Leadership can be described as the conduct of a person when guiding the activities of a given group towards a common goal. The main concept of roles of leadership is based on impacting group activities and adjusting to change Sullivan, E. J. (2013. There is a challenge when considering leadership in the healthcare context because most theories are not formed within the healthcare framework for they were normally established for business contexts and later applied to the healthcare. Management can simply be defined as the process that managers utilize to realize organizational goals. It’s the process of achieving organizational set goals through the available resources.
How do management and leadership activities contribute to the success of change initiatives:
The activities involved in this process include planning, staffing, organizing, directing, controlling, and making decisions in the operations of the system to attain anticipated result and improve its entire performance. Management involves a combination of steps to follow to achieve the set requirements. Leaders look for suitable ways of doing things for instance they will establish the goals and purposes for the people (American Nurses Association, 2010; Sullivan, 2013). Anyone can be a leader without necessarily being in an authority position.
Transformational leadership is necessary along with good management of staff and resources. This entails committing individuals to action, converting those under you into leaders and converting the leaders themselves into change agents. It has less to do with use of power to pressure and suppress others in order to achieve result. Instead, it involves empowering them to understand and own the vision of the organization and trusting them to work towards the goals that profit not just themselves but the organization in general (Sullivan, 2013). In nursing empowerment would result to enhanced patient care, reduced sick days, and less attrition.
Through this, the staff will have higher job satisfaction and there will be higher retention of staff amongst there nursing functions.
What change projects are needed in your agency at this time?
Among the changes required is to ensure nurses practice to the full capacity of their education and training and programs to be formed to ensure nurses attain higher training to much the growing demands in the sector. These can only be achieved through good leadership and proper management. INTRODUCTION
Quality health care “means doing the right thing at the right time, in the right way, for the right person, and getting the best possible results.”1 The term quality, by definition, can mean excellence, status, or grade; thus, it can be measured and quantified. The patient, and perhaps the patient's family, may interpret quality health care differently from the way that health care providers interpret it. Therefore, it is important to determine—if possible—what is “right” and what is “wrong” with regard to quality health care. The study and analysis of health care are important to maintain a level of quality that is satisfactory to all parties involved. As a result of the current focus on patient safety, and in an attempt to reduce deaths and complications, providing the best quality health care while maintaining cost controls has become a challenge to all involved. Current quality initiatives are multifaceted and include government-directed, private sectorsupported, and consumer-driven projects.
This chapter explores the historical development of health care quality including a review of the important pioneers and the tools they developed. Their work has been studied, refined, and widely used in a variety of applications related to performance-improvement activities. Risk management is discussed, with emphasis on the importance of coordination with quality activities. The evolution of utilization management is also reviewed, with a focus on its relationship to quality management.

As humans, we are always looking to improve on what we have, innovations are created by trying new ideas. Problems are solved with changes that are implemented from the current system. Implementation of the change needs to be thoroughly monitored for the smooth transition from the established practice. There are various steps to be followed to execute a change in the established practice. Formulation of the idea, evaluation of the formed idea, through planning, implementation of the planned idea, reviewing the response of the new proactive and validation of the new practice. The purpose of this paper is to point out a growing problem in the unit I am employed in and to suggest a change for the better of the unit which is realistic in nature.
Problem
I work in the Emergency Room, specifically the Observation unit, the patient turnover is extremely high, in turn the stress levels can also be extremely high. Staffing shortage has become a growing problem in our unit thus leading to excess work and decreased quality of care in the unit. Staffing shortage is also taking a toll in the managerial level because the managers are now being forced to hire travel nurses to fill the vacant spots temporarily and this is leading to excess cost for the unit and organization(Bisk,2016). Nursing shortage is created because an increased number of nurses are leaving because of dissatisfaction about the increased workload, and the patient ratio(Marquis,2015). The staff will be asked to float to other floors on a regular basis due to the increased turn over on the unit. It is important for the organization to take steps to try to retain their staff nurses to improve patient satisfaction and employee satisfaction(Marquis,2015).
Realistic Change
A viable innovation or change that will improve satisfaction of nursing staff in the organization would be to allow the nurses to stay in their units and not have to float to other units so much. Scheduling or enough staff during the shift is also an important aspect that will improve patient safety and nurse satisfaction in the unit(Bisk,2016). Implementation of a float pool system will be a good idea to decrease the floating required for the full-time staff nurse. Floating the staff from an observation unit to an IMU unit (Intermediate Care) can lead to unsafe working situations for the nurse and the patient because of lack of experience with the acuity of the patient condition. A feasible solution to this problem is training new nurses on the frequently needy floor so that they will be well equipped to float to a unit that is short of staff, and provide them with incentives to float so that the nurse satisfaction will be on the rise. These incentives will be a good investment for the organization as it will help them retain the staff in the long run reducing costs of hiring temporary staff like travel nurses.

