Patient Monitoring Tool (“WDRMT”)
Online and mobile application tool designed for health care providers, as a means
to monitor patient stats as per the WDMA. The RPM is accessible via mobile
application and via a secure login on the WINNING DIABETICS™ Multi-Lingual
Social Network, hosted on a cloud-based server. The tool is:
1. Activated by the prescriber (health care provider) and transmits
messages to the patient based on his or her blood glucose values and trends
instructing patients on what to do differently if they need to improve glucose
levels;
2. Hosted on a cloud-based service intended to provide wireless
connectivity and managed network services for glucose meters, managing the
wireless access, transfer and storage of the glucose meter data (streamed in real-
time), as well as WDMSN member data input (medication dosage and food
consumption logs). Glucose monitor data can be transferred seamlessly to
the WINNING DIABETICS™ data center via a cellular connection in a stand-alone
device and uploaded with the touch of a button, to the WINNING
DIABETICS™ cloud server.
