could be!
Of course, it’s usual for optimism to dominate your thoughts at the start of a new year - there’s
a sense that anything is possible, and this wave of positivity provides the perfect conditions for
starting or extending a family.With that in mind, we look at four reasons for fertility optimism
in this issue, as well as presenting a host of other features which, we hope, will energise you for
your journey ahead.
We’ve got an update on our Fertility Journeys, pieces on fitness, clinics and the work/life
balance, plus we’ve been granted insider access to find out how one of the specialists at the
University College London Hospital operates at the front line of fertility science.

The Fertility Glow Programme begins with a
comprehensive individual assessment, guiding
each couple through the eight-week step-by-
step journey, where they will learn to transform
mind, body and spirit. From the comfort of
their own homes, Fertility Glow participants
will receive weekly videos teaching them about
nutrition, how to deal with stress, and lifestyle

At IVF Spain we are proud to announce that,
once again, we will be part of the exciting
project Fertility Journeys.This has been a
hugely original, inspirational, global project
that we have endorsed fully, and to sign
up for a third year is a privilege.After all,
we know how important it is for people
with fertility problems to fulfil their dream
of becoming parents. Considering that
treatment is provided free of charge for one
lucky couple, we hope that the economic
factors which control so much of our lives
should not always be a barrier to starting or
extending a family.
So for the third year, and together with the
support of the IVF Spain Foundation, a new
family will have the opportunity to start an
egg donation fertility treatment working with
the medical team of Dr Aizpurua.The IVF
Spain Foundation is a non-profit organisation
established by the IVF Spain fertility clinic
in 2012.The foundation focuses its efforts
on two essential aspects: supporting families
through the ‘Amadrina’ (sponsoring) social
programme, and committing to research,
training and teaching. Under the chairmanship
of Dr Jon Aizpurua, the multidisciplinary team –
committed to both causes – collaborates on a
daily basis for the IVF Spain Foundation to play
a significant role with families being helped.
The couple we have been supporting through
the journey,Anita and Simon, recently travelled
to Spain in order to make their latest visit to the
IVF Spain fertility clinic.They are very excited
about the treatment and are looking forward to
a happy conclusion – namely being able to form
the family they have always dreamed of.
They were due to come to IVF Spain on December
28 – when this piece was going to press – for an
echography and then on January 5, 2016 for their
embryo transfer.The doctors and the entire team

have put all their effort and hope into achieving
successful implantation and, of course, fulfilling
Anita and Simon’s dream.
IVF Spain pays special attention to providing
comfortable patient care. People sometimes
think that begins and ends with specialists, but
the reality is the attention to detail goes right
the way down to furniture distribution, the
colours used in each room and other elements
of decoration. Everything has been chosen
carefully following the Feng Shui principle
in order to make couples feel as settled and
relaxed as possible.We complement this with
holistic treatments such as acupuncture. These
kinds of treatments have been scientifically
demonstrated to improve responses to fertility
medication and, ultimately, increase pregnancy
rates. After all, a comfortable state of mind
promotes blood flow and relaxes patients
before and after treatment, thus significantly
reducing stress.

