Welcome to the June Edition of Crisis Management’s RecallRegister, Aon’s monthly recall and product
safety newsletter. This publication provides a review of the month’s recalls as reported by the U.S.
Consumer Product Safety Commission (CPSC), the U.S Food and Drug Administration (FDA), the U.S.
Department of Agriculture (USDA), National Highway Traffic Safety Administration (NHTSA) and the
Canadian Food Inspection Agency (CFIA). In addition to recall announcements, RecallRegister
provides an update on the product recall and contamination insurance marketplace and
environment. Each month, we highlight issues of importance including new markets and capacity,
significant recall events and changes in legislation affecting the consumer products industry.

Largest Automotive Recall in History
Expands to 34 Million Vehicles
On May 19th, U.S. Transportation Secretary Anthony Foxx announced that air bag manufacturer
Takata has acknowledged that a defect exists in its air bag inflators. These inflators were made
with a propellant that can degrade over time and has led to ruptures that have been blamed for
six deaths worldwide. The action expands the number of vehicles to be recalled for defective
Takata inflators to nearly 34 million. This latest expansion of the Takata recall doubles the
original number of vehicles affected making it the largest automotive recall in U.S. history.
In all, eleven automakers have been involved in the recall.
It’s anticipated that the remedy of vehicles will be prioritized based upon risk, with the vehicles
that present the greatest risk in terms of age and geographic location to be serviced first.
Testing and investigation by Takata, auto manufacturers, and independent researchers have
not yet established a definitive root cause of the inflator malfunctions. NHTSA’s analysis of
test results and engineering reports from independent organizations points to moisture
infiltrating the defective inflators over extended periods of time as a factor. Over time, that
moisture causes changes in the structure of the chemical propellant that ignites when an air
bag deploys. The degraded propellant ignites too quickly, producing excess pressure that
causes the inflator to rupture and sends metal shards into the passenger cabin that can lead
to serious injury or death.
The National Highway Traffic Safety Administration (NHTSA) issued a Consent Order to Takata
requiring the company to cooperate in all future regulatory actions that NHTSA undertakes in
its ongoing investigation and oversight of Takata. In addition, NHTSA announced its intent to
begin a formal legal process to organize and prioritize the replacement of defective Takata
inflators under the agency’s legal authority.

Faster Tests for Listeria Detection Are Hitting the Market
Listeria is a very scary pathogen. Not only because
it is the third leading cause of death among
foodborne illnesses, but it also has the ability to
thrive at freezing temperatures and is extremely
difficulty to eradicate once it sets up camp in a food
processing plant. Listeria contamination has been
responsible for some of the largest food recalls
as of late, such as Blue Bell Creameries and Jeni’s
Splendid Ice Cream, among others.
However, long before these extremely publicized
incidents food manufacturers have been looking
for a faster way to detect Listeria monocytogenes.
Traditional means of detection could take up
to five days before obtaining the results from a
laboratory, but new methods are emerging.
One example is through a biosensor chip that can
detect Listeria in a food sample in less than three
minutes. Invented by Dr. Carmen Gomes of Texas
A&M’s Department of Biological and Agricultural
Engineering the technology can detect as little
as one bacterium in a one-ounce sample of food
and could further be developed to detect other
pathogens other than Listeria.
Another example is the Sample6 Detect/L.
Currently certified by the USDA and the
Association of Official Agricultural Chemists,
Sample6 is the first in-plant, in-shift pathogen
detection for Listeria. This technology significantly
cuts down the time required do a test by providing
in-house results so that an outside laboratory
is not needed. Within minutes, it can detect as
little as one cell of Listeria without enrichment,
automatically reading and recording results.
Although on-site testing won't remove the Listeria
contamination, if it’s manifestation could be
detected earlier, greater cost-effective measures
could be taken to remove its presence from the
food processing plant, thus increasing efficiency
and reducing the number of illnesses associated
with the pathogen.

May Recall Announcements
The following recalls have been announced through the U.S. Food and Drug Administration (FDA),
the U.S. Department of Agriculture (USDA), the Consumer Product Safety Commission (CPSC),
National Highway Traffic Safety Administration (NHTSA) and the Canadian Food Inspection
Agency (CFIA). These agencies are charged with protecting consumers from products that may pose
injury to the public. This is only a representative list of recalls announced by these agencies during

Kent Nutrition Group, Inc. Initiates Recall
of Poultry Feed
Kent Nutrition Group of Muscatine, Iowa
initiated a recall of its Blue Home Fresh Starter
Amp Medicated Poultry Feed due to an
excessive amount of salt in the feed
concerning lot number 1015072. While salt is
an essential nutrient for all animals, an excess
amount can be toxic to birds resulting in
thirst, difficulty breathing, fluid discharge
from the beak, wet droppings, weakness or
paralysis of the legs, and even death.
The recall was initiated after a routine
sampling by Kent Nutrition Group indicated
an excess amount of salt in the feed. The
feed being recalled was manufactured in
Columbus, Nebraska at lot number 1015072,
and was distributed to six dealerships in
Nebraska.
64 50lb bags of the poultry feed from lot
1015072 remains unaccounted for.
Consumers who have purchased Blue Seal
Home Fresh Starter Amp Medicated from lot
number 1015072 are urged to return them to
their Kent Dealer for a full refund.
IKEA Expands Recall of Crib Mattresses Due
to Risk of Entrapment
IKEA North America Services of
Conshohocken, Pennsylvania is recalling an
additional 131,000 SULTAN crib mattresses in
the U.S. and about 44,000 in Canada. The
recall is in addition to 169,000 mattresses
recalled in January of 2015 when IKEA
received two reports of infants becoming
entrapped between the mattress and the end
of the crib. No injuries have been reported.
Consumers should immediately stop using a
recalled mattress and should return it to any
IKEA store for an exchange or a full refund.
Poco Dolce Confections Announces Recall
of Individually Wrapped Tile Varieties
Poco Dolce Confection of San Francisco,
California is voluntarily recalling all Almond,
Aztec Chile, Burnt Caramel, Almond Coconut,
Ginger, and Sesame Toffee Individually
Wrapped Tiles because the label fails to
identify the food allergen milk in addition to
the listing for “Butter” in the labels ingredient
statement. People who have an allergy or
sensitivity to this specific type of allergen run
the risk of serious or life threatening allergic
reaction if they consume the product.
Consumers who have purchased the product
are urged to return them to the place of
purchase. No illnesses have been reported to
date in connection with this problem.
Felt Bicycles Recalls Cruiser Bicycles Due
to Crash Hazard
Felt Bicycles of Irvine, California is recalling
about 200 Felt Deep Six and El Guapo model
Cruiser bicycles due brake failure, posing a
crash hazard. Felt Bicycles has received 26
reports of incidents with the recalled models.
No injuries have been reported. Consumers
should immediately stop using the recalled
bicycles and contact your local Felt bicycle
deal for a free inspection and replacement of
the rear hub cog/driver.
Sun Rich Fresh Foods Inc. Recalls Apple
Slices Because of Possible Heath Risk
Sun Rich Fresh Foods of Richmond, BC,
Canada announced a voluntary recall of sliced
apple and products containing sliced apples
from its Northeast Fresh Facility located in
Brampton, ON, Canada because they have
the potential to be contaminated with Listeria
monocytogenes. The bacteria can cause
serious and potentially fatal infections to
individuals with weakened immune systems.
The recall was initiated after a routine sample
by the Canadian Food Inspection Agency
(CFIA) found that the sliced apples possibly
contained the bacteria. The recalled apple
slice products were distributed to retail
stores, distributors and food service
establishments in several states.
Individuals who purchased apple slices or
products containing sliced apples should
dispose of or return the product to the place
of sale. No illness has been reported to date.