Change Proposal: Reimplementation of a Bedside Shift Report
The identified clinical problem that is relevant to nurses in the workplace is the bedside shift report (BSR). The bedside shift report may be used to help the development of teamwork, ownership, and accountability thus increasing the satisfaction of the nurses. The reason for selecting the reimplementation of a bedside shift report is because; at a targeted facility the implementation of the bedside shift report is becoming poor. This has led to the poor compliance with the standards of practice, and increase costs in the facility. Also, to have a reduced cost by removing overtime pay and scrubbing off legal errors which result in legal fees (Clark et al., 2020). (*Re-phrase this sentence, avoid slang).
The reimplementation of a bedside shift report is important for the nurse practicing in the organization. Bedside shift report assists in improving the experience of the client and improving the handoff care between the nurses through including the family and client in the delivery of care. Bedside shift report is leading to the introduction of a change in the nursing culture i.e. introduction of various methods of communicating patient-specific care at a shift transformation. Shift reports given at the bedside is helping in changing the manner in which nurses are communicating and practicing (Clark et al., 2020).
At the targeted facility, the execution of the bedside shift report is poor and this is associated to the lack of knowledge concerning the role of bedside shift reporting on the patient outcome. The healthcare providers at the facility are not directly engaged in the provision of healthcare. There is poor communication of the goals of the bedside shift reporting and their importance about the positive outcomes and benefits that are involved in the change (Clark et al., 2020).
According to the study by Gregory et al., (2014), the process of assessing the transformation in practice that involves transitioning of the bedside nurse shift handoffs is important. The evaluation process is helpful in the re-implementation plan to help in improving the areas of weaknesses. The evaluation process of the success in the implementation in the practice is assessing the perception of the nurses about the proposed changes i.e. the re-implementation plans. There is evidence showing the positive impacts of the bedside shift report in the safety of the patient, the control of the pain, the satisfaction of the patients, and improvement in the nurse to patient or nurse to nurse communication (Gregory et al., 2014).

Managed care refers to the practice of managing some aspects of the medical services i.e. the cost and the quality of the medical services. Managed care can be managed by healthcare providers when providing healthcare services. Utilization management refers to the processes or the activities that are involved in the management of care. for example Uutilization management is relied on in terms of managing the cost in the health plan. It also refers to the routine functions which are utilized for the management of the cost of the medical services. Utilization management is entailing aspects such as prospective, concurrent, and the retrospective review. A prospective review is involving the process of tackling the utilization before its occurrence; con-current review is addressing the utilization as it happens; whereas the retrospective review occurs after the fact. Case management, on the other hand, is relied on in delivering substantial savings. Stroke health care providers would have to learn how to use tele-health and there may be push back to using it due to health care providers typically using hands on assessment skills, they may not find assessing patients this way adequate. Finding a group of health care providers that are willing to start treating patients this way is the first step.