But our attention to detail goes even
further.The therapeutic, technological and
biotechnological equipment found at IVF Spain
is exceptional compared to most Spanish clinics,
and as a result the clinic ranks among the most
advanced in Europe.We have a flow cytometer
for semen analysis, genetic tests to analyse the
endometrial receptivity, preconception genetic
testing to avoid monogenic diseases, protamine
tests to improve male fertility, specific genetic
screening protocols that improve the chances of
pregnancy, technologies to predict the viability
of embryos and many other technologies that
ensure our visitors are in the best and most
experienced hands.
IVF Spain devotes all its effort to obtaining and
retaining the best pregnancy rates and does not
hesitate when it comes to improving progress
and scientific knowledge.This line of action
has meant the consolidation of IVF Spain as a
globally regarded reference point in research
applied to assisted reproduction, as evidenced by
the recognition and the impact their scientific
work has had throughout 2015.
During the annual meeting of the American
Society of Reproductive Medicine – one of the
fertility world’s most important conferences,
where the latest developments in assisted
reproduction are disclosed – IVF Spain attracted
great interest with two revealing scientific
studies. Each study has demonstrated that the
use of the growth hormone on patients with
poor prognosis improves the implantation rate,
based on the use of a protamine ratio as a new
marker for male infertility.
Another important event for IVF Spain has been
its recent participation in the most important
national meeting on embryology organised by
the Association for the Study of Reproductive
Biology (ASEBIR), where the reproduction clinic
has been recognised for excellence in scientific
innovation by the Scientific and Organising
Committee. Out of the 11 presented studies
two important procedures were awarded the
highest honour:
EMBRYOBANKINGWITHPGS
Thanks to this technique embryos are generated
and accumulated after several cycles (usually
two to four) and ultimately analysed using
the Preimplantation Genetic Screening (PGS).
Its function is to detect both numerical and
structural chromosomal alterations with the
aim of transferring embryos with a normal
chromosomal content.The chromosomal
alterations increase with age, hence it is advisable
to apply the PGS to patients older than 38 years.
SPERMVITRIFICATION
Unlike traditional freezing, through this process
we achieve an improvement in survival of the
semen sample after cryopreservation.This
improvement allows patients to use the sample
obtained during the first visit.This way it is
possible to achieve better results and, hence, it
prevents the male partner from travelling the day
of the embryo transfer.
The aim of focusing on innovation and
technological advances in fertility lies in the need
to offer patients the highest pregnancy rates –
70% to 85% depending on the type of treatment
and placing the clinic above the Spanish average
according to the data provided by the SEF
(Spanish Fertility Society) – as well as providing
a satisfactory solution to the most complex and
difficult cases.
Repeated miscarriages, implantation failure
and male fertility factors are the primary
causes of not achieving pregnancy. Detection
is as important as applying an appropriate
treatment for each patient, hence IVF Spain has
a mechanism for an early detection of factors
that constitute the cause for not achieving a
successful pregnancy.
In short, in most cases, having a multidisciplinary
team at our disposal, capable of implementing
the advances in reproductive medicine, makes
a difference when trying to achieve the desired
pregnancy.

Dr Godtlibsen completed her MD in 2002 and
has since worked at NHS equivalent hospitals in
Bergen, Stavanger and Haugesund specialising
in obstetrics and gynecology. She joined the

Klinikk Hausken is really excited in joining
this year’s Fertility Journeys project. Our
team is ready to help the chosen couple
on their road to parenthood through the
use of IVF treatment. Medical Director Dr
Jon Hausken and his dedicated team will
guide the selected couple throughout their
treatment cycle.
The clinic will cover all costs in connection
with the IVF treatment as well as all travel
expenses and accommodation. If donor
sperm is needed Klinikk Hausken will also
cover these expenses. In the event of
surplus embryos the clinic will cryo preserve
these at no extra cost.And if the couple
need to use the cryo preserved embryos
due to a negative result on the fresh cycle,
Klinikk Hausken are similarly prepared to
cover any cost connected to this cycle.
The outline above also includes travel and
accommodation expenses.
If the couple gives birth on the fresh cycle
they are free to use the cryo preserved
embryos for sibling treatment at their
own expense.The embryos will be cryo
preserved for five years according to
Norwegian Law.

Time-Lapse imaging technology.
We were the first clinic in Norway, as well as
one of the pioneers in Europe, to successfully
implement this novel concept.
Upon receiving its EmbryoScope, Klinikk
Hausken, led by IVF Laboratory Director
Shabana Sayed, began a series of in-house
trials to confirm its safety and efficacy.This
resulted in an algorithm which has allowed us
to achieve a relative increase in live birth rates
per embryo transferred by a remarkable 24%. It
uses, in addition to the traditional morphology,
deselection parameters and kinetic behavior of
embryos to select the most viable embryo.
The benefits of this technology has resulted
in Klinikk Hausken implementing the use of