Teva Parenteral Medicines Initiates
Voluntary Nationwide Recall of Select Lots
of Adrucil Due to Particulate Matter
Teva Parenteral Medicines initiated a
voluntary recall of eight lots of Adrucil
because of the potential presence of
particulate matter identified as silicone
rubber pieces from diaphragm and
fluorouracil crystals. Administration of a
contaminated product with particulate
matter has the potential to result in
inflammation, allergic reactions, or blockage
of blood vessels.
Teva has distributed Adrucil through the
usual chain of wholesalers, retailers, and
pharmacies throughout the United States.
Customers should immediately stop use and
distribution, and contact Teva as how to
return the product to them. To this date,
no injuries have been reported related to
this recall.

Consumer Alert: Undeclared Peanuts
in “Cumin Powder”
Routine sampling by New York State
Department of Agriculture and Markets’ food
inspectors and subsequent analysis of Crown
Food International’s “Cummin Powder”
revealed the product to contain peanut
allergens. Accordingly, New York State
Agriculture Commissioner Richard A. Ball
issued a consumer alert warning of the
undeclared peanuts in Crown Food
International “Cummin Powder.”
People who have severe sensitivity to peanuts
may run the risk of serious or life threatening
allergic reaction if they consume this
product. No illnesses have been reported to
date to this Department in connection with
this product.
Golden Krust Caribbean Bakery & Grill
Issues Allergy Alert on Undeclared Eggs in
Jamaican Style Shrimp and Soya Patties
Golden Krust Caribbean Bakery & Grill of
Bronx, New York is recalling 6,188 cases of
Shrimp and Soya Patties due to an undeclared
egg allergen. People who have an allergy or
severe sensitivity to eggs run the risk of
serious or life-threatening allergic reaction
if they consume these products.
The recall was initiated after Golden Krust
discovered that product containing egg was
distributed in packaging that did not reveal
the presence of the egg.
Consumers who have purchased Shrimp
and Soya Patties are urged to dispose of the
product. No illness has been reported to
date in connection with this problem.

Snow Peak Recalls Japanese Axe Due to
Laceration and Impact Hazards
Snow Peak USA of Portland, Oregon recalled
about 220 of the Japanese Axe M because the
handle of the axe can crack, posing a
laceration or impact hazard to the user or
bystanders. Snow Peak has received three
reports of a cracked handle or loose head on
the axe. No injuries have been reported.
Consumers should immediately stop using
the recalled Japanese Axe M and contact
Snow Peak for a free replacement axe.
Power Reclining Furniture Recalled by
Franklin Due to Fire Hazard; Power Switch
Can Fail
Franklin Corporation of Houston, Mississippi
recalled about 81,000 power reclining
furniture because the switch used to operate
the reclining function can overheat, posing a
fire hazard to consumers. There has been
two reports of switches igniting and three
reports of switches smoking on showroom
samples. No injuries have been reported.
Consumers should immediately unplug the
recalled units and contact Franklin for a
replacement operating switch.

Good Seed Inc Recalls Soybean Sprouts
& Mung Bean Sprouts Due to Possible
Health Risk
Good Seed Inc. of Springfield, Virginia is
recalling all packages of soybean sprouts and
mung bean sprouts because they have the
potential to be contaminated with Listeria
monocytogenes.
The contamination was discovered after
sampling by the Virginia Department of
Agriculture and Consumer Services Food
Safety Program and subsequent analysis by
the Virginia Division of Consolidated
Laboratory Services revealed the presence
of the bacteria in the product. The recalled
products were distributed to retail stores
in Virginia, Maryland, New Jersey and
North Carolina.
Individuals who have purchased the product
can return the soybean sprouts and mung
bean sprouts to the place of sale for a full
refund. No illnesses have been reported to
date in connection with this problem.
Baby’s Dream Recalls Cribs and Furniture
Due to Violation of Lead Paint Standard
Baby’s Dream Furniture Inc. of Buena Vista,
Georgia recalled about 4,600 cribs and
furniture pieces because the vintage grey
pain on the cribs, furniture, and accessories
exceeds federal lead limits. If ingested, lead
can cause adverse health effects. There have
no injuries reported. Consumers should
immediately contact Baby’s Dream to arrange
for an exchange.