Changes in hospitals do not happen overnight. At my state run hospital it seems to take forever to get any changes made. Implementing education regarding how to reduce stroke readmissions would require research and then approval from many different committees to even be approved for implementation. Once approved then it has to be sent all to all hospital staff involved. Implementation of patient safety at Davis Health Care will not be an easy task as successful implementation is dependent on many factors going in the right manner. Following the proper criteria and allocating every person the necessary tasks required for successful implementation will be key for the plan to pull through. The board of directors of the health care facility will be responsible in ensuring that critical decisions are made pertaining to the implementation. They need to make decisions such as when is it best to start the implantation process.
They are responsible for appointing the executive leadership that will see through all of the implementation plans. They also need to assess the current situation of the healthcare so as to determine the best method to use to ensure quality improvement of the facility. The board needs to hold constant meetings throughout the time period of the implementation plan. The purpose of this meetings is to keep up to date on the current progress of the implementation plan. Any challenges that require their immediate attention will also be addressed during such meetings.
The executive leadership selected by the board is responsible for ensuring that plans are implemented successfully. They are the ones to guide the rest of the staff and team working on the implementation. The executive leadership has a responsibility to allocate duties and assign tasks to the most qualified personnel. They should also be aware of all parts taking course of the implementation process. They need to check regularly for progress updates from the personnel assigned to implement the necessary plans (Vincent, 2010).
The executive management is responsible for leading the rest of the personnel in the right direction. They also act as informants to the board of directors as to the progress of the implementation plan. The quality improvement committee is appointed by the executive leadership. These are the actual persons at the site implementing various patient safety improvement methods. The ethical implications of financial decisions that have a direct effect on nursing practice consist of the reduction in available money that is spent on staffing in order to ensure there are appropriate ratios at all times as well as cutting costs related to specific equipment and or tools needed to perform our jobs. Specific nurse to patient ratios have been implemented in some places however, it is not currently the norm regardless of numerous studies that have been conducted and shown that the higher the ratio the worse a patient's outcome. Although facilities will save a substantial amount of money when they cut down on staff, which is why they often choose to do so, an immoral and unethical act in and of itself, the end result effects the patients in often times very negative ways. If patients are having poor experiences they are either not likely to return because they are afraid the care that they receive will continue to be less than adequate or they will have to return due to complications that could have been prevented had there been an appropriate nurse to patient ratio when they were being cared for. As a study on this very subject has found "there is already a significant amount of empirical evidence showing the relationship between certain individual and organizational characteristics of hospital nursing and patient outcomes. These characteristics include nurses' level of education, patient-to-ratios, percentage of RNs among all nursing staff (skill mix), and the nurse practice environment" (Simonetti, 2019, p. 79).
Often times, more expensive equipment makes our jobs easier because it is more efficient and or effective. If we begin to "cut corners" in these ways it will undoubtedly have a direct impact on how well we are able to perform our jobs in certain situations. This is unethical because equipment could mean the difference between accuracy and efficiency among other things. This then means that it could then make or break a patient's outcome. If safety is compromised it is completely inappropriate to substitute equipment that might be unsafe thus putting the patient at an increased risk for illness or injury. This is not only incredibly unethical, it will have an all around negative impact on the facility's reputation and financial standing in the long run. Nurses should have a say in how money is spent because they are often times the most knowledgeable about all of the above. One article that looks at lifting equipment or lack there of states that "the results indicate that fewer than 12 percent of the responding nurses told us they have a "No Lift Policy". SBAR communication tool is an effective and simple communication method that works across all the healthcare disciplines to create a safer working environment. Müller et al. All the team member in the hospital identified the implementation of SBAR (Situation-Background-Assessment-Recommendation) as a solution for the persisting communication problem among caregivers, especially during patient handoffs within the hospital. The use of SBAR as a communication tool has been highly effective, reading the SBAR handoff note only allows you to know a lot about the patient situation and history. Implementing the evidence-based intervention is timely and in line with the organization's policies that prioritize patient safety and high-quality care delivery. Standardized processes, including SBAR, have effectively eradicated missing care information incidents and promoted information sharing among healthcare professionals. Nursing shortage is created because an increased number of nurses are leaving because of dissatisfaction about the increased workload, and the patient ratio(Marquis,2015). The staff will be asked to float to other floors on a regular basis due to the increased turn over on the unit. It is important for the organization to take steps to try to retain their staff nurses to improve patient satisfaction and employee satisfaction(Marquis,2015).
Realistic Change
A viable innovation or change that will improve satisfaction of nursing staff in the organization would be to allow the nurses to stay in their units and not have to float to other units so much. Scheduling or enough staff during the shift is also an important aspect that will improve patient safety and nurse satisfaction in the unit(Bisk,2016). Implementation of a float pool system will be a good idea to decrease the floating required for the full-time staff nurse. Floating the staff from an observation unit to an IMU unit (Intermediate Care) can lead to unsafe working situations for the nurse and the patient because of lack of experience with the acuity of the patient condition. A feasible solution to this problem is training new nurses on the frequently needy floor so that they will be well equipped to float to a unit that is short of staff, and provide them with incentives to float so that the nurse satisfaction will be on the rise. These incentives will be a good investment for the organization as it will help them retain the staff in the long run reducing costs of hiring temporary staff like travel nurses.