Following on from the last issue Christina has
been matched with a recipient and started
another egg sharing IVF treatment.After
going through the data obtained from her
last stimulation cycle, I have modified her
stimulation protocol slightly; hopefully it will
help to achieve a better outcome.
At Lister we routinely invite couples to come back
for a follow-up appointment after a failed cycle.
We will use the information that we obtained
from their last failed cycle to tailor a better
stimulation protocol for a future treatment cycle.
In Christina’s case I agreed that she should have an
endometrium scratch to enhance implantation.
Christina started her treatment cycle in
November – she had 13 days of injection for
stimulation and during that stimulation period
she attended the Lister clinic five times to
monitor her follicular growth, and she was
subsequently ready for egg collection in mid-
November. Her response to the stimulation
in this cycle was very good: she had more
follicles than in her previous cycle.At Lister
we routinely monitor the patient’s oestrogen,
LH and progesterone levels during stimulation
cycles.We would only proceed to do vaginal egg
collection when the blood test levels are at the
optimum level with satisfactory follicular size.
After 13 days of stimulation Christina’s
oestrogen levels were very good and had
a good number of follicles. However, her
progesterone levels were elevated. From our
data and medical publications it shows that if
the progesterone levels are elevated before egg
collection the chances of achieving a pregnancy
are significantly lower.This is most likely due
to the endometrium maturation being out
of synchronisation with the implantation of
embryo.Therefore I advised Christina to proceed
with egg collection but to freeze all the embryos
at two pronuclear stage, instead of having
fresh embryo transfer. From our experience
of performing frozen embryo transfer in this
situation the chances of achieving a pregnancy
is as good as when having fresh embryo transfer.
Christina produced 13 eggs – she donated
six eggs and kept the rest for her own use.
ICSI was performed on six eggs, five achieved
fertilisation and four embryos were frozen at
the two pronuclear stage. The egg collection
was uneventful and Christina had a speedy
recovery from the procedure.
Christina was understandably upset that she did
not have a fresh embryo transfer. I invited her to
come for a follow-up appointment to discuss the
future plan for her frozen embryo transfer which
will be done on her natural cycle. Christina is keen
to get on with the treatment and I agreed that
we would try to have this done in December… so,
by the time you read this.
Christina called the nurses at the beginning of
December when her period started – she had
a baseline scan to assess her lining and her
ovaries in view of planning for frozen embryo
transfer in this natural cycle.The scan showed
a satisfactory endometrium but her ovaries
were still quite swollen from her egg collection.
Christina was sent home and would go on to
book another scan on day 10 of her cycle.The
scan on day 10 still showed that her lining has
not developed yet. I suggested that we continue
to monitor her cycle and hopefully we can do
the frozen embryo transfer in this cycle; if not
we will do it in her next cycle.
Christina has four embryos in storage which
appear to be good quality embryos and I think
she has a good chance to achieve a positive
outcome for her forthcoming frozen embryo
transfer treatment. If that is not the case, I will
continue to work with the couple to help them
achieve their goal.

We are now about to embark on our second
round of IVF – we have been matched
very quickly and cannot wait to get things
started again!
As we have already gone through the procedure
once before we feel more confident as we
know what to expect.
After things didn’t work out the first time, we
had our follow-up appointment with Dr Thum
which we found very useful. He was able to
look at our last cycle and modify this new one
to suit our needs better as there was data to
look at. Upon looking at our last cycle Dr Thum
made changes which will hopefully give us the
positive outcome we so desperately want.
Dr Thum increased our dose of medicated
injections as he now knows how I respond to
them without overstimulating me, and also
said we should have the endometrium scratch
to help with implantation.When we left we
felt reassured that Dr Thum was making all the
changes he could to help us this time round.
The first step into our IVF cycle was to
have the scratch performed. This was quite
uncomfortable but only lasted for a few
seconds, and it was worth all the pain as it’s
another step closer to making our dream come
true. Once this was done we were called in to
collect our bag of medication and start the
injections the next day.
As we have done the process before the
injections felt okay and quite normal. To be
honest, it becomes part of a nightly routine.We
had our routine scans throughout to check that
the follicles were growing nicely, which they
were. After 13 days stimulating we were ready
for egg collection; we couldn’t believe how
quickly it came around and that this was really
happening again. For some reason we felt quite
relaxed through the whole process this time; I
think it’s because we knew what to expect at
each scan.
On the day before we were booked in for
egg collection we were called by Dr Thum
who advised that everything looked great.
We had more follicles this time, though
unfortunately my progesterone level was
slightly raised which meant that while we could
go ahead with the scheduled egg collection
the transfer would have to be postponed as
my chances would have been significantly
lower if we were to proceed straight away. On
that note Dr Thum did the best thing for us
and decided we do a ‘freeze all’ cycle so my
eggs would be protected and we could still use
them, albeit it at a later date.
I was a bit upset but realised this was the best
thing for us to do and was happy DrThum checked
the levels so our eggs would not be wasted.
Egg collection went well with 13 eggs
collected – we gave six away and kept seven
for ourselves.We were really happy that we
had more this time. I had no issues with the
procedure and went home a few hours later.
We had the call the following day advising
us that four were fertilised with ICSI and
were frozen for us the same day. This was
an amazing achievement for us: during the
last cycle, only one had fertilised, so you can
imagine how relieved and happy we were.
I wanted to move on to the next step as quickly
as possible. I felt a bit empty and wanted my
eggs back where they belonged so Dr Thum
agreed that we would start on my next period.
I was called in to go through the procedure as
we haven’t done a frozen transfer before. It all
seemed quite straightforward with no daily
injections involved and a few scans to monitor
my lining as this was a natural frozen transfer.
When my period started I went to Lister for
my baseline scan to see how everything was
looking. It was okay but my ovaries were still
quite inflamed from the egg collection and a
few bruises were seen, so I was told to come
back to see if they reduced as we really wanted
to go ahead whilst not risking losing our
precious embryos.When we went back for our
day 10 scan it still showed large ovaries and my
lining had not thickened enough to continue
with the frozen embryo transfer. Again, this
was quite upsetting but you cannot rush these
things – I have realised that now. The body
needs time to heal and my embryos need to be
put back into the best environment possible.
We really want this to work this time round so
to wait a few more weeks to my next period is
the best thing for them.
Our journey continues but we are so close and
we know that Dr Thum has done everything
he can to make this work, and of course, we
remain very optimistic and excited.

