The case to be discussed will be as follows: Ms. Marisela Sanchez is a widowed 58-year-old Nicaraguan woman, who is an immigrant, and has lived in Miami for ten years. She lives in a second floor walk-up apartment with her daughter and son-in-law and their four children in Hialeah. Her son-in-law, who is the sole breadwinner, lost his job five months ago. They are two months behind in rent and can barely afford to pay for food and utilities. About a month ago, Ms. Sanchez's daughter found a job as a custodian in a daycare center. She was laid-off, though, after two weeks, for being constantly late to work due to lack of transportation. One of Ms. Sanchez's granddaughters, Leana, is 6 years old and was diagnosed with autism spectrum disorder. Though she is uninsured, she has benefitted from home therapy services in the past and is now experiencing problems with communication in the home and school settings. Ideally, the family would like to continue therapy services, but they can no longer afford them. Ms. Sanchez, also a month ago, suffered a stroke and was hospitalized for five days. Because she has no health insurance, she is worried about her hospital bill and is unsure where to pursue primary care followup. On discharge, the hospital social worker gave her a list of several community health centers and other programs in Miami-Dade County, but she cannot decide. Part of her concern is that she is undocumented. The family hears of a health fair in Hialeah and brings Ms. Sanchez to the event, hoping to get some advice. You are part of an interdisciplinary team of students staffing a health-screening booth. Your team notices that she is able to walk slowly with a wide- based quad cane. As she answers your questions, she lets you now that she has fallen twice since discharge, without significant injury, and informs you that she has diabetes and high blood pressure. She takes oral medications, but she is getting worried. She reports that she sometimes feels like food gets caught in her throat and sometimes coughs when she drinks.
