This study evaluated chronic pelvic pain in 150 females of reproductive age from two hospitals in Pakistan. The subjects had a mean age of 34.8 years. The most common presenting complaint was vaginal discharge, reported in 56% of subjects. Other common complaints included infertility (7%) and dysmenorrhea with pelvic pain (7%). The conclusion is that physicians should take a detailed gynecological history when evaluating and treating chronic pelvic pain in female patients.

This study prospectively observed 120 women referred to a hospital with postcoital bleeding over one year. The study found that over 50% of women were under 35, over 90% had PCB for more than 4 weeks, and 19.2% had severe episodes. Significant pathology was found in 53.8% of women under 35, suggesting age alone may not determine pathology. None of the 11 women with cervical cancer or CIN3 had previous abnormal smears. The study recommends urgent referral for all women with PCB and evaluating all for infections, given the high rate of detected pathology.

Mother gave ibuprofen susp. With diclofenat in alternative way aiming to control temperature .
On the second day the mother asking to medical advice for this illness and she was diagnosis as acute tonsillephyrgitis and azithoromcin described to ttt the infection
At that time only the increase in temperature & mild abdominal pain there is no vomiting or diarrhea
At night the mother called here doctor prescribing a bloody stool twice with no vomiting or colic
He ask the mother to do abdominal ultra sound & erect X RAY for her daughter
But the results was –ve nothing wa found to explain the bloody stool
He ask here to make alab evaluation for C B C &stool analysis
On the third day she pass a fleshy, bulky bloody mass twice was passed one of them referred to pathology lab. & the patient was referred to be evaluated by surgeon who ask to repeat the ultrasound and the results referred to signs of intussusception but there is no any clinical signs of obstruction like vomiting or bouts of colic she referred to Cairo for further evaluation .
Ultrasound was done for the third time but surprising us
There is no any suggestive signs of intussusceptions so the decision was wait and see with the time no blood in stool general condition improved with time temperature disappeared
So she was referred to PEDIATRIC G. I .T consultant for revaluation

This study examined the use of maintenance tocolysis therapy to prevent preterm labor in Albania. The study included 325 pregnant women between 28-32 weeks gestation hospitalized for preterm labor. Women received tocolysis therapy for over one month. Those between 28-30 weeks (n=200) had greater benefits, with pregnancy prolonged by 6-8 weeks on average. The authors conclude maintenance tocolysis therapy can considerably prolong pregnancy for women diagnosed with preterm labor between 28-32 weeks gestation, though larger studies are still needed.
Hysteroscopy & IUI
This research article studied the impact of performing routine hysteroscopy (examination of the uterus) prior to intrauterine insemination (IUI) on pregnancy rates. 180 infertile women undergoing IUI were divided into two groups: one group received hysteroscopy before IUI to check for any uterine abnormalities, while the other group did not receive hysteroscopy. Hysteroscopy found uterine abnormalities in 41% of women. 1. This study compared screening for trisomies 21, 18 and 13 using cell-free DNA (cfDNA) analysis of maternal blood at 10-11 weeks' gestation to screening using the combined test at 11-13 weeks.
2. CfDNA analysis identified all cases of trisomy 21 and the majority of trisomy 18 cases, with a lower false positive rate than the combined test. Detection of trisomy 13 was lower.

This study examined the use of maintenance tocolysis therapy to prevent preterm labor in Albania. The study included 325 pregnant women between 28-32 weeks gestation hospitalized for preterm labor. Women received tocolysis therapy for over one month. Those between 28-30 weeks (n=200) had greater benefits, with pregnancy prolonged by 6-8 weeks on average. The authors conclude maintenance tocolysis therapy can considerably prolong pregnancy for women diagnosed with preterm labor between 28-32 weeks gestation, though larger studies are still needed.
Hysteroscopy & IUI
This research article studied the impact of performing routine hysteroscopy (examination of the uterus) prior to intrauterine insemination (IUI) on pregnancy rates. 180 infertile women undergoing IUI were divided into two groups: one group received hysteroscopy before IUI to check for any uterine abnormalities, while the other group did not receive hysteroscopy. Hysteroscopy found uterine abnormalities in 41% of women. Pregnancy rates were significantly higher in the group that received hysteroscopy before IUI (46.25%) compared to the group without hysteroscopy (25%). A 47-year-old female presented with a one-year history of rectal bleeding. Examination revealed a low rectal ulcer involving half the rectal circumference, from which fresh blood was observed. She was initially treated for hemorrhoids and dysentery without success. A colonoscopy was reported as normal except for internal hemorrhoids. Upon further examination, Dr. Al-Gailani discovered a rectal carcinoma. The patient was advised to undergo a biopsy of the rectal ulcer and staging scans before considering an abdomino-perineal resection of the rectum. The case highlights the importance of performing a thorough physical examination, including a digital rectal exam, for any patient presenting with rectal bleeding,
Screening what tests when
Chromosomal microarray analysis (CMA) provides a higher resolution than traditional karyotyping for detecting deletions and duplications. CMA can detect deletion and duplication at higher resolution than karyotyping and cannot detect inversions and balanced translocations. CMA is useful for detecting submicroscopic abnormalities that may cause intellectual disability, structural defects, and physical or ultrasound findings. It offers a more comprehensive evaluation of genetic abnormalities than karyotyping alone.

Ectopic pregnancy
This patient is a 30-year-old female who presented with vaginal spotting during her first trimester of pregnancy. An ultrasound was ordered which did not show an intrauterine or ectopic pregnancy. The patient's beta-hCG level was measured at 1056. At follow-up two days later, the patient's symptoms had resolved but her beta-hCG had risen to 1465. A repeat ultrasound now showed a right tubal ectopic pregnancy, so the patient underwent a laparoscopic right salpingectomy to remove the ectopic pregnancy from her fallopian tube.

Overall life span (LS) was 1671.7±1721.6 days and cumulative 5YS reached 62.4%, 10 years – 50.4%, 20 years – 44.6%. 94 LCP lived more than 5 years without cancer (LS=2958.6±1723.6 days), 22 – more than 10 years (LS=5571±1841.8 days). 67 LCP died because of LC (LS=471.9±344 days). AT significantly improved 5YS (68% vs. 53.7%) (P=0.028 by log-rank test). Cox modeling displayed that 5YS of LCP significantly depended on: N0-N12, T3-4, blood cell circuit, cell ratio factors (ratio between cancer cells-CC and blood cells subpopulations), LC cell dynamics, recalcification time, heparin tolerance, prothrombin index, protein, AT, procedure type (P=0.000-0.031). Neural networks, genetic algorithm selection and bootstrap simulation revealed relationships between 5YS and N0-12 (rank=1), thrombocytes/CC (rank=2), segmented neutrophils/CC (3), eosinophils/CC (4), erythrocytes/CC (5), healthy cells/CC (6), lymphocytes/CC (7), stick neutrophils/CC (8), leucocytes/CC (9), monocytes/CC (10). Correct prediction of 5YS was 100% by neural networks computing (error=0.000; area under ROC curve=1.0).
