beheersen, zullen zij keer op keer problemen tegen komen en zich afvragen
waarom het niet wil lukken. Dit is tevens een voedingsbron voor import uit
lagelonenlanden. Daarom is het van belang dat wij onze plek, op de
Nederlandse markt, zeker stellen door werkstukken te maken die qua
vormgeving volledig anatomisch zijn en die qua kleur natuurlijke transparantie
bevatten. Deze twee eigenschappen vormen samen de esthetische basis.
Wanneer de functie klopt zullen kronen en bruggen optisch dichter bij de
natuurlijke vorm komen. De vormgeving die volledig anatomisch moet zijn,
verkrijgen we door met het occlusale kompas te werken. Het probleem is: hoe
realiseren we dit in het laboratorium, hoe brengen we de samenwerking met de
tandarts tot stand en hoe overtuigen we de tandarts van het nut van het
occlusaal kompas.
Met deze scriptie wil ik voor zowel laboratorium als tandarts de baten en de

Voor u ligt een scriptie dat is opgesteld namens een afstudeeropdracht van de opleiding Maatschappelijk Werk en Dienstverlening aan de Haagse Hogeschool in Den Haag. De focus van deze scriptie ligt bij een groeiende bevolkingsgroep, waarbij mij aan het hart gaat dat hier meer begrip voor komt. Tevens streeft de schrijver naar opheldering over het dagelijks leven van kwetsbare ouderen binnen een culturele gemeenschap. Met dit document wil de schrijver de behoeften van thuiswonende kwetsbare ouderen in kaart brengen en uitzoeken wat hen in het hart ligt om de dag door te komen. De schrijver acht de “social quality of life” bij kwetsbare ouderen in combinatie met de samenwerking voor relevante instanties noodzakelijk. De Wet medezeggenschap op scholen (WMS) is in januari 2007 van kracht geworden. De wet heeft zijn werking in het primair onderwijs, het voortgezet onderwijs en de ex- pertisecentra. Deze onderwijssectoren krijgen met de invoering van de WMS een eigen wettelijk kader voor medezeggenschap.
Op verzoek van de Projectgroep WMS heeft het ITS van de Radboud Universiteit Nij- megen de invoering van de Wet medezeggenschap op scholen in kaart gebracht. Van dit onderzoek wordt hier verslag gedaan. Het onderzoek startte april 2007 en is afgesloten in oktober 2007.
Het onderzoek is uitgevoerd binnen het thema Organisatie van het onderwijs van het ITS door Frederik Smit en Sanne Elfering. De analyses van het eerste deel van het onderzoek zijn uitgevoerd door Roderick Sluiter.
De onderzoekers stellen er prijs op te verklaren dat zij op een zeer constructieve wijze bij opzet en rapportage begeleid zijn door een commissie vanuit de Projectgroep WMS bestaande uit:

de ongeveer zestig illustraties die hij maakte voor de Divina Proportione, een
schriftstuk van de wiskundige Luca Pacioli. Leonardo da Vinci baseerde zich op de
beschrijvingen van Vitruvius, maar hij voerde onderzoek uit om zich op eigen
ervaringen te kunnen baseren voordat hij de tekening maakte. De tekening werd
ingedeeld bij het anatomisch onderzoek waar Leonardo de Vinci al enige tijd mee
bezig was. Voorafgaand aan de tekening nam hij systematisch lichaamsmaten van
mannen op. Proportieleer werd in deze tijd meer toegepast, onder meer door Leone
Battista Alberti.]
Rond 1490 tekende Leonardo da Vinci de Man van Vitruvius. Na anatomisch
onderzoek en vergelijkingen met mannelijke lichamen, trok hij de conclusie dat
er terugkomende vergelijkingen zijn tussen mensen hun lichaamverhoudingen.
Dit vinden we ook terug bij de schedel. Hierbij hebben de oren, ogen, neus en
mond allemaal dezelfde lichaamsverhouding. Het is ook in de mond verder
door te voeren. Dit is onderzocht door Michael Heinz Polz [†] en in 2001
wetenschappelijk bewezen, hieruit volgde de naam het “occlusaal kompas” voor
dit concept.
De afbeelding op de voorkant is het logo van mijn website. Hierin zitten alle
elementen verwerkt van het occlusaal kompas.
Door deze scriptie heb ik veel mogen leren over het occlusale kompas, daarom
wil ik mijn drie werkgevers van Sophisticated Ceramics bedanken voor de ruimte