PRE-EMBRYO CREATION
With pre-embryo creation it is possible to
gather genetic history-focused data and
conduct recessive carrier gene screening on
each individual contributing eggs and sperm
for the creation of embryos.
By collecting and analysing family genetic history,
ORM’s genetics counsellors help identify areas that
may warrant further investigation and/or testing.
Through a saliva or blood sample it is possible to
screen whether individuals contributing eggs and
sperm for the creation of embryos are carriers
for a recessive genetic disease.
The person being tested may not suffer from the
disease being tested, but still be a carrier.
About 35% of people who we test at ORM are
carriers for recessive genetic diseases. If both
people contributing eggs and sperm are carriers
for the same disease then there is an increased risk
that a baby would develop that genetic disease.To

The application period for Oregon Reproductive Medicine’s Fertility Journey – a donor
egg IVF cycle working with an ORM egg donor – has now closed and the team is
evaluating all the applications received. While this is ongoing, we share details of the
genetic screening and testing that ORM is a leader in making available for patients.

screening are available to patients before they

POST-EMBRYO CREATION
Pre-implantation genetic screening (PGS) and
pre-implantation genetic diagnosis (PGD) are
the two forms of post-embryo creation genetic
screening which patients should understand.
PGS involves the testing of embryos to determine
whether they carry the correct number of
chromosomes to develop into a healthy pregnancy
and baby. Embryos with the incorrect number of
chromosomes will typically not develop, resulting
in a failed IVF cycle or miscarriage, or a baby
with one of several conditions – most of which
are incompatible with life but which also include
Down Syndrome.The most advanced form of
PGS involves testing all of the chromosomes by a
technique known as Comprehensive Chromosome
Screening (CCS).At ORM we utilise the most
newest, powerful version of this technique called
Next-Generation Sequencing (NGS).
The formation of abnormal embryos (not
possessing the correct number of chromosomes)
is a natural phenomenon – around 25% of
embryos from a young woman’s eggs will be
naturally abnormal and this rate increases with
the age of a woman’s eggs.These embryos will
not make healthy babies and in most cases
miscarry or do not implant at all. Because of
this, CCS is a form of standardised genetic
screening that is appropriate for every patient to
consider as part of their IVF treatment and as a
means to optimise the chances for success.

