Systems in an effort to be able to exchange data with other systems in their working
environment. The importance of interoperability will continue to grow as one of the major
drivers of this has been the formation of accountable care organizations (ACO’s).Electronic
health records have a large potential to support nursing and advanced nursing knowledge
through the elimination of redundant efforts, redesign of the workflow, demonstrating the
contributions that nurses make to patient care and outcomes as well as contributing to the
body of nursing knowledge through the incorporation of standardized nursing
language.Assignment Descriiption:The patient presents to your office with a Personal
Health Records (PHR). Remembering, medical records and PHRs are not the same thing,
complete both of the following:1. Develop and create an educational tool (brochure, flyer
pamphlet) that you can provide to your patients that address the following areas:• How do

and create an educational tool (brochure, flyer pamphlet) for providers to understand the
PHR that address the following areas:• How can we make sure PHR information is accurate,
reliable, and complete?• What obstacles might patients have with control over how their
health information is accessed, used, and disclosed?• Frequently Asked Questions• What
advantages and disadvantages are there?• What does the clinician need to understand?May
choose to do a flyer for one page and pamphlet or brochure for the other.