The identified clinical problem that is relevant to nurses in the workplace is the bedside shift report (BSR). The bedside shift report may be used to help the development of teamwork, ownership, and accountability thus increasing the satisfaction of the nurses. The reason for selecting the reimplementation of a bedside shift report is because; at a targeted facility the implementation of the bedside shift report is becoming poor. This has led to the poor compliance with the standards of practice, and increase costs in the facility. Also, to have a reduced cost by removing overtime pay and scrubbing off legal errors which result in legal fees (Clark et al., 2020). (*Re-phrase this sentence, avoid slang).

Bedside shift report assists in improving the experience of the client and improving the handoff care between the nurses through including the family and client in the delivery of care. Bedside shift report is leading to the introduction of a change in the nursing culture i.e. introduction of various methods of communicating patient-specific care at a shift transformation. Shift reports given at the bedside is helping in changing the manner in which nurses are communicating and practicing (Clark et al., 2020).
At the targeted facility, the execution of the bedside shift report is poor and this is associated to the lack of knowledge concerning the role of bedside shift reporting on the patient outcome. The healthcare providers at the facility are not directly engaged in the provision of healthcare. There is poor communication of the goals of the bedside shift reporting and their importance about the positive outcomes and benefits that are involved in the change (Clark et al., 2020).
According to the study by Gregory et al., (2014), the process of assessing the transformation in practice that involves transitioning of the bedside nurse shift handoffs is important. The evaluation process is helpful in the re-implementation plan to help in improving the areas of weaknesses. The evaluation process of the success in the implementation in the practice is assessing the perception of the nurses about the proposed changes i.e. the re-implementation plans. There is evidence showing the positive impacts of the bedside shift report in the safety of the patient, the control of the pain, the satisfaction of the patients, and improvement in the nurse to patient or nurse to nurse communication (Gregory et al., 2014).

Managed care refers to the practice of managing some aspects of the medical services i.e. the cost and the quality of the medical services. Managed care can be managed by healthcare providers when providing healthcare services. Stroke patients require adequate follow-up care with their health provider team, tele-health is a great way to provide these follow-up appointments however stroke patients may not be able to navigate computers to be able to do these appointments as they frequently have deficits.
Stroke health care providers would have to learn how to use tele-health and there may be push back to using it due to health care providers typically using hands on assessment skills, they may not find assessing patients this way adequate. Finding a group of health care providers that are willing to start treating patients this way is the first step.
Institutional structures
Changes in hospitals do not happen overnight. At my state run hospital it seems to take forever to get any changes made. Implementing education regarding how to reduce stroke readmissions would require research and then approval from many different committees to even be approved for implementation. Once approved then it has to be sent all to all hospital staff involved. Examples of committees that a hospital will have and that any changes would have to go through are finance, safety and quality, strategic planning, and audit and compliance committee (Price, 2018).

