Case Manager Mar 1998– Present
 Provide case management and multidisciplinary collaboration to address the immediate and
anticipated care needs of members.
 Facilitate communications and coordination among all participants of the health care team and
the member to ensure that the services are provided to promote quality cost effective outcomes,
successful transitions across healthcare settings
 Provide Case management interventions on behalf of members who encountered an illness
while away from their contracted providers
 Experience with transitioning patients from one care setting to another
 Educate patients and staff on TMC programs
 Knowledgeable using clinical based criteria to determine appropriateness of lever of care and
length of stay.
 Develop, implement and monitor a care plan through the interdisciplinary team process in
conjunction with the individual
 Experience as a reviewer of patients who in the acute hospital, skilled nursing, rehabilitation
center or Long Term acute care settings
 Conduct a thorough and objective assessment of the members current status including, physical,
psychosocial environmental and gather all information pertinent to the case
 Identify diagnosis and determine need for continuing hospitalizations and monitor the LOS as
per established guidelines and professional judgment
 Effectively communicates with peers, Managers and the Medical Director concerns or issues
 Able to prioritize and reprioritize an ever changing case load
 Access and evaluate community resources to meet patients’ needs
 Knowledge of age related developmental stages and needs.
 Ability to work effectively under pressure due to changing priorities, interruptions, high and
low census, payer demands, and discharge planning demands.
 Demonstrates the knowledge and skills necessary to communicate with third party payers to
obtain authorization for the appropriate treatment setting for patients requiring medical, psych
and chemical dependency treatment.