4. 1 Inleiding
In de tandtechniek zijn veel mensen werkzaam die dag in dag uit proberen
esthetische werkstukken te maken. Echter, wanneer zij de functieleer niet
beheersen, zullen zij keer op keer problemen tegen komen en zich afvragen
waarom het niet wil lukken. Dit is tevens een voedingsbron voor import uit
lagelonenlanden. Daarom is het van belang dat wij onze plek, op de
Nederlandse markt, zeker stellen door werkstukken te maken die qua
vormgeving volledig anatomisch zijn en die qua kleur natuurlijke transparantie
bevatten. Deze twee eigenschappen vormen samen de esthetische basis.
Wanneer de functie klopt zullen kronen en bruggen optisch dichter bij de
natuurlijke vorm komen. De vormgeving die volledig anatomisch moet zijn,
verkrijgen we door met het occlusale kompas te werken. Het probleem is: hoe
realiseren we dit in het laboratorium, hoe brengen we de samenwerking met de
tandarts tot stand en hoe overtuigen we de tandarts van het nut van het
occlusaal kompas.
Met deze scriptie wil ik voor zowel laboratorium als tandarts de baten en de
toepassing van het occlusaal kompas uitleggen.

5. 2 Wat houdt het occlusaal
kompas in
2.1 Inleiding
Aan de hand van het occlusaal kompas
zijn de verschillende kauwbewegingen
zichtbaar gemaakt. Hiermee wordt
aangetoond waar de vrijruimtes nodig zijn
voor de juiste occlusale vormgeving.
Zonder dit concept verdwaal je in de vormgeving.
De functionaliteit is in samenhang met de kauwvlakken.
Dit betekent dat de knobbels niet storen met de articulatie.
Het occlusaal kompas kunnen we vergelijken met “schilderen op nummers”.
Zonder dit concept is het heel moeilijk om te leren schilderen en is de weg lang
om het schilderen onder de knie te krijgen. We kunnen uiteindelijk wel een
goed resultaat bereiken, maar dit alles is zonder kennis.
Met het “schilderen op nummers” is er overzicht wat de juiste werkwijze is,
duidelijkheid wat op welke plaats komt en het is relatief makkelijk te leren.
Dachten we zonder het occlusaal kompas al het perfecte resultaat te kunnen
bereiken, met het occlusaal kompas kunnen we de natuurlijke vormgeving pas
echt benaderen.

6. 2.2 Uitleg van het occlusaal kompas
De bewegingen heeft men aangeduid met de kleuren:
De rode punt in het midden is de antagonist en de central fossa.
Zwart is de protrusie
Rood is de retrusie
Blauw is de laterotrusie
Geel is de lateroprotrusie RS
Groen is de mediotrusie
ISS
De twee rode vlakken zijn de grensbewegingen.
Het ene vlak is de ISS (immediate side-shift) die zich aan de mediotrusie en de
retrusie aansluit. Deze vormt zich:
• als er in het andere kwadrant, van dezelfde kaak, een lateroretrusie of
een laterotrusie plaatsvindt.
• bij de antagonist als er een ook een ISS plaatsvindt.
Het andere vlak is de RS (retrusive surtrusie) die zich aan de laterotrusie aansluit.
• Deze vormt zich als er in het andere kwadrant een mediotrusie
plaatsvindt.

7. 2.3 Rotatieassen van de onderkaak
Hieronder kunnen we een beeld krijgen bij de verschillende beweging van de
onderkaak.
Voor de proale beweging gebruiken we de transversale as die naar distaal
beweegt (tegenovergesteld aan de retrale beweging).
Voor de retrale beweging gebruiken we de transversale as.
Voor de laterotrusie beweging gebruiken we de verticale as.
Voor de lateroproale beweging gebruiken we de transversale en de verticale as
(combinatie van de laterotrusie en de protrusie).
Voor de mediotrusie beweging gebruiken we de sagitale as.