Implementation of patient safety at Davis Health Care will not be an easy task as successful implementation is dependent on many factors going in the right manner. Following the proper criteria and allocating every person the necessary tasks required for successful implementation will be key for the plan to pull through. The board of directors of the health care facility will be responsible in ensuring that critical decisions are made pertaining to the implementation. They need to make decisions such as when is it best to start the implantation process.
They are responsible for appointing the executive leadership that will see through all of the implementation plans. They also need to assess the current situation of the healthcare so as to determine the best method to use to ensure quality improvement of the facility. The board needs to hold constant meetings throughout the time period of the implementation plan. The purpose of this meetings is to keep up to date on the current progress of the implementation plan. Any challenges that require their immediate attention will also be addressed during such meetings.
The executive leadership selected by the board is responsible for ensuring that plans are implemented successfully. They are the ones to guide the rest of the staff and team working on the implementation. The executive leadership has a responsibility to allocate duties and assign tasks to the most qualified personnel. They should also be aware of all parts taking course of the implementation process. They need to check regularly for progress updates from the personnel assigned to implement the necessary plans (Vincent, 2010).
The executive management is responsible for leading the rest of the personnel in the right direction. They also act as informants to the board of directors as to the progress of the implementation plan. The quality improvement committee is appointed by the executive leadership. These are the actual persons at the site implementing various patient safety improvement methods. They assess the progress as well as any risks that may occur during implementation and report these to the executive leadership. Their sole task is on quality improvement.

The ethical implications of financial decisions that have a direct effect on nursing practice consist of the reduction in available money that is spent on staffing in order to ensure there are appropriate ratios at all times as well as cutting costs related to specific equipment and or tools needed to perform our jobs. Specific nurse to patient ratios have been implemented in some places however, it is not currently the norm regardless of numerous studies that have been conducted and shown that the higher the ratio the worse a patient's outcome. Although facilities will save a substantial amount of money when they cut down on staff, which is why they often choose to do so, an immoral and unethical act in and of itself, the end result effects the patients in often times very negative ways. If patients are having poor experiences they are either not likely to return because they are afraid the care that they receive will continue to be less than adequate or they will have to return due to complications that could have been prevented had there been an appropriate nurse to patient ratio when they were being cared for. As a study on this very subject has found "there is already a significant amount of empirical evidence showing the relationship between certain individual and organizational characteristics of hospital nursing and patient outcomes. These characteristics include nurses' level of education, patient-to-ratios, percentage of RNs among all nursing staff (skill mix), and the nurse practice environment" (Simonetti, 2019, p. 79).
Often times, more expensive equipment makes our jobs easier because it is more efficient and or effective. If we begin to "cut corners" in these ways it will undoubtedly have a direct impact on how well we are able to perform our jobs in certain situations. This is unethical because equipment could mean the difference between accuracy and efficiency among other things. This then means that it could then make or break a patient's outcome. If safety is compromised it is completely inappropriate to substitute equipment that might be unsafe thus putting the patient at an increased risk for illness or injury. This is not only incredibly unethical, it will have an all around negative impact on the facility's reputation and financial standing in the long run. Nurses should have a say in how money is spent because they are often times the most knowledgeable about all of the above. SBAR communication tool is an effective and simple communication method that works across all the healthcare disciplines to create a safer working environment. Müller et al. (2018) reveal that using SBAR enhances efficacy, accuracy, and efficiency of handoff reports which boosts patient safety by improving communication and encouraging patient involvement Ineffective, absent, or inadequate communication leading to patient safety concerns has become a big problem in many care facilities, including where I work. (2019), implementing a well-developed SBAR program of communication grounded on learning theory improves nurses' communication skills and boosts their confidence during patient handover. All the team member in the hospital identified the implementation of SBAR (Situation-Background-Assessment-Recommendation) as a solution for the persisting communication problem among caregivers, especially during patient handoffs within the hospital. The use of SBAR as a communication tool has been highly effective, reading the SBAR handoff note only allows you to know a lot about the patient situation and history. Implementing the evidence-based intervention is timely and in line with the organization's policies that prioritize patient safety and high-quality care delivery. Standardized processes, including SBAR, have effectively eradicated missing care information incidents and promoted information sharing among healthcare professionals. Studies have established poor communication during patient handoffs as the main cause of misguided actions, lost information, and misinterpretation of critical care information (Uhm et al., 2019). American opinion has indeed shaped politic consequences, political interests, and policymaking. Even with little or no interest in policymaking and politics, the assumption of democracy gives the citizens the power to freely air out their issues and give their opinion in matters of political concern. Taking the war in Iraq, it posed a significant economic and political imbalance. However, support from the politicians was negligible. And because a majority of the Americans opposed the war in Iran, they voted for a Democratic congressional candidate. Their opinion played a great deal in making concrete policies in response to the war in Iraq.
Public opinion is a reflection of the citizens’ view on how the government responds to national politics. Political actions are driven by the citizen’s opinion (Erikson, & Tedin, 2015). It sheds light on the outcomes of specific policies and helps the political candidates identify the characters demanded of them by the citizens. Political scholars argued that the perception of old public opinions was changed because of ambiguity and inaccuracy (Dür, 2019). Modern theories came to identify public opinion as either latent or a broad expression. Latent opinions are formed on the spot, while broad expressions are opinions that had earlier been formed and remained stable (Cantril, 2015).
When convincing policymakers, it proves difficult, interest groups may indirectly influence public opinion. They can achieve this through the media, holding rallies, or handing out leaflets to the public (Dür, 2019). Because the citizens have little or no information on policymaking, they can easily be swayed by interest groups. Interest groups can, therefore, successfully source their support from public opinion or not.
Public opinion remains relevant in American politics. Journalists, politicians, and political scientists should focus on getting the public’s opinion on state affairs. In as much as views might differ or change, establishing a common ground will help in policymaking (Dür, 2019). For the war in Iraq, the Democratic gained power over the senate and House. This was greatly influenced by the failure of public support that shifted the pro-Democratic in 2006 and the 2008 elections. Because opinions are not fixed, establishing a connection between public views and political outcomes might be impossible.

