Sharp Chula Vista Medical Center Sharp Chula Vista Medical Center (SCVMC) is a large 343–bed hospital that has been serving the South Bay
community in San Diego County since 1979. This facility is affiliated with over 450 physicians and employs in excess of 2,000 people. Needless to
say, the SCVMC Health Information Management Department is extremely busy! The HIM department of SCVMC has a staff of 51 people in
various positions including Clerical Assistants, Optical Imaging Specialists, Medical Coders, and Supervisors. At this time, Sharp employs XX RHITS
and XX RHIAS in the department. Records Organization Sharp is an early adopter of electronic patient records. In fact, this facility first started using a
basic form of patient electronic records in 1984. Since 2008, the HIM department has been fully integrated with the admissions department which all
works together to ensure that registration, healthcare records and billing data management is as seamless as possible. Registration Sharp uses a
program called OnBase for patient registration. When patients arrive at the hospital, the admitting clerk scans their driver's license and insurance card.
Conditions of admission are displayed in electronic form and patients sign on an electronic tablet to indicate consent. Paper versions of patient
registration is printed only when requested by the patient, otherwise registration is totally paperless. Information in OnBase is fed into several

Dr. Fox is an emergency physician at UCI Medical Center, treating and diagnosing patients using an ultrasound. His scope of research involves looking
at patients' internal organs via ultrasound, which is much quicker and less harmful than using x–ray scans that are traditionally used in emergency
departments. He is also a part of the American Institute of Ultrasound in Medicine ("Faculty and Staff"). Dr. Fox is also a director of instructional
ultrasound and is the assistant dean at the UCI School of Medicine ("John Christian Fox"). He started an ultrasound rotation at the UCI School of
Medicine, and with a grant given by SonoSite, Inc., the ultrasound curriculum is now embedded within the UCI School of Medicine curriculum
("Faculty and Staff"). He went to Tufts University School of Medicine, receiving his MD in 1997 ("John Christian Fox"). His main research focus is to
try to incorporate ultrasound more into the emergency department, and tries to find revolutionary ways to utilize ultrasound, especially because it is
very cost–efficient and images are seen real–time, unlike MRI scans or x–rays, which can emit harmful radiation or results take much longer to

Baylor University Medical Center at Dallas was established as Texas Baptist Memorial Sanitarium. Chartered on Oct. 16, 1903, by the State of Texas,
the hospital began in a 14–room renovated house. In 1900, there were 3 contributors who founded Baylor and one of them was who are Charles M.

most modern hospital. The Baylor University School of Nursing began around the same time and this was due to a generous donation from Col. C.C.

"brief" procedure to relieve her lingering back pain. This was not the case and two days later, Carcerano was dead. Carcerano's surgery was
supposed to be a simple procedure where surgeons were going to insert a pump under her skin so that medication could get into her spine quickly and
help alleviate her lingering pain. Carcerano has previously fallen and broken several vertebrae, but even after fusing the vertebra together, the pain,
along with leg spasms and weakened bones, was still a chronic issue. The surgery went as planned is what the article reports. Carcerano was put to
sleep, the pump was inserted, and a dye was pushed through the pump to make sure that the placement in the spine was correct. The dye that was
ordered for the surgery wasn't in stock at the hospital that day, so the pharmacist sent up a different dye that was supposed to be equal to what was
ordered. Unfortunately, the dye that was used in the surgery was completely wrong and even had the statement "not for intrathecal use"

remembering less and less of me, my siblings, and my parents. His death came abruptly, but brought catharsis, acceptance, and the celebration of
all our memories with him. He is a constant source of motivation and inspiration to become a doctor. My own loss sparked a desire to help others
through tough times. I therefore began volunteering at the Orlando Regional Medical Center, where I consoled people going through those times,
and let them disclose their feelings with tears and words the way I needed to when I was in their shoes. Unfortunately, I also met people who were
dying and had no visitors to support them through their death. For instance, I visited a woman on several occasions and shared a few laughs and
stories with her. She ultimately died from C. difficile over a period of three weeks in the hospital. At those moments, I was glad patients like her had
someone with them, even if I was a stranger to them. Still, there were much happier occasions where families learned their sons and daughters and
parents were alive, recovering, or asking for their company. Through all this, I recognized the fear and pain they felt, and helped them move forward.

Prior to walking in, my expectations for what I was about to partake in and experience were all over the place. I didn't know whether to expect the
absolute worst types of situations going on such as people being rushed into medical rooms or the most basic situations such as patients waiting for a

Sharp Chula Vista Medical Center Sharp Chula Vista Medical Center Sharp Chula Vista Medical Center (SCVMC) is a large 343–bed hospital that has been serving the South Bay community in San Diego County since 1979. This facility is affiliated with over 450 physicians and employs in excess of 2,000 people. Needless to say, the SCVMC Health Information Management Department is extremely busy! The HIM department of SCVMC has a staff of 51 people in various positions including Clerical Assistants, Optical Imaging Specialists, Medical Coders, and Supervisors. At this time, Sharp employs XX RHITS and XX RHIAS in the department. Records Organization Sharp is an early adopter of electronic patient records. In fact, this facility first started using a basic form of patient electronic records in 1984. Since 2008, the HIM department has been fully integrated with the admissions department which all works together to ensure that registration, healthcare records and billing data management is as seamless as possible. Registration Sharp uses a program called OnBase for patient registration. When patients arrive at the hospital, the admitting clerk scans their driver's license and insurance card. Conditions of admission are displayed in electronic form and patients sign on an electronic tablet to indicate consent. Paper versions of patient registration is printed only when requested by the patient, otherwise registration is totally paperless.