PGD involves the testing of embryos to
determine whether a patient-specific genetic or
chromosomal disorder is present. PDG testing is
therefore customised and is only appropriate in
certain circumstances with select patients.
COMPREHENSIVE CHROMOSOME
SCREENING (CCS)
In every IVF cycle embryos are first examined
under the microscope in order to select the best-
looking ones. In addition to this visual inspection
of the embryos, it is possible to count the number
of chromosomes in each embryo. Chromosomes
are packages of genes, or DNA, which are present
in every cell of a person. Normally, there are 46
chromosomes (23 pairs) in each cell. Embryos
with the correct number of chromosomes have
the best potential for implanting and resulting in
a successful pregnancy.
Abnormalities in chromosome number (extra or
missing chromosomes) are the most common
cause of failed implantation in IVF cycles and
miscarriages, and are the only type of problem
that becomes more common as women get older.
CCS is the term for this type of testing.This
process involves the precise laser removal from an
embryo of about five cells that would otherwise
develop into the placenta at the blastocyst (day

five to six) stage.The biopsied cells are laboratory
tested using advanced technology to determine
the number of chromosomes present.The
embryos are generally frozen for future transfer
while the testing is being performed, but in select
circumstances (based on the development of the
embryos and availability of an in-house genetics
testing laboratory as is the case at ORM) it is
possible to complete the testing and transfer the
selected embryo(s) prior to freezing.
Transferring only embryos with the correct
number of chromosomes can often improve
the chances of a successful pregnancy and
also decrease the chance of suffering through
a miscarriage or having a baby born with a
disorder linked to a chromosomal abnormality.
CCS can be especially helpful for patients who are:
• Women 35 years of age or older using their
own eggs
• Couples who have a history of repeated
miscarriage
• Couples who have had a pregnancy or baby
with a chromosomal abnormality
• Couples who have had multiple failed IVF cycles
However, embryos with an incorrect number of
chromosomes are very common, can occur at any
age, and are not related to one’s family history so
CCS is an option for all patients undergoing IVF.
New research shows that CCS can improve the
implantation rates of embryos from egg donors who
are in their twenties. Currently, at ORM over 80% of
IVF patients now choose CCS as part of their cycle.
CCS USING NEXT-GENERATION
SEQUENCING (NGS)
CCS using NGS technology is the most advanced
form of this screening. Compared to conventional
technology, NGS involves the attachment of several
hundred thousand genetic probes to each biopsied
cell. NGS technology delivers the greatest accuracy
and speed for CCS procedures. ORM’s in-house
genetics laboratory employs NGS technology.