There is a challenge when considering leadership in the
healthcare context because most theories are not formed within
the healthcare framework for they were normally established for
business contexts and later applied to the healthcare.

utilize to realize organizational goals. It’s the process of

resources.
How do management and leadership activities contribute to the
success of change initiatives:
The activities involved in this process include planning,
staffing, organizing, directing, controlling, and making
decisions in the operations of the system to attain anticipated
result and improve its entire performance. Management involves
a combination of steps to follow to achieve the set
requirements. Leaders look for suitable ways of doing things for
instance they will establish the goals and purposes for the
people (American Nurses Association, 2010; Sullivan, 2013).
Anyone can be a leader without necessarily being in an
authority position.
Transformational leadership is necessary along with good
management of staff and resources. This entails committing
individuals to action, converting those under you into leaders
and converting the leaders themselves into change agents. It has
less to do with use of power to pressure and suppress others in
order to achieve result. Instead, it involves empowering them to
understand and own the vision of the organization and trusting
them to work towards the goals that profit not just themselves
but the organization in general (Sullivan, 2013). In nursing
empowerment would result to enhanced patient care, reduced
sick days, and less attrition.
Through this, the staff will have higher job satisfaction and
there will be higher retention of staff amongst there nursing
functions.

Among the changes required is to ensure nurses practice to the
full capacity of their education and training and programs to be
formed to ensure nurses attain higher training to much the

that practicing nurses lacked knowledge on Skin and wound
care management. Because of this, they also did not know how
to operate wound care equipment.
What Data did you use to Identify the Safety Issue?
Collection of data is important in the process of identifying a
safety issue. Once data have been collected; evaluation is done
so as to identify a safety issue and thereafter come up with ways
of addressing the safety issue.
In this case, the data used to identify the safety issue was the
number of nursing staff that participated in the educational
activities involving skin care issues and wound care
management. In addition, nurses filled questionnaires in the on
skin and wound care management processes; skin and wound
care management systems and equipments. Results from the
evaluation of these questionnaires informed the conclusion that
lack of proper knowledge of skin and wound care management
was indeed a safety issue.
How will the Practicum Change Project Address the Safety
Issue?
The Practicum Change Project will address this safety issue
firstly by procurement of hospital equipment to be used for skin
care and wound management. The institution’s procurement
system would also be adjusted to involve the top management
more in the process.
Secondly, setting up a system through which all nurses and their

