Karen’s Corner: Medical Records Updates
ATTENDING PHYSICIANS:
After reviewing and signing a resident’s dictated H&P or Discharge
Summary:
If the attending physician agrees and signs off on dictation, he or she
may forward to the resident for a signature. If the attending does not
agree and/or wants changes, he or she should refuse for review to the
HIM inbox, who will in turn contact the resident.
RESIDENTS
If a resident does not specify an attending’s name when dictating
and the document populates to the resident’s inbox, the resident
should send it to the attending for a signature. Do not sign before the
attending.
Any corrections will be made by the resident and will be added as an
“addendum” or dictated as an addendum.
The document will then be forwarded to the attending or sent to the
HIM inbox for reassignment and for the attending to sign off.