Schylling Recalls Police Press and Go Toy
Vehicles Due to Choking Hazard
Schylling Inc. of Rowley, Massachusetts
recalled about 13,200 Police Press and Go
Toy vehicles in the U.S. and 2,100 units in
Canada because the hat can detach from the
policeman’s head, posing a choking hazard to
young children. Schylling has received one
report of the police hat detaching. No
injuries have been reported.
La-Z-Boy Recalls Wall Saver Power
Reclining Furniture Due to Fall Hazard
La-Z-Boy Incorporated of Monroe, Michigan
is recalling about 2,600 PowerReclineXRw
chairs because the furniture can tip forward if
the consumer exits without bring the chair to
an upright position, posing a fall hazard.
La-Z-Boy has received five dealer reports of
the furniture tipping forward. No injuries
have been reported. Consumers should stop
using the chair and contact the local deal
where the chair was purchased for a refund
or a repair.
IKEA Recalls Pressure-Mounted Safety
Gates Due to Fall Hazard
IKEA North America Services of
Conshohocken, Pennsylvania recalls about
58,000 PATRULL KLAMMA and PATRULL
SMIDIG safety games in the U.S. and 17,000
units in Canada because the friction between
the wall and the pressure-mounted safety
gate in insufficient to hold the gate in its
intended position, posing a fall hazard. There
have been 18 incidents worldwide, including
three in which children have been insured as
a result of falling down stairs. No injuries
have been reported in the U.S. Consumers
should stop using the safety gate and return
it to any IKEA store for a full refund.
Cost Plus World Market Recalls Twist Swivel
Stools Due to Fall Hazard
Cost Plus Management Services Inc. of
Oakland, California recalled about 125,000
Twist Swivel Stools because the weld joint
attaching the stool seat to the cent post can
break, posing a fall hazard. The firm has
received 12 reports of the stool’s joint
breaking. No injuries have been reported.
Consumers should immediately stop using
the recalled stool and return it to any Cost
Plus World Market for a full refund.
Avalanche Rescue Snow Shovels Recalled
by Mountain Safety Research Due to Risk
of Failure During Emergency Rescues
Mountain Safety Research recalled about
4,300 Avalanche rescue snow shovels in the
U.S. and 340 in Canada because the lower
lock button can fail to secure the blade,
causing the shovel’s shaft and blade to come
apart and render the shovel unable to be
used as intended. No incidents or injuries
have been reported. Consumers should
immediately stop using the recalled shovels
and return them to the place of purchase for
a full refund or to mountain Saftey Research
for a free replacement shaft.

Natural Creations Issues Allergy Alert on
Undeclared Milk Ingredient in Dietary
Supplement New Zealand Colostrum
Natural Creations of Woodbine, Iowa is
recalling a small quantity of Natural Creations
New Zealand Colostrum because the product
was distributed in packaging that did not
inform consumers that the product is milk-
based. Natural Creations was notified of
potential risk from a contract manufacturer.
People who have an allergy or sever
sensitivity to milk protein run the risk of
serious allergic reaction if they consume this
product.
The recall was initiated after a contract
manufacturer notified Natural Creations of
the potential risk. Consumers who have
purchased Natural Creations New Zealand
Colostrum are urged to return it to the place
of purchase for a full refund. No illnesses
have been reported to date.

OC Raw Dog Recalls Turkey & Produce
Raw Frozen Canine Formulation Because of
Possible Salmonella Health Risk
OC Raw Dog of Rancho Santa Margarita,
California is voluntarily recalling 2055 lbs. of
Turkey & Produce Raw Frozen Canine
Formulation, because it has the potential to
be contaminated with Salmonella. Salmonella
can affect animals eating the products and
there is risk to humans from handling
contaminated pet products.
This recall is a result of a routine sampling
program by the Nebraska Department of
Food and Agriculture which revealed a
presumptive positive to Salmonella. These
products were distributed in Minnesota,
Missouri, Pennsylvania, and Colorado and
sold to consumers through independent pet
specialty retailers.
OC Raw Dog has ceased the production and
distribution of the product as FDA and the
company continues their investigation as to
what caused the problem. Consumers who
have purchased Turkey & Produce Raw
Frozen Canine Formulation should return the
products where they were purchased for a
full refund or replacement product.

Aurora Products, Inc. Announces
Voluntarily Recall of Aurora Natural
Brand and Certain Private Label Branded
Raw Macadamia Nuts Due to Possible
Health Risk
Aurora Products of Orange, Connecticut is
voluntarily recalling Raw Macadamia Nuts
packaged under the Aurora brand label and
various other store brand labels because of a
potential Salmonella contamination. The
bacteria can cause serious and sometimes
fatal infections in people with weakened
immune systems.
The recall was initiated after routine testing
by an outside company contracted by the
FDA revealed the presence of Salmonella in
one retail container of Raw Macadamia Nuts.
Distributors of the products should
immediately remove the product from the
shelves. Consumers should dispose of or
return the product to the place of purchase.
No illnesses have been reported to date.

TenPoint Crossbow Technologies Recalls to
Repair Crossbows Due to Injury Hazard
Hunter’s Manufacturing Company Inc., (dba
TenPoint CrossBow Technologies) of
Mogadore, Ohio recalled about 127,000
TenPoint and Wicked Ride crossbows because
after the safety has been re-engaged, the
crossbows can fire under certain
circumstances if a consumer pulls the trigger,
posing an injury hazard. TenPoint has
received 19 reports that arrows released from
the crossbows after the safety mechanism
was re-engaged. There have been no
reported injuries. Consumers should
immediately stop using the recalled
crossbows and contact TenPoint for detailed
instructions on how to receive a free repair.
Jose Madrid Salsa Issues Voluntary Recall
Jose Madrid Salsa of Zanesville, Ohio has
issued a voluntary recall of Jose Madrid Salsa
Original Mild Salsa Packaged in 13oz jars, due
to potentially improperly sealed jars.
Compromised seals or seams could result
in product contamination by spoilage
organisms or pathogens and may lead to
illness if consumed.
The recalled product has been distributed in
the Zanesville, Ohio and Baltimore, Maryland
areas. Customers are urged to return the
product to the place of purchase or to Jose
Madrid Salsa directly for either a replacement
jar or full refund. No illnesses have been
reported to date.