Conclusion
Finally, clear and constant communication will be maintained as
this will be the most important step in addressing the safety
matter. This will ensure that the management keeps abreast on
matters of skin care and wound management. Thus, the nurses
will be constantly reminded and sensitized on the requirements
and importance of the issue to the success of the change
project. Current wound care expertise encompasses numerous
dressing-related skills including, treating the cause of the
wound and addressing patient centered concerns to set the stage
for local wound care, Properly assessing the wound and
identifying the dressing requirements, selecting dressings based
on their form and function for an individual wound’s need, and
meeting setting-specific requirements for dressing change
frequency and maintenance. There is always a safety risk if the
nurse is not able to follow the above step, when wrong dressing
changing material is used; when wound is not properly staged
and poor wound information is provided to the patient.
Dressings alone will not promote wound healing, unless the
underlying cause(s) for the wound are also addressed (e.g.
treatment of the wound cause, blood supply, nutrition, patient
centered concerns, local wound care etc.). As the wound
changes, the plan of care must change and dressing products
may have to be changed and this is where effective

Implementing a change that improves the way an organization
performs work may sometimes come with the requirement of
heavy resource inputs. Without the proper checks and balances,
the change may counterproductive in terms of economy. In the
light of this, this paper evaluates the budgetary estimates for the
proposed change at the United Healthcare Group.
In a bid to reduce the readmission rates in the United Healthcare
Group, it is imperative that follow-up calls are made to the
released patients. The successful implementation of the change
calls for additional resources in a number of areas. The
resources have to be allocated their respective amounts, which
should reflect in their different budgets, such as capital
budgeting, salary budget, and the operating budget (Sullivan,
2012).
The change project involves the implementation of post-
discharge calls to monitor patients, hence prevent readmissions.
Thus, the hospital needs resources that include personnel,
equipment, technology, and travel in some instances.
Additionally, Sullivan (2012) argues that employee motivation
and continual development helps improve the job performance.
As such, the hospital will be required to put additional
resources for the purposes of employee motivation and
education.
Telephone calls will require the installation of an additional
four telephone lines in the hospital, specifically for the follow-
up calls. The hospital will incur the purchase cost of $500 for
the phones. The installation cost will amount to $100. The
maintenance and the subscription fees for the phones will be
about $300 per year. Thus, the hospital will incur a capital
budget of $600 for the telephone lines, acquisition, and $300
operating budget for the telephones.
The change involves keeping in touch with discharge patients
8. through telephone calls. As such, the hospital will need to
employ additional staff, in the form of nurses, who will
supplement the existing body nurses in assessing the patients to
be put on the follow-up program and making the calls. In this
area, the hospital will need to add an additional two nurses.
These will reinforce the existing workforce, who will be used to
make the required follow-up calls and offer the necessary
instructions. Additional tasks require the addition of staff
(Sullivan, 2012). The salaries of the staff will be about $8400
for the additional staff. Thus, the hospital will incur an
additional $8400 salary budget for the program.
Education for the nursing staff will be required so as they are
acquainted with communicating with the patients, assessing
their condition remotely, making critical decisions and offering
the necessary instructions to the patients. Thus, a seminar to
educate the nurses on phone etiquette, the questions to ask and
the kind of information to guide on will be held. This exercise
will cost the hospital about $1000, for a week’s education of the
nurses at the hospital premises. The change entails the
introduction of tasks that the nurses were not accustomed to,
which requires the enrichment of their knowledge (Sullivan,
2012).
The major objective of the project is to reduce the instances of
hospital readmissions for the patients. As such, the nurses may
be called to task by visiting some of the patients in their homes,
depending on their conditions. Such cases are expected to be
very few. As such, the hospital will set aside $1000 per year for
such visits as the need arise.

