This document analyzes factors contributing to high 30-day readmission rates at four hospitals within Meridian Health. It identifies the leading causes of readmissions for each hospital based on Diagnosis Related Groups. For the most common cause, heart failure, an intervention is proposed involving increased post-discharge follow up through phone calls, physician visits, and weekly in-office visits for four weeks. The effectiveness of this intervention will be evaluated by comparing readmission rates to a control group not receiving the additional follow up.
