidemiology: Male Cancer Statistics The estimated incidence of various cancer types and number of cancer deaths due to these cancers among men in the United States are shown in this table. Whereas, prostate cancer is the most common cancer type among males, representing 31% of all cancers, cancers of the lung and bronchus are the most common causes of cancer deaths among men, accounting for 31% of all cancer deaths.
As of 1995, among members of the European Union, breast cancer incidence and mortality tended to be higher in Northern European countries than in Mediterranean countries. For instance, both incidence and mortality from breast cancer are nearly twice as much in Denmark and The Netherlands as they are in Spain or Greece. 16. Breast Cancer: Signs and Symptoms at Presentation Although the use of mammography is increasing, more than 80% of all breast cancers are still diagnosed as a result of symptoms, most often a painless mass. However, as many as 10% of patients present with breast pain and no mass. Less common symptoms include nipple discharge, nipple erosion or ulceration, diffuse erythema of the breast, axillary adenopathy, and symptoms associated with distant metastases.
7. Breast Cancer: Risk Factors In addition to female gender, several risk factors for developing breast cancer have been identified in epidemiologic studies. However, approximately half of all cases of breast cancer occur in women with no identifiable risk factors beyond being female and aging.
In the Netherlands about 15% of the population is > 65 yr In your country this is about 5%, which means that your cancer incidence will be much lower.

40. Breast Cancer: Commonly Assessed Prognostic Factors A prognostic factor is defined as a biologic or clinical measurement associated with disease-free or overall survival in the absence of adjuvant systemic therapy . The most useful prognostic factor is the number of positive axillary lymph nodes based on a level I/II (or higher) axillary dissection and detailed histologic evaluation. Tumor size correlates with the number of involved nodes, but has independent prognostic significance.
6. Breast Cancer: Natural History Breast cancer displays considerable interpatient variability and has a slow growth rate relative to other cancers. The long preclinical period of breast cancer facilitates early detection and treatment. Patients who receive conventional treatment for metastatic disease have a median survival >2 years.
