1. URINARY TRACT INFECTION.
Urinary tract infection (UTI) is defined as the presence of microorganisms in the urine
that cannot be accounted for by contamination. The organisms have the potential to
invade the tissues of the UT and adjacent structures.

2. PATIENT DETAILS-
• NAME: XYZ
• AGE:50 years
• SEX: female
• DOA:16/08/18
• SPECIALITY:FMW

3. SUBJECTIVE DATA-
A 50 years old female patient was admitted to female medical ward with
complaints of decreased urine output since 3 months , pain in abdomen since 1
week, burning micturition since 3 days and fever since 3 days.

4. OBJECTIVE DATA-
CNS: conscious, oriented
CVS: S1,S2 heard no murmur
P/A: soft non tender no organomegaly
SPO2:98% at RA
GFR: 23.375
BP:160/90 mm Hg
PR:94bpm
Pallor +ve.

6. ASSESSMENT-
• DIAGNOSIS- UTI , CKD

8. INTERACTIONS-
• Cefotaxime has ADRs like nausea, vomiting, diarrhoea, etc
• Tab.Paracetamol has side effects like hepatotoxicity.
• Tab. Iron and folic acid has ADR like stomach upset, hard stools, change in
colour of stools.
• Tab. Calcium has ADR nausea, constipation, weakness.
• Inj.Pantoprazole has ADR like headache, stomach pain, dizziness
• Tab.Amlodipine has ADR like stomach pain, dizziness, flushing, tiredness.

9. PATIENT COUNSELLING-
LIFE STYLE MODIFICATIONS-
• Drink cranberry juice
• Increase Vitamin C intake
• Maintain good sexual hygiene
• Exercise regularly.
• Avoid excess salt intake.
• Hydrate, but don’t overdo it.