Another important event for IVF Spain has been its recent participation in the most important national meeting on embryology organised by the Association for the Study of Reproductive Biology (ASEBIR), where the reproduction clinic has been recognised for excellence in scientific innovation by the Scientific and Organising Committee. Out of the 11 presented studies two important procedures were awarded the highest honour: EMBRYOBANKINGWITHPGS Thanks to this technique embryos are generated and accumulated after several cycles (usually two to four) and ultimately analysed using the Preimplantation Genetic Screening (PGS). Its function is to detect both numerical and structural chromosomal alterations with the aim of transferring embryos with a normal chromosomal content.The chromosomal alterations increase with age, hence it is advisable to apply the PGS to patients older than 38 years. SPERMVITRIFICATION Unlike traditional freezing, through this process we achieve an improvement in survival of the semen sample after cryopreservation.This improvement allows patients to use the sample obtained during the first visit.This way it is possible to achieve better results and, hence, it prevents the male partner from travelling the day of the embryo transfer. The aim of focusing on innovation and technological advances in fertility lies in the need to offer patients the highest pregnancy rates – 70% to 85% depending on the type of treatment and placing the clinic above the Spanish average according to the data provided by the SEF (Spanish Fertility Society) – as well as providing a satisfactory solution to the most complex and difficult cases. Repeated miscarriages, implantation failure and male fertility factors are the primary causes of not achieving pregnancy. Detection is as important as applying an appropriate treatment for each patient, hence IVF Spain has a mechanism for an early detection of factors that constitute the cause for not achieving a successful pregnancy. In short, in most cases, having a multidisciplinary team at our disposal, capable of implementing the advances in reproductive medicine, makes a difference when trying to achieve the desired pregnancy.   • 16. 30 FertilityRoad WWW.FERTILITYROAD.COM ...WITH LISTER FERTILITY CLINIC Dr Thum writes… Following on from the last issue Christina has been matched with a recipient and started another egg sharing IVF treatment.After going through the data obtained from her last stimulation cycle, I have modified her stimulation protocol slightly; hopefully it will help to achieve a better outcome. At Lister we routinely invite couples to come back for a follow-up appointment after a failed cycle. We will use the information that we obtained from their last failed cycle to tailor a better stimulation protocol for a future treatment cycle. In Christina’s case I agreed that she should have an endometrium scratch to enhance implantation. Christina started her treatment cycle in November – she had 13 days of injection for stimulation and during that stimulation period she attended the Lister clinic five times to monitor her follicular growth, and she was subsequently ready for egg collection in mid- November. Her response to the stimulation in this cycle was very good: she had more follicles than in her previous cycle.At Lister we routinely monitor the patient’s oestrogen, LH and progesterone levels during stimulation cycles.We would only proceed to do vaginal egg collection when the blood test levels are at the optimum level with satisfactory follicular size. After 13 days of stimulation Christina’s oestrogen levels were very good and had a good number of follicles. However, her progesterone levels were elevated. From our data and medical publications it shows that if the progesterone levels are elevated before egg collection the chances of achieving a pregnancy are significantly lower.This is most likely due to the endometrium maturation being out of synchronisation with the implantation of embryo.Therefore I advised Christina to proceed with egg collection but to freeze all the embryos at two pronuclear stage, instead of having fresh embryo transfer. From our experience of performing frozen embryo transfer in this situation the chances of achieving a pregnancy is as good as when having fresh embryo transfer. Christina produced 13 eggs – she donated six eggs and kept the rest for her own use. ICSI was performed on six eggs, five achieved fertilisation and four embryos were frozen at the two pronuclear stage. The egg collection was uneventful and Christina had a speedy recovery from the procedure. Christina was understandably upset that she did not have a fresh embryo transfer. I invited her to come for a follow-up appointment to discuss the future plan for her frozen embryo transfer which will be done on her natural cycle. Christina is keen to get on with the treatment and I agreed that we would try to have this done in December… so, by the time you read this. Christina called the nurses at the beginning of December when her period started – she had a baseline scan to assess her lining and her ovaries in view of planning for frozen embryo transfer in this natural cycle.The scan showed a satisfactory endometrium but her ovaries were still quite swollen from her egg collection. Christina was sent home and would go on to book another scan on day 10 of her cycle.The scan on day 10 still showed that her lining has not developed yet. I suggested that we continue to monitor her cycle and hopefully we can do the frozen embryo transfer in this cycle; if not we will do it in her next cycle. Christina has four embryos in storage which appear to be good quality embryos and I think she has a good chance to achieve a positive outcome for her forthcoming frozen embryo transfer treatment. If that is not the case, I will continue to work with the couple to help them achieve their goal. EGG SHARING Christina produced 13 eggs - she donated six eggs and kept the rest for her own use. We are now about to embark on our second round of IVF – we have been matched very quickly and cannot wait to get things started again! As we have already gone through the procedure once before we feel more confident as we know what to expect. After things didn’t work out the first time, we had our follow-up appointment with Dr Thum which we found very useful. He was able to look at our last cycle and modify this new one to suit our needs better as there was data to look at. Upon looking at our last cycle Dr Thum made changes which will hopefully give us the positive outcome we so desperately want. Dr Thum increased our dose of medicated injections as he now knows how I respond to them without overstimulating me, and also said we should have the