Rocky Brands Recalls Rocky Rubber Snake
Boots Due to Failure of Snake Guard
Rocky Brands of Nelsonville, Ohio is recalling
about 1800 hunting boots because the
boots’ snake guard can fail to protect the
wearer’s feet from a snake bite. Rocky Brands
reports one incident of the boots’ snake
guard failing in which a Rocky Brands
contractor sustained injuries from a snake
bite. Consumers should stop wearing the
recalled boots immediately and contact
Rocky Brands for a full refund or for a free
replacement pair of boots.
A&S Food Trading Inc. Issues an Alert on
Undeclared Sulfites in Hua Zhi Ying Brand
Gourd Candy
A&S Food Trading of Brooklyn, New York is
recalling Hua Zhi Ying Brand Gourd Candy
because it contains undeclared sulfites.
People who have severe sensitivity to sulfites
run the risk of serious or sometimes fatal
allergic reactions if they consume this
product.
The recall was initiated after a routine
sampling by New York State Department of
Agriculture and Markets Food Inspectors and
subsequent analysis of the product by Food
Laboratory personnel revealed the presence
of sulfites in package which were not
declared on the label. The product was
produced in China and distributed in the
New York State region.
Consumers should return purchased Hua Zhi
Ying Brand Gourd Candy to the place of
purchase. No illnesses have been reported
to date.
Kaldi’s Coffee Roasting Recalls Cup Sleeves
Due to Fire Hazard
Kladi’s Coffee Roasting Company of St. Louis,
Missouri recalled about 700,000 disposable
cup sleeves because it can ignite if used in a
microwave, posing a fire hazard. Kaldi’s has
received two reports of the cup sleeves
catching fire when heated in the microwave.
No injuries have been reported. Consumers
should immediately stop using and discard
the recalled cup sleeves.

Kenmore Elite Ranges Recalled by
Electrolux Due to Laceration Hazard
Electrolux Home Products of Charlotte, North
Carolina recalled about 5,300 Sears Kenmore
Elite slide-in Ranges because the stainless
steel trim below the range control panel can
have a sharp edge, posing a laceration hazard
to consumers. One consumer reported
needing stitches after being cut on the
stainless steel trim. Consumers should
immediately contact Sears for a free
inspection and repair.
Electrolux Recalls Kenmore Elite Ranges
Due to Fire and Burn Hazards
Electrolux Home Products of Charlotte, North
Carolina recalled about 250 Sears Kenmore
Elite dual fuel ranges because the burner
flame can go out while the gas is turned on.
This allows gas to escape and poses fire and
burn hazards. No incidents or injuries have
been reported. Consumers should
immediately stop using the ranges and
contact Sears for a free inspection and repair.

Valor Athletics Recalls Olympic Weight
Bench due to Injury Hazard
Valor Athletics of St. Petersburg, Florida
recalled 100 weight benches because the
weld joining from the leg to the main frame
can break, posing an injury hazard to the
user. No incidents or injuries have been
reported to date. Consumers should stop
using the bench and contact Valor Fitness for
a free repair.
Hewitt Recalls Marine Winches Due
to Injury Hazard
Hewitt Machine & Manufacturing of Nicollet,
Minnesota recalled about 1,700 chain driven
winches because the top and bottom roller
chains in the winch and break, causing the
winch and marine lift to fail. This can pose an
injury hazard to the user or bystander. There
have been six incidents of the roller chains
breaking in the recalled winch housing. No
injuries have been reported. Consumers
should immediately stop using the recalled
winches and contact Hewitt Machine &
Manufacturing for a free replacement of the
roller chains.

Viking Range Recalls Gas Ranges Due
to Burn Hazard
Viking Range LLC, of Greenwood, Minnesota
recalled 52,000 gas ranges in the U.S. and
about 8,300 in Canada because the ranges’
ovens can turn on by themselves, posing a
burn hazard to consumers. Viking Rang has
received 75 reports of gas ranges turning on
by themselves, including three reports of
burns and four reports of property damage
claims. Consumers should immediately
contact Viking Range to schedule a free
in-home repair.
OXO Recalls Nest Booster Seats Due to
Fall Hazard
OXO of El Paso, Texas recalled about 25,000
Nest Booster Seats in the U.S. and 130 in
Canada because the stitching on the restraint
straps can loosen which allows the straps to
separate from the seat, posing a fall hazard to
children. OXO has received five reports of
the stitching coming undone releasing the
straps. No injuries have been reported.
Consumers should immediately stop using
the Nest booster seats and contact OXO for a
free repair kid with redesigned safety straps.
Vivint Recalls Remotec Dimming Switch
Modules Due to Shock Hazard
Vivint, Inc. of Provo, Utah recalled about
213,000 dimming switch modules because
the module wiring is incorrectly installed,
posing a shock hazard to consumers. There
have been no incidents or injuries reported
to date. Consumers should contact Vivint
Inc. immediately for instructions for a free
replacement module.

Falcon Trading Company, Inc. dba
SunRidge Farms Is Voluntarily Recalling
Coconut-Medium Shred Due To Possible
Health Risk
Falcon Trading Company (also known as
SunRidge Farms) of Royal Oaks, California
announced a voluntarily recalling Coconut-
medium shred because the product may
contain very small stainless steel wire that is
used in the screening process during
manufacturing. Consumers who have
purchased the product should return to the
place of purchase.

Whole Foods Market's Oakland Store
Voluntarily Recalls Jivara Lactee Milk
Chocolate Valrhona Cut and Wrap Pieces
Due to Undeclared Tree Nut Allergen
Whole Foods Market’s Oakland, California
store is voluntarily recalling Jivara Lactee Milk
Chocolate Valrhona cut and wrap pieces due
to undeclared hazelnuts on the product
label. People who have an allergy or
sensitivity to hazelnuts run the risk of serious
or life threatening allergic reaction if they
consume the product.
This recall only applies to Whole Foods

Gerber Recalls Cohort Knives Due to
Laceration Hazard
Gerber Legendary Blades of Portland,
Oregon recalled about 150,000 Cohort
Folding Knives in the U.S. and about 11,000
in Canada because locking mechanism can
fail to hold the blade, posing a laceration
hazard. Gerber received six reports of
laceration injuries, two of which required
stitches. Consumers should stop using the
knife and contact Gerber for a replacement

Brown Betty Coconut Milk Powder Due to
Presence of Undeclared Milk
Bedessee Imports of Brooklyn, New York is
voluntarily recalling Brown Betty Coconut
Milk Powder Mix because the product fails to
identify a milk allergen on the label.
The recalled “Coconut Milk Powder” was sold
in several states and consumers should return
products back to the place of purchase for a
full refund. No illnesses have been reported
to date.

Terrain Vehicles Due to Violation of the
Federal ATV Standard
Bombardier Recreational Products of Canada
recalled about 12,500 youth ATVs because
the fail to meet the performance
requirements of the federal ATV standard for
maximum unrestricted speed and parking
brakes, posing a crash hazard. There have
been no incidents or injuries reported to
date. Consumers should immediately stop
using the recalled ATVs and contact a BRP
dealer to schedule a free repair.

