This study investigated drug resistance patterns among Mycobacterium tuberculosis isolates from 239 re-treatment tuberculosis patients in Sudan. 143 bacterial isolates were recovered, with 98.6% identified as M. tuberculosis complex and 1.4% as non-tuberculous mycobacteria. Drug susceptibility testing found that 38.3% of isolates were multidrug-resistant and a further 17% were resistant to a single first-line drug. Multidrug resistance was highest among patients who had previously failed treatment (75% of isolates), while over half of patients who had defaulted on previous treatment were susceptible to all first-line drugs. The findings suggest a considerable level of drug resistance in re-treatment TB patients in Sudan.
Meta analysis on the efficacy of foot-and-mouth disease하일 홍
This study aimed to summarize the efficacy of emergency foot-and-mouth disease (FMD) vaccination through a systematic review and meta-analysis of available literature. 31 studies were included that evaluated the clinical protection and virological protection provided by FMD vaccines in cattle, swine, and sheep. The meta-analysis found that emergency vaccination effectively protected livestock against both clinical signs of FMD and against FMD infection based on laboratory tests. No significant biases were found that would alter the conclusions. Meta-analysis can be a useful tool for summarizing vaccine efficacy results from multiple studies to help inform FMD control strategies.
A novel coronavirus associated with severe acute respiratory syndromeJuan Rubio
This document describes research conducted to identify the cause of a SARS outbreak in 2003. Researchers tested clinical samples from SARS patients for a variety of pathogens. They were able to isolate a novel coronavirus from patients meeting the SARS case definition. Electron microscopy revealed coronavirus-like particles. RT-PCR and sequencing confirmed it was a unique coronavirus strain. This provided evidence that a coronavirus was associated with the SARS outbreak. The researchers proposed naming the first isolate the "Urbani strain" after Dr. Carlo Urbani who died from SARS.

This document discusses the management of multidrug-resistant tuberculosis (MDR-TB) and the roles of pharmacists. It provides an overview of TB, defines various types of drug resistance, and reviews the epidemiology and standardized treatment regimen for MDR-TB. The roles of pharmacists in MDR-TB management include treatment monitoring to improve adherence, monitoring patient weight and side effects, and providing counseling to support treatment completion. Studies show better treatment outcomes when pharmacists are involved in TB patient care and management.
This document summarizes the antimicrobial stewardship program (AMS) implemented at a 500-bed tertiary care hospital. The AMS program focused on education, developing an antibiogram to guide empiric therapy, monitoring antibiotic use and resistance, and getting support from hospital management. An evaluation found high rates of appropriate initial antibiotic therapy in the medical ICU according to treatment protocols, with a quarter to third of patients having antibiotics de-escalated. Mortality and length of stay in the ICU were comparable to international standards. However, the program was limited to the medical ICU and surgical prophylaxis, and broader implementation and monitoring across the hospital remained a challenge.

Therapeutic Agents in Perioperative Third Molar Surgical proceduresAndres Cardona
This document provides a literature review on therapeutic agents used to minimize complications from third molar surgery. It discusses various chemotherapeutic agents including antibiotics, steroids, and other drugs. Antibiotics are commonly used preventatively or empirically before and after surgery. Timing of administration is important, with ideal timing being 30 minutes to 2 hours before surgery. Antibiotic choice depends on the likely organisms and surgical classification. Studies show variable postoperative infection rates ranging from 0.5-27%, with higher risks for mandibular extractions, increased surgery time, and complexity. Steroids help reduce inflammation and pain.
Medicinal Mushroom Preparations against Lung CancerNeven Jakopovic
In this cohort study, 13 patients with advanced small cell lung carcinoma and 52 with non-small cell lung carcinoma used medicinal mushroom extract (Dr Myko San company) from 2004 to 2007, and their status was assessed in 2009.
Using medicinal mushroom extracts with standard oncological therapy resulted in these significant dose-depended effects:
improved cancer survival and delayed mortality
decreases in tumor size
improved quality of life scores
when compared with standard therapy alone. Significant side effects or decreases in performance status, tolerance to therapy or outcome was not observed.
This work was presented by Dr. Ivan Jakopovic at the 5th International Medicinal Mushroom Conference in Nantong, China, in 2009.