8. 2.4 Uitleg van de bewegingen
Protrusie
De Protrusie (zwart) loopt evenwijdig aan de
mediaanlijn. Hierbij maakt de onderkaak een
voorwaartse beweging t.o.v. de centrale relatie
(protrusion).
Retrusie
De retrusie (rood) loopt net zoals de protrusie
evenwijdig aan de mediaanlijn. Dit is de
achteruitschuiving van de onderkaak (retrusie).

9. Laterotrusie en mediotrusie naar rechts
De laterotrusie (blauw) is een zijdelingse,
buiten het middelpunt gelegen, beweging
van de onderkaak.
Mediotrusie (groen) is een beweging van de
onderkaak naar het midden.
Voor de laterotrusie en mediotrusie geldt
dat deze betrekking hebben op één van de
kaakhelften.
We spreken hier van een laterale
condylusbaan, het traject dat door de
condylus (gewrichtsuitsteeksel) wordt
afgelegd bij een laterale beweging van de
onderkaak; aan de dragende knobbel is dit
traject zeer kort, aan de andere (niet
actieve) zijde naar medioprotrusie zijde is
dit lang.
Lateroprotrusie en medioprotrusie naar
rechts
De lateroprotrusie is de beweging die loopt
tussen de laterotrusie en de proale
beweging.
De medioprotrusie is de beweging die loopt
tussen de mediotrusie en de
proalebeweging.
Deze ontstaat wanneer er in het andere
kwadrant, van dezelfde kaak, een
lateroprotrusie plaatsvindt.

10. Immediate side-shift
Dit is de transversale verplaatsing van de
roterende condylus van de onderkaak direct na
het begin van een laterale beweging.
Alle kaakbewegingen naar rechts
Hier is duidelijk zichtbaar welke bewegings-
mogelijkheden er zijn bij een beweging naar
rechts.
Hierbij geldt dat er in het:
1e kwadrant een laterotrusie of lateroprotrusie
plaatsvindt,
2e kwadrant een mediotrusie of medioprotrusie
plaatsvindt,
3e kwadrant een mediotrusie of medioprotrusie
plaatsvindt,
4e kwadrant een laterotrusie of lateroprotrusie
plaatsvindt.

11. 3 Occlusaal kompas toepassen
in het laboratorium
3.1 Inleiding
Om het occlusaal kompas toepasbaar te maken voor een laboratorium is LEF
nodig. Het woord LEF staat voor: Leerschool van Esthetiek en Functies.
Een Leerschool is erg belangrijk want: “wat men niet weet ziet men niet”.
Daarom is het belangrijk dat iemand die de stof beheerst de anderen
onderwijst. Wanneer we stap voor stap de vormgeving van body, randlijsten,
knobbels en fissuren doorspreken volgens een vast patroon, zal dit zich uiten in
de Esthetiek. Deze esthetische meerwaarde zal zich ook laten gelden in de
Functies, hierdoor zullen alle vrijruimtes aanwezig en is er een goede
slijmverdeling in het occlusale vlak van de elementen.
3.2 Kennis in het hele laboratorium
Wanneer een laboratorium optimaal het occlusaal kompas wil toepassen dan
dienen alle medewerkers het concept te kennen. Elke handeling in de
tandtechniek is van belang om het occlusaal kompas te kunnen uitvoeren. Om
het occlusaal kompas goed te willen doorvoeren moeten we ons afvragen welke
kennis op welke afdeling nodig is.
We kunnen de belangrijkste aandacht punten van het occlusaal kompas in drie
fases verwoorden om afdelingsgericht te kunnen werken.
Per afdeling zal ik beschrijven hoe we dit het beste kunnen aanpakken.