Euro Import Distributions Inc. Issues
an Alert on Undeclared Sulfites in
Dried Apricots
Euro Import Distributions of Brooklyn, New
York announced a recall of dried apricots in
3.5 ounce plastic bags because the product
contains undeclared sulfites. People who
have severe sensitivity to sulfites run the risk
of serious or life-threatening allergic
reactions if they consume this product.
The recall was initiated after a routine
sampling by the New York State
Department of Agriculture and Markets
Food Inspectors and subsequent analysis
did not declare sulfites on the product’s
label. The dried apricots are a product of
Russia, and have been distributed in the
New York State region.
Consumers who have purchased Dried
Apricots should return it to the place of
purchase. No illnesses have been reported
to date.
Natural Grocers issues recall of Macadamia
Nuts due to possible health risk
Vitamin Cottage Natural Food Markets of
Lakewood, Colorado is recalling two lots of
Natural Grocers brand Macadamia Nuts
because the product has the potential to be
contaminated with Salmonella. Salmonella
can cause serious and sometimes fatal
infections in people with weakened
immune systems.
The recall was initiated after the FDA routine
sampling showed positive signs of Salmonella
in the product. The product was distributed
to Natural Grocers 96 stores located across
the United States.
Consumers who may have purchased this
product should return it to the store for
credit or refund. No illnesses have been
reported to date.

Nantucket Distributing Recalls Mason Jar
Night Light Due to Burn Hazard
Nantucket Distributing Company of
Middleboro, Massachusetts recalled about
12,000 Mason Jar Night Lights because the
plastic around the base of the light bulb can
melt, posing a burn hazard. There have
been two reported incidents involving the
night lights with the silver jar in which the
place around the base of the light bulb
melted. No injuries or property damage has
been reported. Consumers should
immediately stop using the recalled night
lights and return them to and Christmas
Tree Shops.
The Popcorn Factory, Inc. Issues Allergy
Alert on Undeclared Milk in Product
The Popcorn Factory of Lake Forest, Illinois
is voluntarily recalling 600 4oz bags of “Lite
Works Dill Pickle Popcorn” because it
potentially contains undeclared milk. .
The recall was initiated after it was discovered
that the wrong food product ingredients
were packaged in the foil bags during
production. Subsequent investigation
indicates the problem was caused by an error
in the company's production and packaging
processes. The product was distributed
nationally in Burlington Coat Factory retail
stores.
Customers who have purchased the product
are urged to discard the product, and may
contact The Popcorn Factory directly for a
full refund. No illnesses have been reported
to date.
Cycling Sports Group Recalls GT Fury
Mountain Bicycles Due to Crash, Injury
Hazards
Cycling Sports Group of Wilton, Connecticut
recalled about 160 GT downhill mountain
bicycles because the front wheel hub can
break and cause the disc brake system to fail,
posing crash and injury hazards to the
consumer. Cycling Sports Group has
received two reports of broken hubs. No
injuries have been reported. Consumers
should stop using the recalled bicycles and
return them to the nearest GT dealer to have
the complete front when replaced for free.
Great States Recalls Earthwise Cordless
Electric Lawn Mowers Due to Injury Hazard
The Great States Corporation of Shelbyville,
Indiana recalls about 1,200 cordless 17-inche
electric law mower because the mower can
start the key in not n that starting position,
posing and n=injury hazard to the user.
There have been no incidents or injuries
reported to date. Consumers should
immediately stop using the recalled
electric mowers, remove the battery and
contact Great State Corporation for a
replacement mower.

Undeclared Milk And Sulfite In Ice Cream
Bars And Popsicles
Paleteria La Jalpita of Pasco, Washington
issues a recall of ice cream bars and popsicles
because the product may contain undeclared
milk and sulfite.
The recall was initiated when a routine
inspection by Washington State Department
of Agriculture discovered the presence of
milk and sulfite which were not declared on
the label. The recalled ice cream bars and
popsicles were distributed through the state
of Oregon and Washington through retail
stores and direct delivery services.
Consumers who have purchased Paleteria La
Jalpita are urged not to consume products if
they have an allergy or severe sensitivity to
milk or sulfite and should return product to
the place of purchase for a full refund.

