 OCCUPATION: Owns a footwear
store
 ADDRESS: Hoskote, Bengaluru

3. CHIEF COMPLAINTS:
Complains of diminision of vision in BE
since childhood, increased since 2-3
years

4.  HISTORY OF PRESENT ILLNESS:
Patient gives history of diminision of vision in BE
since childhood which was gradual in onset,
progressive, painless in nature, increased towards
evening, which has increased in the last 2-3
years.
It was associated with photophobia which was
present occasionally.

5.  No H/O diplopia/coloured halos
 No H/O sudden loss of vision
 No H/O trauma/pain in the eye
 No H/O ocular surgeries
 No H/O discharge/watering from
the eye

6. PAST HISTORY:
 Not a K/C/O Diabetes
mellitus/Hypertension/TB/Asthma
 No H/O prolonged intake of any
systemic or topical medications
 No H/O ocular surgeries

7. FAMILY HISTORY:
 H/O similar complaints in three of
the family members.

8.  No H/O diabetes, hypertension,
tuberculosis, malignancies,
arthritis, bowel problems in any of
the family members.

9. PERSONAL HISTORY:
 Diet : Mixed
 Sleep : Regular
 Appetite: Normal
 Bowel and bladder: Regular

10. GENERAL PHYSICAL
EXAMINATION
 A 35 year old male, well built and
nourished.
 He is conscious, co-operative, well
oriented to time place and person.
 Pulse - 78 bpm
 BP: 120/80 mm of Hg
 Afebrile

11. SYSTEMIC EXAMINATION
 Respiratory system: B/L NVBS
heard, no added sounds.
 CVS: S1,S2 heard, no murmurs
 CNS: HMF intact
 Per abdomen: soft, non tender, no
organomegaly

12. HEAD TO TOE EXAMINATION
 No pallor, icterus, cyanosis,
clubbing, lymphadenopathy, edema

13. OCULAR EXAMINATION
 Facial symmetry; Symmetrical
 Head posture : normal
 Forehead: normal