12. 3.3 Fase I
Fase I is het begin van de tandtechniek oftewel de ingangscontrole. Hier worden
de afdrukken gecontroleerd op vertrekkingen en luchtbellen. Wanneer dit
correct is worden hier de modellen van vervaardigd. Ook moeten deze modellen
worden gecontroleerd op vertrekkingen en luchtbellen. Vanaf hier kan ook
worden gekeken of er voldoende geprepareerd is voor de ruimte die nodig is.
Voor porseleinwerkstukken is dit bij voorkeur minimaal 1,5 mm ruimte voor
onderstructuur en het porselein.
Voor goudkronen is dit minimaal 0,7 mm ruimte.
Vaak zien we dit soort preparaties. Buccaal en
linguaal is de ruimte redelijk, maar occlusaal
kunnen we absoluut geen mooi fissuur inleggen.
Wanneer de preparatie occlusaal niet naar wens is leg de tandarts dan aan de
hand van een preparatievoorbeeld uit waar meer ruimte gewenst is. Dit is voor
het laboratorium fijner werken en het geeft de tandarts het genot van mooiere
kronen en minder inslijpen.
Het bekendste voorbeeld hiervan betreft de ondermolaren waar een kroon
moet komen. Als daar occlusaal te weinig ruimte is, in het midden fissuur, zal de
antagonist moeten worden ingeslepen. Dit is dan ook nog een dragende
knobbel in de bovenkaak.
Het verschil is duidelijk zichtbaar: occlusaal waar het
hoofdfissuur van de kroon ligt is er meer ruimte en
ook zijn de toppen van de knobbels meer afgerond.
Dit is voor de tandarts maar even werk, maar het
maakt de kronen veel mooier en het kan veel
tijdswinst opleveren omdat inslijpen van de
antagonist niet aan de orde is.
Als dit alles doorgenomen is komen we bij het
ingipsen. Om alle bewegingen goed te kunnen
nabootsen is het ingipsen van de modellen zeer
belangrijk.
We letten hierbij op het incisief punt van de
bovenkaak, deze moet precies in het midden van
de articulator zijn. Een handig hulpmiddel is het
incisaal pinnetje dat in de incisaalpen
gestoken kan worden.

13. Ook dient er met ingipsen gelet te worden
op het vlak van Occlusie. Deze ligt parallel
aan het vlak van Camper, en heeft een
hoek van 15° met de Frankfurter
Horizontale. Het ingipsen met een
beetregistratie is natuurlijk optimaal.
Nog een controle punt is de ∆ van
Bonwill. Alle zijden zijn even lang.
De articulatorinstellingen zijn:
1. bennetthoek 15°
2. condylusbaan 30°
3. incisaalpen op nul.
Doordat de modellen nu perfect in de articulator staan zijn de
articulatiemogelijkheden goed controleerbaar.
Om de articulatie goed in de articulator
te kunnen geleiden gebruiken we het
geleidingsplaatje dat door middel van
de incisaalpen de dynamiek laat zien
van het functiepatroon.
Wanneer we de beet van een patiënt stabiel willen houden mogen we nooit aan
dragende knobbels slijpen, want de centrale occlusie rust op de dragende
knobbels en de articulatie gaat altijd over de dragende knobbels.

14. 3.4 Fase II
Hierbij gelden alle punten van Fase I, alleen met extra toevoegingen.
Bij Fase II weten we: - van elk element hoe de randlijsten eruit zien,
- het aantal knobbels per element,
- de verschillende knobbelhoogtes.
Al deze kennis is nodig voor degene die onderstructuren modelleert of scant.
De modelleur dient een mooi gevormde onderstructuur te modelleren met de
juiste knobbelhoogtes en mesiaal en distaal voldoende ruimte.
De medewerker die de scanner bedient heeft met een beeldscherm te maken
waar hij op modelleert, hierdoor is de modellatie niet tastbaar en is het moeilijk
de antagonist te bekijken. Echter wordt erbij het Zeno-Tec systeem perfect op
ingespeeld. Ook moeten we de onderstructuren uitbouwen en de hoogtes van
knobbels aanpassen. Wanneer we kennis van zaken hebben, wordt het
modelleren al een stuk eenvoudiger.
Hieronder zijn de randlijstvormen van de boven- en onderkaak zichtbaar.

15. Hieronder zien we bovenkaak met daarop de bewegingskleuren.
Hierdoor zijn alle knobbelhoogteverschillen ook te beredeneren.

de 2e molaar distaal boven valt in de distale fossa van de 2e molaar onder.