This publication provides a review of the month’s recalls as reported by the U.S. Consumer Product Safety Commission (CPSC), the U.S Food and Drug Administration (FDA), the U.S. Department of Agriculture (USDA), National Highway Traffic Safety Administration (NHTSA) and the Canadian Food Inspection Agency (CFIA). In addition to recall announcements, RecallRegister provides an update on the product recall and contamination insurance marketplace and environment. Each month, we highlight issues of importance including new markets and capacity, significant recall events and changes in legislation affecting the consumer products industry. Recall Register June, 2015 In this Issue 01 Largest Automotive Recall in History Expands to 34 Million Vehicles 02 Faster Tests for Listeria Detection Are Hitting the Market 03 May Recall Announcements Largest Automotive Recall in History Expands to 34 Million Vehicles On May 19th, U.S. Transportation Secretary Anthony Foxx announced that air bag manufacturer Takata has acknowledged that a defect exists in its air bag inflators. These inflators were made with a propellant that can degrade over time and has led to ruptures that have been blamed for six deaths worldwide. The action expands the number of vehicles to be recalled for defective Takata inflators to nearly 34 million. This latest expansion of the Takata recall doubles the original number of vehicles affected making it the largest automotive recall in U.S. history. In all, eleven automakers have been involved in the recall. It’s anticipated that the remedy of vehicles will be prioritized based upon risk, with the vehicles that present the greatest risk in terms of age and geographic location to be serviced first. Testing and investigation by Takata, auto manufacturers, and independent researchers have not yet established a definitive root cause of the inflator malfunctions. NHTSA’s analysis of test results and engineering reports from independent organizations points to moisture infiltrating the defective inflators over extended periods of time as a factor. Over time, that moisture causes changes in the structure of the chemical propellant that ignites when an air bag deploys. The degraded propellant ignites too quickly, producing excess pressure that causes the inflator to rupture and sends metal shards into the passenger cabin that can lead to serious injury or death. The National Highway Traffic Safety Administration (NHTSA) issued a Consent Order to Takata requiring the company to cooperate in all future regulatory actions that NHTSA undertakes in its ongoing investigation and oversight of Takata. In addition, NHTSA announced its intent to begin a formal legal process to organize and prioritize the replacement of defective Takata inflators under the agency’s legal authority.   • 2. Faster Tests for Listeria Detection Are Hitting the Market Listeria is a very scary pathogen. Not only because it is the third leading cause of death among foodborne illnesses, but it also has the ability to thrive at freezing temperatures and is extremely difficulty to eradicate once it sets up camp in a food processing plant. Listeria contamination has been responsible for some of the largest food recalls as of late, such as Blue Bell Creameries and Jeni’s Splendid Ice Cream, among others. However, long before these extremely publicized incidents food manufacturers have been looking for a faster way to detect Listeria monocytogenes. Traditional means of detection could take up to five days before obtaining the results from a laboratory, but new methods are emerging. One example is through a biosensor chip that can detect Listeria in a food sample in less than three minutes. Invented by Dr. Carmen Gomes of Texas A&M’s Department of Biological and Agricultural Engineering the technology can detect as little as one bacterium in a one-ounce sample of food and could further be developed to detect other pathogens other than Listeria. Another example is the Sample6 Detect/L. Currently certified by the USDA and the Association of Official Agricultural Chemists, Sample6 is the first in-plant, in-shift pathogen detection for Listeria. This technology significantly cuts down the time required do a test by providing in-house results so that an outside laboratory is not needed. Within minutes, it can detect as little as one cell of Listeria without enrichment, automatically reading and recording results. Although on-site testing won't remove the Listeria contamination, if it’s manifestation could be detected earlier, greater cost-effective measures could be taken to remove its presence from the food processing plant, thus increasing efficiency and reducing the number of illnesses associated with the pathogen. May Recall Announcements The following recalls have been announced through the U.S. Food and Drug Administration (FDA), the U.S. Department of Agriculture (USDA), the Consumer Product Safety Commission (CPSC), National Highway Traffic Safety Administration (NHTSA) and the Canadian Food Inspection Agency (CFIA). These agencies are charged with protecting consumers from products that may pose injury to the public. This is only a representative list of recalls announced by these agencies during the month. A complete list of recalls, as well as actual press releases, is available at www.recalls.gov and http://www.inspection.gc.ca/english/toce.shtml May 1, 2015 Kent Nutrition Group, Inc. Initiates Recall of Poultry Feed Kent Nutrition Group of Muscatine, Iowa initiated a recall of its Blue Home Fresh Starter Amp Medicated Poultry Feed due to an excessive amount of salt in the feed concerning lot number 1015072. While salt is an essential nutrient for all animals, an excess amount can be toxic to birds resulting in thirst, difficulty breathing, fluid discharge from the beak, wet droppings, weakness or paralysis of the legs, and even death. The recall was initiated after a routine sampling by Kent Nutrition Group indicated an excess amount of salt in the feed. The feed being recalled was manufactured in Columbus, Nebraska at lot number 1015072, and was distributed to six dealerships in Nebraska. 64 50lb bags of the poultry feed from lot 1015072 remains unaccounted for. Consumers who have purchased Blue Seal Home Fresh Starter Amp Medicated from lot number 1015072 are urged to return them to their Kent Dealer for a full refund. IKEA Expands Recall of Crib Mattresses Due to Risk of Entrapment IKEA North America Services of Conshohocken, Pennsylvania is recalling an additional 131,000 SULTAN crib mattresses in the U.S. and about 44,000 in Canada. The recall is in addition to 169,000 mattresses recalled in January of 2015 when IKEA received two reports of infants becoming entrapped between the mattress and the end of the crib. No injuries have been reported. Consumers should immediately stop using a recalled mattress and should return it to any IKEA store for an exchange or a full refund. Poco Dolce Confections Announces Recall of Individually Wrapped Tile Varieties Poco Dolce Confection of San Francisco, California is voluntarily recalling all Almond, Aztec Chile, Burnt Caramel, Almond Coconut, Ginger, and Sesame Toffee Individually Wrapped Tiles because the label fails to identify the food allergen milk in addition to the listing for “Butter” in the labels ingredient statement. People who have an allergy or sensitivity to this specific type of allergen run the risk of serious or life threatening allergic reaction if they consume the product. Consumers who have purchased the product are urged to return them to the place of purchase. No illnesses have been reported to date in connection with this problem. Felt Bicycles Recalls Cruiser Bicycles Due to Crash Hazard Felt Bicycles of Irvine, California is recalling about 200 Felt Deep Six and El Guapo model Cruiser bicycles due brake failure, posing a crash hazard. Felt Bicycles has received 26 reports of incidents with the recalled models. No injuries have been reported. Consumers should immediately stop using the recalled bicycles and contact your local Felt bicycle deal for a free inspection and replacement of the rear hub cog/driver. Sun Rich Fresh Foods Inc. Recalls Apple Slices Because of Possible Heath Risk Sun Rich Fresh Foods of Richmond, BC, Canada announced a