This project is very interesting to me because as nurses, we face
are faced with the challenge preventing skin tear and breakdown
with incontinent patients. Continuous exposure to urine and
feces is one of the most common causes of skin breakdown. The
chemical present in these waste products have an effect on the
skin making it fragile and causing it to break and injure easily,
so changing patient’s position and keeping them dry is very
crucial in preventing this problem. But because of cost, proper
method are not utilize. Even though the chux are less expensive
but the frequent use of barrier ointment or moisture sealants
available in the market which are helpful for they form a
protective barrier on the skin can become expensive, retraining
staff members and new employees on skincare and prevention of
decubs can also incur some expenses.

Change in an integral process for any organization aiming to
continually the quality of its service delivery. This paper
presents how effective communication and collaboration with
11. other stakeholders are critical to the implementation of a change
at the United Healthcare Group, which entails the introduction
of post-discharge calls aimed at preventing further patient

At the multidisciplinary meeting to evaluate the hospital how
the institution could improve the inpatients’ experience at the
hospital teamwork. The institution’s medical director acted as
the meeting coordinator, who employed a democratic leadership
style. The leader allowed the input of every participant in the
discussion. Conflicts in the form of disagreements were solved
through compromise (Losa Iglesias & Bengoa Vallejo, 2012).
The decision and counterarguments were based on the principles
of nursing. At the end, the group proposed a plan to improve the
patient experience at the institution.
The interview with the director of the pharmacy division of the
institution proved much insightful. The director describes
himself as a servant leader, who is focused on the achievement
of the hospital goals and upholding of its values (Sullivan,
2012). The leader argued that communication is critical to
influencing the workforce, which is why he endeavors to pass
important messages through face-to-face communication. The
leader believes in the uniqueness of every member of his
department and works towards harnessing their different
strengths.
The proposed change at the institution calls for the involvement
of every stakeholder; from the top-level management, nursing
staff to the patients. The Rogers Theory will help implement the
change in phases, which will enable its evaluation and
modification (Sullivan, 2012). The theory enables the change to
be tried, and its benefits realized which would influence its
adoption (Sullivan, 2012). The multidisciplinary team will help
refine the change to make it compatible with every department,
which will help in its swift adoption. The best way to
communicate the project is through the multidisciplinary team
meetings. The leaders will then relay the change to their staff
after the discussions, which will facilitate its support by the

Change is inevitable in any sphere of life. In the healthcare
sector, change is even more critical. The dynamic nature of
technologies, the healthcare sector, and health care
requirements necessitate for any healthcare institution review
its practice and adopt the best available. This paper addresses
change in a hospital setup.
Several change theories exist, which help implement varying
nature of changes in hospitals (Sullivan, 2012). The most useful
theory of change applied to my place of practice, The United
Healthcare Group, is the Roger’s theory. The Roger’s Theory,
also referred to as the Diffusion of Innovation Theory, was
advanced by Rogers and entails a description of the process by
which people in any setup respond to change (Sullivan, 2012).
The Theory describes five steps towards the adoption of a
change, which comprise the creation of awareness stage,
interest creation, evaluation of effectiveness, implementation
and the adoption stages (Sullivan, 2012).
My project entails the introduction of post-discharge calls to
monitor the patients and help prevent hospital readmissions.
The Roger’s Change Theory allows a stepwise evaluation of the
project progress. This helps the change process to be modified
as necessary during the implementation process.
The United Healthcare Group’s mission is to provide people
with healthier lives and make convenient the health care system
cost effectively. The Group seeks to uphold integrity,
compassion, relationships, performance and innovation as its
core values. The vision to provide a high value of service to
their clients and become the medical center of choice guides the
group. The change aims to improve the patients’ health while
reducing incidences of re-hospitalization. This is in line with