17. 3.5 Fase III
Hierbij gelden alle punten van Fase I en II, alleen met extra toevoegingen.
Bij het occlusaal kompas is het altijd zo dat de protrusie (zwart) in de bovenkaak
naar mesiaal is gericht. Dit komt omdat de bovenkaak star is en de onderkaak
als enige kaak kan bewegen. Dus bij een protrusie gaat de onderkaak naar
voren en betekent dat de knobbel van de bovenkaak vanuit de centrale fossa
naar mesiaal gaat.
De laterotrusie is in de bovenkaak altijd naar buccaal en in de onderkaak altijd
linguaal.
De lateroprotrusie is altijd tussen de laterotrusie en de protrusie in.
De protrusie is in de bovenkaak altijd naar mesiaal en in de onderkaak altijd
naar distaal.
De mediotrusie is in de bovenkaak altijd naar mesio-palatinaal gericht en in de
onderkaak altijd disto-buccaal. Een makkelijk ezelsbruggetje is: “dragende over
dragende knobbel.”
De twee rode vlakken zijn de grensbewegingen.
Het ene vlak is de ISS (immediate side-shift) die zich aan de mediotrusie en de
retrusie aansluit. Deze vormt zich:
• als er in het andere kwadrant, van dezelfde kaak, een lateroretrusie of

Het andere vlak is de RS (retrusive surtrusie) die zich aan de laterotrusie aansluit.
• Deze vormt zich als er in het andere kwadrant een mediotrusie
plaatsvindt.

18. 3.6 Gipsafdeling
Voor de gipstechnicus geldt fase I, deze hoeft niet het gehele concept te weten
omdat deze aan het begin van het proces werkzaam is.
Op de gipsafdeling wordt bestudeerd of de modellen vertrekkings- en
luchtbelvrij zijn en of er genoeg ruimte is. In dit stadium kan de tandarts al
worden ingelicht over eventueel bijprepareren. Wanneer er moet worden
bijgeprepareerd kunnen we de modellen van de eerste afdruk bijwerken als
voorbeeld preparatie, dit is zeker raadzaam wanneer dit vaker voorkomt.
Ook worden de modellen, zoals in Fase I beschreven staat, ingegipst, zodat de
modellen perfect in de articulator staan en de articulatiebewegingen te
controleren zijn.
3.7 Modelleerafdeling
Voor de modelleur van onderstructuren geldt fase I en II, het hele concept is
niet nodig, maar wel de vormgeving qua randlijsten en knobbelhoogtes.
Op de modelleerafdeling verzorgt men dat de kappen en bruggen op de goede
hoogtes worden gemodelleerd, zodat de porseleinmassa een gelijke dikte heeft
naar occlusaal toe. Vaak wordt er een waskapje gemaakt met eventuele
dummy’s ertussen, zonder ook maar kennis te hebben van functie en
anatomische vormgeving. Als deze functies en vormgeving al niet in wasfase
aanwezig zijn wordt het voor de porselein technici wel heel moeilijk om deze
wel toe te passen.
Voor de modelleur die naast onderstructuren ook goudkronen modelleert geldt
hetzelfde als het porselein werk, fase I, II en III, want ook hier wordt het
occlusaal kompas toegepast.
3.8 Goudafdeling
Voor de goudtechnicus geldt fase I en II. Hij controleert zijn werk op hoogte en
verzorgt het kauwvlak van de goudkronen. Alle medewerkers moeten het
occlusale kompas kennen omdat ze een schakel zijn, die de collega’s
controleren.
3.9 Porseleinafdeling
De porseleintechnicus dient het hele proces te begrijpen om het toe te kunnen
passen in zijn werkstukken. Daarom dient hij de materie geheel tot zich te
nemen.