voluntary recall of sliced apple and products containing sliced apples from its Northeast Fresh Facility located in Brampton, ON, Canada because they have the potential to be contaminated with Listeria monocytogenes. The bacteria can cause serious and potentially fatal infections to individuals with weakened immune systems. The recall was initiated after a routine sample by the Canadian Food Inspection Agency (CFIA) found that the sliced apples possibly contained the bacteria. The recalled apple slice products were distributed to retail stores, distributors and food service establishments in several states. Individuals who purchased apple slices or products containing sliced apples should dispose of or return the product to the place of sale. No illness has been reported to date. May 4, 2015 Teva Parenteral Medicines Initiates Voluntary Nationwide Recall of Select Lots of Adrucil Due to Particulate Matter Teva Parenteral Medicines initiated a voluntary recall of eight lots of Adrucil because of the potential presence of particulate matter identified as silicone rubber pieces from diaphragm and fluorouracil crystals. Administration of a contaminated product with particulate matter has the potential to result in inflammation, allergic reactions, or blockage of blood vessels. Teva has distributed Adrucil through the usual chain of wholesalers, retailers, and pharmacies throughout the United States. Customers should immediately stop use and distribution, and contact Teva as how to return the product to them. To this date, no injuries have been reported related to this recall. May 6, 2015 Consumer Alert: Undeclared Peanuts in “Cumin Powder” Routine sampling by New York State Department of Agriculture and Markets’ food inspectors and subsequent analysis of Crown Food International’s “Cummin Powder” revealed the product to contain peanut allergens. Accordingly, New York State Agriculture Commissioner Richard A. Ball issued a consumer alert warning of the undeclared peanuts in Crown Food International “Cummin Powder.” People who have severe sensitivity to peanuts may run the risk of serious or life threatening allergic reaction if they consume this product. No illnesses have been reported to date to this Department in connection with this product. Golden Krust Caribbean Bakery & Grill Issues Allergy Alert on Undeclared Eggs in Jamaican Style Shrimp and Soya Patties Golden Krust Caribbean Bakery & Grill of Bronx, New York is recalling 6,188 cases of Shrimp and Soya Patties due to an undeclared egg allergen. People who have an allergy or severe sensitivity to eggs run the risk of serious or life-threatening allergic reaction if they consume these products. The recall was initiated after Golden Krust discovered that product containing egg was distributed in packaging that did not reveal the presence of the egg. Consumers who have purchased Shrimp and Soya Patties are urged to dispose of the product. No illness has been reported to date in connection with this problem. May 7, 2015 Snow Peak Recalls Japanese Axe Due to Laceration and Impact Hazards Snow Peak USA of Portland, Oregon recalled about 220 of the Japanese Axe M because the handle of the axe can crack, posing a laceration or impact hazard to the user or bystanders. Snow Peak has received three reports of a cracked handle or loose head on the axe. No injuries have been reported. Consumers should immediately stop using the recalled Japanese Axe M and contact Snow Peak for a free replacement axe. Power Reclining Furniture Recalled by Franklin Due to Fire Hazard; Power Switch Can Fail Franklin Corporation of Houston, Mississippi recalled about 81,000 power reclining furniture because the switch used to operate the reclining function can overheat, posing a fire hazard to consumers. There has been two reports of switches igniting and three reports of switches smoking on showroom samples. No injuries have been reported. Consumers should immediately unplug the recalled units and contact Franklin for a replacement operating switch. May 8, 2015 Good Seed Inc Recalls Soybean Sprouts & Mung Bean Sprouts Due to Possible Health Risk Good Seed Inc. of Springfield, Virginia is recalling all packages of soybean sprouts and mung bean sprouts because they have the potential to be contaminated with Listeria monocytogenes. The contamination was discovered after sampling by the Virginia Department of Agriculture and Consumer Services Food Safety Program and subsequent analysis by the Virginia Division of Consolidated Laboratory Services revealed the presence of the bacteria in the product. The recalled products were distributed to retail stores in Virginia, Maryland, New Jersey and North Carolina. Individuals who have purchased the product can return the soybean sprouts and mung bean sprouts to the place of sale for a full refund. No illnesses have been reported to date in connection with this problem. Baby’s Dream Recalls Cribs and Furniture Due to Violation of Lead Paint Standard Baby’s Dream Furniture Inc. of Buena Vista, Georgia recalled about 4,600 cribs and furniture pieces because the vintage grey pain on the cribs, furniture, and accessories exceeds federal lead limits. If ingested, lead can cause adverse health effects. There have no injuries reported. Consumers should immediately contact Baby’s Dream to arrange for an exchange. May 12, 2015 Schylling Recalls Police Press and Go Toy Vehicles Due to Choking Hazard Schylling Inc. of Rowley, Massachusetts recalled about 13,200 Police Press and Go Toy vehicles in the U.S. and 2,100 units in Canada because the hat can detach from the policeman’s head, posing a choking hazard to young children. Schylling has received one report of the police hat detaching. No injuries have been reported. La-Z-Boy Recalls Wall Saver Power Reclining Furniture Due to Fall Hazard La-Z-Boy Incorporated of Monroe, Michigan is recalling about 2,600 PowerReclineXRw chairs because the furniture can tip forward if the consumer exits without bring the chair to an upright position, posing a fall hazard. La-Z-Boy has received five dealer reports of the furniture tipping forward. No injuries have been reported. Consumers should stop using the chair and contact the local deal where the chair was purchased for a refund or a repair. IKEA Recalls Pressure-Mounted Safety Gates Due to Fall Hazard IKEA North America Services of Conshohocken, Pennsylvania recalls about 58,000 PATRULL KLAMMA and PATRULL SMIDIG safety games in the U.S. and 17,000 units in Canada because the friction between the wall and the pressure-mounted safety gate in insufficient to hold the gate in its intended position, posing a fall hazard. There have been 18 incidents worldwide, including three in which children have been insured as a result of falling down stairs. No injuries have been reported in the U.S. Consumers should stop using the safety gate and return it to any IKEA store for a full refund. Cost Plus World Market Recalls Twist Swivel Stools Due to Fall Hazard Cost Plus Management Services Inc. of Oakland, California recalled about 125,000 Twist Swivel Stools because the weld joint attaching the stool seat to the cent post can break, posing a fall hazard. The firm has received 12 reports of the stool’s joint breaking. No injuries have been reported. Consumers should immediately stop using the recalled stool and return it to any Cost Plus World Market for a full refund. Avalanche Rescue Snow Shovels Recalled by Mountain Safety Research Due to Risk of Failure During Emergency Rescues Mountain Safety Research recalled about 4,300 Avalanche rescue snow shovels in the U.S. and 340 in Canada because the lower lock button can fail to secure the blade, causing the shovel’s shaft and blade to come apart and render the shovel unable to be used as intended. No incidents or injuries have been reported. Consumers should immediately stop using the recalled shovels and return them to the place of purchase for a full refund or to mountain Saftey Research for a free replacement shaft. May 14, 2015 Natural Creations Issues Allergy Alert on Undeclared Milk Ingredient in Dietary Supplement New Zealand Colostrum Natural Creations of Woodbine, Iowa is recalling a small quantity of Natural Creations New Zealand Colostrum because the product was distributed in packaging