endometrium scratch to help with implantation.When we left we felt reassured that Dr Thum was making all the changes he could to help us this time round. The first step into our IVF cycle was to have the scratch performed. This was quite uncomfortable but only lasted for a few seconds, and it was worth all the pain as it’s another step closer to making our dream come true. Once this was done we were called in to collect our bag of medication and start the injections the next day. As we have done the process before the injections felt okay and quite normal. To be honest, it becomes part of a nightly routine.We had our routine scans throughout to check that the follicles were growing nicely, which they were. After 13 days stimulating we were ready for egg collection; we couldn’t believe how quickly it came around and that this was really happening again. For some reason we felt quite relaxed through the whole process this time; I think it’s because we knew what to expect at each scan. On the day before we were booked in for egg collection we were called by Dr Thum who advised that everything looked great. We had more follicles this time, though unfortunately my progesterone level was slightly raised which meant that while we could go ahead with the scheduled egg collection the transfer would have to be postponed as my chances would have been significantly lower if we were to proceed straight away. On that note Dr Thum did the best thing for us and decided we do a ‘freeze all’ cycle so my eggs would be protected and we could still use them, albeit it at a later date. I was a bit upset but realised this was the best thing for us to do and was happy DrThum checked the levels so our eggs would not be wasted. Egg collection went well with 13 eggs collected – we gave six away and kept seven for ourselves.We were really happy that we had more this time. I had no issues with the procedure and went home a few hours later. We had the call the following day advising us that four were fertilised with ICSI and were frozen for us the same day. This was an amazing achievement for us: during the last cycle, only one had fertilised, so you can imagine how relieved and happy we were. I wanted to move on to the next step as quickly as possible. I felt a bit empty and wanted my eggs back where they belonged so Dr Thum agreed that we would start on my next period. I was called in to go through the procedure as we haven’t done a frozen transfer before. It all seemed quite straightforward with no daily injections involved and a few scans to monitor my lining as this was a natural frozen transfer. When my period started I went to Lister for my baseline scan to see how everything was looking. It was okay but my ovaries were still quite inflamed from the egg collection and a few bruises were seen, so I was told to come back to see if they reduced as we really wanted to go ahead whilst not risking losing our precious embryos.When we went back for our day 10 scan it still showed large ovaries and my lining had not thickened enough to continue with the frozen embryo transfer. Again, this was quite upsetting but you cannot rush these things – I have realised that now. The body needs time to heal and my embryos need to be put back into the best environment possible. We really want this to work this time round so to wait a few more weeks to my next period is the best thing for them. Our journey continues but we are so close and we know that Dr Thum has done everything he can to make this work, and of course, we remain very optimistic and excited. PGD involves the testing of embryos to determine whether a patient-specific genetic or chromosomal disorder is present. PDG testing is therefore customised and is only appropriate in certain circumstances with select patients. COMPREHENSIVE CHROMOSOME SCREENING (CCS) In every IVF cycle embryos are first examined under the microscope in order to select the best- looking ones. In addition to this visual inspection of the embryos, it is possible to count the number of chromosomes in each embryo. Chromosomes are packages of genes, or DNA, which are present in every cell of a person. Normally, there are 46 chromosomes (23 pairs) in each cell. Embryos with the correct number of chromosomes have the best potential for implanting and resulting in a successful pregnancy. Abnormalities in chromosome number (extra or missing chromosomes) are the most common cause of failed implantation in IVF cycles and miscarriages, and are the only type of problem that becomes more common as women get older. CCS is the term for this type of testing.This process involves the precise laser removal from an embryo of about five cells that would otherwise develop into the placenta at the blastocyst (day Dr John HeslaDr Brandon Bankowski The ORM team five to six) stage.The biopsied cells are laboratory tested using advanced technology to determine the number of chromosomes present.The embryos are generally frozen for future transfer while the testing is being performed, but in select circumstances (based on the development of the embryos and availability of an in-house genetics testing laboratory as is the case at ORM) it is possible to complete the testing and transfer the selected embryo(s) prior to freezing. Transferring only embryos with the correct number of chromosomes can often improve the chances of a successful pregnancy and also decrease the chance of suffering through a miscarriage or having a baby born with a disorder linked to a chromosomal abnormality. CCS can be especially helpful for patients who are: • Women 35 years of age or older using their own eggs • Couples who have a history of repeated miscarriage • Couples who have had a pregnancy or baby with a chromosomal abnormality • Couples who have had multiple failed IVF cycles However, embryos with an incorrect number of chromosomes are very common, can occur at any age, and are not related to one’s family history so CCS is an option for all patients undergoing IVF. New research shows that CCS can improve the implantation rates of embryos from egg donors who are in their twenties. Currently, at ORM over 80% of IVF patients now choose CCS as part of their cycle. CCS USING NEXT-GENERATION SEQUENCING (NGS) CCS using NGS technology is the most advanced form of this screening. Compared to conventional technology, NGS involves the attachment of several hundred thousand genetic probes to each biopsied cell. NGS technology delivers the greatest accuracy and speed for CCS procedures. ORM’s in-house genetics laboratory employs NGS technology. CCS is a form of standardised genetic screening that is appropriate for every patient to consider as part of their IVF treatment and as a means to optimise the chances for success.
