Today, I would like to discuss about the changing role of nurses and concept of continuum of care and the new health care delivery models such as Accountable Care Organizations (ACOs), medical homes, and nurse -managed health clinics.

Health care system is changing; it is not just focused on the curative care but the disease prevention and wellness. Also, the new healthcare reform has approach to the development of healthcare delivery models with the changes in nursing role from acute care settings to community health care. Patient Protection and Affordable Care Act (PPACA) formed at 2010 support the establishment of ACOs, medical homes and nurse-managed health clinics; also new payment policies. With the formation of Affordable Care Act (ACA), health has transformed from traditional treating approach to the preventive and wellness. Nurse’s role has also changed from front-line caregiver to the care integrators and coordinators and help in providing the health services to the underserved people. All the health models are focused on the concept of continuity of care which is defined as “a concept involving an integrated system of care that guides and tracks patient over time through a comprehensive array of health services spanning all levels of intensity of care” (Evashwick, 1989). It covers the delivery of healthcare over a period of time and for all levels and stages of care. Let’s talk about the three types of health care delivery model.
Accountable Care Organizations (ACOs)
In accordance to the Center for Medicare and Medicaid services, “Accountable Care Organizations (ACOs) are groups of doctors, hospitals, and other health care providers, who come together voluntarily to give coordinated high quality care to their Medicare patients” (Cms.gov, n.d.). The ACO is the new unit built by the ACA; the health reform law 2010 which take care of the cost and quality of the care received by the patients. The main aim of the multidisciplinary teams is to ensure that there is improved outcome on the patient’s health status. The multidisciplinary disciplinary teams involve the staffs in the adult day care. Adult day care involves taking care of an individual in all round manner. That is nutritional, health, and the daily living needs. There are different teams involved in each activity in the adult day care facility. There are nutritionist, health specialists, fitness advisor, and living advisor. For health specialists, there are different levels of multidisciplinary levels can be grouped as follows; nurses, technicians, anesthesiologists, and attending physicians (Epstein, 2014). Introduction
Louisiana Medicaid has provided health care to its members for quite some time now. Mostly, the behavioral health system has incorporated the consumers, the community and other health care providers in the process of giving efficient services to the members (Ortenberg & Roth, 2013). Their focus has been to improve access to the treatment services that have become vital to people. Additionally, the system ought to expand the services that are being provided by the Louisiana Medicaid health behavior systems. The demand for that service has grown, and it’s the duty of the system to expand the services to meet the request. Finally, it’s also the goal of the health behavior system to provide care that is accessible to all at ease.
Description of Behavioral Health Services
The following are some of the services provided at the Louisiana Medicaid behavior care
Addiction services- This includes the individual-centered outpatient services. The health care provider provides rehabilitation and recovery process to the patients. The health care provider will help in the promotion of skills that are responsible for coping with the current lifestyle. The department will assist in the elimination of substance use symptoms and behaviors that may prevent recovery.
Crisis intervention- The department is responsible for the individuals experiencing a psychiatric crisis. When a disaster occurs, victims may be affected by such occurrences may cause psychological problems. Therefore, the crisis intervention program will do a preliminary assessment then followed by a crisis resolution. After that, the medical professional will then do referral and linkage to the relevant community service for further treatment processes.
Group psychotherapy- in this section, a group of individuals with similar behavior challenges will have to sit together and share their experiences. Apparently, they share personal coping skills and practices. Typically, the session will help the patient to identify which method works for them and open up to help in the recovery process.
Psychosocial rehabilitation- The section mainly helps in the elimination of behaviors or barriers that may prevent the healing of the mental Illness. Such behaviors may be to stay away from friends who smoke if the patient if fighting an addiction of smoking.

is not true and that the sun is the
center of our universe. Pers -
pectives of health care have
undergone similar, radical changes
in perception. For centuries we
had a hospital-centric view; an
illness-based model, where the majority of care was
provided in hospitals, when we were ill. In the last
few decades, that model has migrated to a more con-
tinuum of care view; a wellness/health maintenance
model, where emphasis of care is outside the hospital
in other venues such as outpatient, ambulatory/clin-
ic, and home care (see Figure 1).
But as we all know, this is still not where we need
to be to support the highest quality care at the right
cost. Despite a focus on moving care out of the hospi-
tals, one only needs to think about the process of
medication reconciliation between care venues to
realize the lack of seamless integration of care deliv-
ery and the challenges of supporting interoperability
across the continuum. Hence, here I am proposing the
patient centric view, where the patient actively partic-
ipates in his or her care and we look at delivering care
from a patient’s point of view. This allows us to break
down some of the barriers we have struggled with on
our journeys to promote higher quality care through
the use of health information technology (HIT). Now
we need to consider how the health care system
should revolve around the patient, rather than the
patient rotating around the hospital. Considering a
patient-centric point of view when implementing and
optimizing the use of HIT provides new perspectives
on the meaning of “integrated” health care.
Patient-Centric Care
It might seem odd that a health care organization
needs to be reminded to involve the patient in his or
her care. After all, this approach would certainly be
supported from a patient’s perspective. And, of
course, the health care industry has compelling rea-
sons to incorporate a strong customer and service
focus in order to improve patient satisfaction and
impact patient loyalty. But as health care systems

Reporting standards Hennes & Mauritz AB (also known as H&M) , the Swedish MNE that is a
trendsetter in the latest fashion trends, has a stated goal “to give customers unbeatable value by

is not true and that the sun is the
center of our universe. Pers -
pectives of health care have
undergone similar, radical changes
in perception. For centuries we
had a hospital-centric view; an
illness-based model, where the majority of care was
provided in hospitals, when we were ill. In the last
few decades, that model has migrated to a more con-
tinuum of care view; a wellness/health maintenance
model, where emphasis of care is outside the hospital
in other venues such as outpatient, ambulatory/clin-
ic, and home care (see Figure 1).
But as we all know, this is still not where we need
to be to support the highest quality care at the right
cost. Despite a focus on moving care out of the hospi-
tals, one only needs to think about the process of
medication reconciliation between care venues to
realize the lack of seamless integration of care deliv-
ery and the challenges of supporting interoperability
across the continuum. Hence, here I am proposing the
patient centric view, where the patient actively partic-
ipates in his or her care and we look at delivering care
from a patient’s point of view. This allows us to break
down some of the barriers we have struggled with on
our journeys to promote higher quality care through
the use of health information technology (HIT). Now
we need to consider how the health care system
should revolve around the patient, rather than the
patient rotating around the hospital. Considering a
patient-centric point of view when implementing and
optimizing the use of HIT provides new perspectives
on the meaning of “integrated” health care.

It might seem odd that a health care organization
needs to be reminded to involve the patient in his or
her care. After all, this approach would certainly be
supported from a patient’s perspective. And, of
course, the health care industry has compelling rea-
sons to incorporate a strong customer and service
focus in order to improve patient satisfaction and

Acetabularia acetabulum is a single-celled green alga that in its vegetative state is morphologically differentiated into a basal rhizoid and an axially elongated stalk, which bears whorls of branching hairs. The single diploid nucleus resides in the rhizoid.
