2. In order to begin, the initial
paperwork will need to be
obtained from your local
Department of Social
Services.
This paperwork will need to
be completed by your doctor.

3. Your doctor will complete
paperwork determining
diagnosis, tasks needed,
and self-directing ability.

4. Your doctor will
submit your
paperwork to the
Local Department of
Social Services who
will determine if you
are eligible for the
program.

5. A home visit is conducted in
collaboration with Cortland
Community Action Program
(CAPCO), the Local Department of
Social Services, and Professional
Nurses to assess the consumer’s
needs.

6. The hours of care needed
After assessment, a
care plan will made
which specifies:
What tasks will need to be
performed

7. Although an assigned nurse will oversee
your ability to make informed decisions,
it is YOU who is in charge.
Duties include:
 Hiring and training aide to meet the
care plan developed.

 Overseeing that you receive care.

8. Contact DSS,
