Department as Senior Executive in M.R.D.Department.

Duties & Responsibility in Medical Record Department
 Medical Record Identification and numbering system.
 Monitoring of Medical Record Documentation.
 Qualitative and Quantitative checks of medical record as per NABH Standards.
 Collection, complication and presentation of statistical information of the patient to
the management.
 Reporting of notifiable diseases to the statutory authorities.
 Diagnostic and procedure coding by utilizing ICD 10 & ICD 9 CM.
 Support special studies for medical audit committees and clinical departments.
 Storage and retrieval of medical record, including record tracking.
 Assistance to Hospital authorities in complying with legal requirements like
Litigation, Court Commissions etc.
 Checking of Medical case sheets for deficiencies and reporting of non compliances to
hospital administration.
 Rectification an deficiencies in the case sheets from Wards & Departments
 Possessing of Case sheets for destruction after completion of Retention period.
 Scanning of case sheet uploading to servers.
 To keep back up and issue on line to user departments.
 Death files (RIP) and MLC (Medico Legal Cases) maintaining separate filing area
under lock and key.