19. 4 Occlusaal kompas
toepassen bij de tandarts
Het occlusaal kompas is een concept dat subliem is voor de tandarts. Dit
betekent dat de tandarts van het laboratorium het perfecte werkstuk mag
verwachten, waarin alle functies vertegenwoordigd zijn en dat esthetisch zeer
fraai is.
Wil een tandarts volgens het occlusaal kompas werken, dan dient hij aan een
paar eisen te voldoen.
1. Hij moet een volledige afdruk van boven- en onderkaak maken.
2. Hij mag nooit een dragende knobbel inslijpen.
3. Er moet genoeg ruimte voor een fraai esthetisch kauwvlak zijn.
4. Hij moet een goede noodvoorziening verzorgen.
En voor een nog fraaier resultaat voegen we er nog een aantal punten bij.
5. Hij moet onder de gingiva, ruime champfer of schouder prepareren.
6. Hij moet de beet vastleggen, indien mogelijk, voor het preparen.
7. Hij moet de outline goed vrijleggen.
Dit betekent in sommige gevallen dat de tandarts een geheel nieuwe werkwijze
gaat gebruiken. Van oudsher maakte hij snelle afdrukken en moest hij
regelmatig inslijpen. Nu kan hij precieze afdrukken maken, zonder vertrekkingen
en luchtbellen, met een duidelijke gingiva, met genoeg ruimte tussen preparatie
en antagonist. Bovendien zal hij nooit meer de dragende knobbels inslijpen,
omdat dit de stabiliteit van de kaak is.
Maar ook met heel specifieke dingen dient men rekening te houden. Wanneer
er wordt afgedrukt en de mond is 20mm of verder open, zullen de kwadranten
van de onderkaak automatisch naar elkaar toe gaan.
Dit betekent ook dat de tandtechnicus de vrijheid heeft om storende factoren in
te slijpen, hierbij denken we aan uitgegroeide elementen en composiet-
vullingen.

20. 5 Problemen oplossen van
tandarts en laboratorium
Het occlusaal kompas is het perfecte concept voor de patiënt, maar hierbij is
een goede samenwerking en openheid tussen tandarts en laboratorium vereist.
Wanneer we met dit concept werken zal dit hoe dan ook van weerskanten
vragen opleveren, zoals:
• waarom geeft een tandarts geen ruimte?
• waarom slijpt de technicus in, terwijl er ruimte is?
• waarom is de distale knobbel van de 1e bovenmolaar hoger dan de
mesiale knobbel?
Om al deze vragen te beantwoorden is er op de website www.tandtechniek.eu
een vraagbaak in het leven geroepen in de vorm van een forum, waarop overleg
mogelijk is tussen tandtechnici en tandartsen voor het hoogst haalbare
resultaat. Verder is er informatie over het occlusaal kompas vermeld op
www.occlusaal-kompas.nl.
Eén van de voorwaarden voor een goede samenwerking, is dat beide partijen
open staan voor opbouwende kritiek.

6.1
Beschrijving van de
elementen
Inleiding
We hebben gezien in hoofdstuk 2 dat het occlusaal kompas relatief makkelijk te
leren is. Daarbij is het van groot belang om alle elementen bij langs te gaan om
de functies, die zich uiten in de vorm, te beduiden. Er is zoveel informatie dat
enige duidelijkheid op zijn plaats is.
Let hierbij op dat we uitgaan van de normale situatie.

30. 7 Slotwoord
In deze scriptie heb ik uitgelegd dat de stappen van het occlusaal kompas
logisch en doordacht zijn. Ze zorgen ervoor dat natuurlijke functie en
vormgeving benaderd kunnen worden.
Als men per afdeling bekijkt welke fase ervoor bedoeld is, zal het kroon- en
brugwerk verbeteren.
Bij het laboratorium waar ik werk hebben we dit concept op de
porseleinafdeling al toegepast en de volgende stap is om het op het hele
laboratorium toe te passen. Het is wel een heel lang proces, maar zeker de
moeite waard. Voorheen kende iedereen de vormgeving via natuurlijke
voorbeeldmodellen, maar de echte reden hiervoor was niet bekend. Zoals ik in
het begin van de scriptie al zei: “wat men niet weet ziet men niet”.
Nu we het occlusaal kompas hebben toegepast weten we waarom elementen
specifieke kenmerken hebben. Dit heeft als gevolg dat de vormgeving niet
alleen te beredeneren is maar dat er zelfs meer kenmerken naar voren komen
die nooit zichtbaar waren.