This study examined the association between isolation of multi-drug resistant (MDR) Enterococcus species from the urine of hospitalized patients, co-morbidity status, and short-term mortality rate. Urine samples from 40 hospitalized patients yielded Enterococcus species, of which 36 (90%) were MDR. All 36 patients with MDR isolates had high co-morbidity scores and short-term mortality rates over 85% per year, and 17 patients expired. In contrast, the 4 patients without MDR isolates had no co-morbidities and were all alive at discharge. The results suggest isolation of MDR Enterococcus from urine may be associated with high co-morbidity and short-term mortality in hospitalized
ANTIMICROBIAL RESISTANCE PATTERNS AMONG ACINETOBACTER BAUMANNII ISOLATED FROM...inventionjournals
Acinetobacter baumannii (Ab) is a troublesome and increasingly problematic healthcareassociated pathogen, especially in critical care unit (ICU) and cardiovascular internal medicine (CIM). This organism has a capacity for long-term survival in the hospital environment. This study aimed to investigate the drug resistance patterns of Ab strains isolated from Thongnhat Dongnai General Hospital and the relationships between Ab isolations with clinical wards and year of patients. The antibiotic susceptibility of 279 Ab isolates for aminoglycosides, fluro-quinolons, cephalosporins, carbapenems, colistin and bactrim was determined using Kirby-Bauer disk diffusion method. The minimum inhibitor concentration (MIC) of 146 Ab isolates for Meropenem was determined using E-test method according to CLSI guide-line. A total 279 A. baumannii strains out of 1,976 positive isolates were collected from various specimens during study period. Among Ab-positive specimens, the most isolated specimen was sputum (26.6%, χ 2 =161.705 p<0.000),><0.000)><0.000),><0.000). Among 279 Ab isolated, resistance from 53.16% – 63.52% to aminoglycosides, 23.6% – 68.58% to fluroquinolons, 59.61% to 97.93% to cephalosporins, 60.27% to 80.7% to carbapenem, 10.53% to 66.48% to antibiotic combinations, 0.75% to colistin and 61.71% to bactrim. Among 146 multidrug-resistant Ab, 53.42% MICmeropenem ≥ 32 μg/ml and only 18.49% strains were susceptible to Meropenem. Due to the high antimicrobial resistance to two clinical wards (ICU and CIM) and carbapenems by disk agar diffusion test and E-test; we must focus on both a wiser use of antimicrobials and the prevention of infection. Continuous monitoring of antimicrobial susceptibility and strict adherence to infection prevention guidelines are essential to eliminate major outbreaks in the future.

ABSTRACT- This study was an attempt to estimate the prevalence of fungal isolates in superficial mycoses cases attending IPD and OPD of IIMS&R, Lucknow, Uttar Pradesh.A prospective study over a period of six (6) months was conducted from January 2015 to June 2015.The suspected cases of superficial mycoses were subjected to mycological examination with direct microscopy using 10%-40% KOH depending on the types of samples (skin, nail, hair) processed and culture on Sabouraud’s dextrose agar with chloramphenicol and cycloheximide (SDCCA) and also on Potato dextrose agar (PDA) medium. Causative agents were identified macroscopically and microscopically from the growth obtained on SDCCA and PDA. Direct microscopy revealed fungal elements in 78 (66.1%) cases whereas 54 (45.7%) were positive on culture. Out of 54 (45.7%) culture positive samples 6 (15%) were negative on microscopy (KOH mount). Tineacorporis 38 (32.2%) was the most common clinical types and male is to female ratio in relation to clinical types was 2.2:1. Commonest age group affected were 21-30 years with 41 (34.7%) cases. Males were predominantly affected 41 (75.9%) and male to female ratio being 3.1:1. 60% of the patients came from the rural background. College students formed a major chunk of the cases 29 (24.6%) followed by housewives 18 (15.3%) and unskilled workers 16 (13.6%). Trichophyton mentagrophytes 20 (37%) was the predominat isolate followed by T. tonsurans15 (27.7%), T. rubrum3 (5.5%), M. audouinii 3 (5.5%) and T. schoenleinii 2 (3.7%) with no Epidermophyton species. A non-pigmented variant of T. rubrum was identified in this study. Both SDCCA and PDA were found equally effective in isolating fungal isolates from clinical samples in our study. We are reporting change in frequency of dermatophytes isolated from superficial mycoses cases in our region.
Key words- Superficial mycoses, Non-pigmented variants, Dermatophytes
Antibiotic Dosing in critical care Catherine mc kenzieisakakinada
- Antibiotics should be administered within the first hour of recognizing severe sepsis or septic shock. Broad-spectrum antibiotics with good penetration of the suspected infection site should be used.
- The antibiotic regimen should be reassessed daily to optimize efficacy, prevent resistance, avoid toxicity, and minimize costs. Extended or continuous infusions of beta-lactam antibiotics may improve outcomes for critically ill patients, especially those with infection from less susceptible organisms.
- Proper dosing of antibiotics in critical care requires considering each patient's individual situation and balancing optimal treatment with minimizing harm from adverse events like toxicity.