that did not inform consumers that the product is milk- based. Natural Creations was notified of potential risk from a contract manufacturer. People who have an allergy or sever sensitivity to milk protein run the risk of serious allergic reaction if they consume this product. The recall was initiated after a contract manufacturer notified Natural Creations of the potential risk. Consumers who have purchased Natural Creations New Zealand Colostrum are urged to return it to the place of purchase for a full refund. No illnesses have been reported to date. May 15, 2015 OC Raw Dog Recalls Turkey & Produce Raw Frozen Canine Formulation Because of Possible Salmonella Health Risk OC Raw Dog of Rancho Santa Margarita, California is voluntarily recalling 2055 lbs. of Turkey & Produce Raw Frozen Canine Formulation, because it has the potential to be contaminated with Salmonella. Salmonella can affect animals eating the products and there is risk to humans from handling contaminated pet products. This recall is a result of a routine sampling program by the Nebraska Department of Food and Agriculture which revealed a presumptive positive to Salmonella. These products were distributed in Minnesota, Missouri, Pennsylvania, and Colorado and sold to consumers through independent pet specialty retailers. OC Raw Dog has ceased the production and distribution of the product as FDA and the company continues their investigation as to what caused the problem. Consumers who have purchased Turkey & Produce Raw Frozen Canine Formulation should return the products where they were purchased for a full refund or replacement product. Announces Voluntarily Recall of Aurora Natural Brand and Certain Private Label Branded Raw Macadamia Nuts Due to Possible Health Risk Aurora Products of Orange, Connecticut is voluntarily recalling Raw Macadamia Nuts packaged under the Aurora brand label and various other store brand labels because of a potential Salmonella contamination. The bacteria can cause serious and sometimes fatal infections in people with weakened immune systems. The recall was initiated after routine testing by an outside company contracted by the FDA revealed the presence of Salmonella in one retail container of Raw Macadamia Nuts. Distributors of the products should immediately remove the product from the shelves. Consumers should dispose of or return the product to the place of purchase. No illnesses have been reported to date. May 18, 2015 TenPoint Crossbow Technologies Recalls to Repair Crossbows Due to Injury Hazard Hunter’s Manufacturing Company Inc., (dba TenPoint CrossBow Technologies) of Mogadore, Ohio recalled about 127,000 TenPoint and Wicked Ride crossbows because after the safety has been re-engaged, the crossbows can fire under certain circumstances if a consumer pulls the trigger, posing an injury hazard. TenPoint has received 19 reports that arrows released from the crossbows after the safety mechanism was re-engaged. There have been no reported injuries. Consumers should immediately stop using the recalled crossbows and contact TenPoint for detailed instructions on how to receive a free repair. Jose Madrid Salsa Issues Voluntary Recall Jose Madrid Salsa of Zanesville, Ohio has issued a voluntary recall of Jose Madrid Salsa Original Mild Salsa Packaged in 13oz jars, due to potentially improperly sealed jars. Compromised seals or seams could result in product contamination by spoilage organisms or pathogens and may lead to illness if consumed. The recalled product has been distributed in the Zanesville, Ohio and Baltimore, Maryland areas. Customers are urged to return the product to the place of purchase or to Jose Madrid Salsa directly for either a replacement jar or full refund. No illnesses have been reported to date. May 19, 2015 Rocky Brands Recalls Rocky Rubber Snake Boots Due to Failure of Snake Guard Rocky Brands of Nelsonville, Ohio is recalling about 1800 hunting boots because the boots’ snake guard can fail to protect the wearer’s feet from a snake bite. Rocky Brands reports one incident of the boots’ snake guard failing in which a Rocky Brands contractor sustained injuries from a snake bite. Consumers should stop wearing the recalled boots immediately and contact Rocky Brands for a full refund or for a free replacement pair of boots. A&S Food Trading Inc. People who have severe sensitivity to sulfites run the risk of serious or sometimes fatal allergic reactions if they consume this product. The recall was initiated after a routine sampling by New York State Department of Agriculture and Markets Food Inspectors and subsequent analysis of the product by Food Laboratory personnel revealed the presence of sulfites in package which were not declared on the label. The product was produced in China and distributed in the New York State region. Consumers should return purchased Hua Zhi Ying Brand Gourd Candy to the place of purchase. No illnesses have been reported to date. Kaldi’s Coffee Roasting Recalls Cup Sleeves Due to Fire Hazard Kladi’s Coffee Roasting Company of St. Louis, Missouri recalled about 700,000 disposable cup sleeves because it can ignite if used in a microwave, posing a fire hazard. Kaldi’s has received two reports of the cup sleeves catching fire when heated in the microwave. No injuries have been reported. Consumers should immediately stop using and discard the recalled cup sleeves. May 20, 2015 Kenmore Elite Ranges Recalled by Electrolux Due to Laceration Hazard Electrolux Home Products of Charlotte, North Carolina recalled about 5,300 Sears Kenmore Elite slide-in Ranges because the stainless steel trim below the range control panel can have a sharp edge, posing a laceration hazard to consumers. One consumer reported needing stitches after being cut on the stainless steel trim. Consumers should immediately contact Sears for a free inspection and repair. Electrolux Recalls Kenmore Elite Ranges Due to Fire and Burn Hazards Electrolux Home Products of Charlotte, North Carolina recalled about 250 Sears Kenmore Elite dual fuel ranges because the burner flame can go out while the gas is turned on. Viking Rang has received 75 reports of gas ranges turning on by themselves, including three reports of burns and four reports of property damage claims. OXO Recalls Nest Booster Seats Due to Fall Hazard OXO of El Paso, Texas recalled about 25,000 Nest Booster Seats in the U.S. and 130 in Canada because the stitching on the restraint straps can loosen which allows the straps to separate from the seat, posing a fall hazard to children. OXO has received five reports of the stitching coming undone releasing the straps. No injuries have been reported. Consumers should immediately stop using the Nest booster seats and contact OXO for a free repair kid with redesigned safety straps. Salmonella can cause serious and sometimes fatal infections in people with weakened immune systems. The recall was initiated after the FDA routine sampling showed positive signs of Salmonella in the product. The product was distributed to Natural Grocers 96 stores located across the United States. May 28, 2015 Nantucket Distributing Recalls Mason Jar Night Light Due to Burn Hazard Nantucket Distributing Company of Middleboro, Massachusetts recalled about 12,000 Mason Jar Night Lights because the plastic around the base of the light bulb can melt, posing a burn hazard. There have been two reported incidents involving the night lights with the silver jar in which the place around the base of the light bulb melted. No injuries or property damage has been reported. Issues Allergy Alert on Undeclared Milk in Product The Popcorn Factory of Lake Forest, Illinois is voluntarily recalling 600 4oz bags of “Lite Works Dill Pickle Popcorn” because it potentially contains undeclared milk. . The recall was initiated after it was discovered that the wrong food product ingredients were packaged in the foil bags during production. Subsequent investigation indicates the problem was caused by an error in the company's production and packaging processes. The product was distributed nationally in Burlington Coat Factory retail stores. The recall was initiated when a routine inspection by Washington State Department of Agriculture discovered the presence of milk and sulfite which were not declared on the label.
