1. STUDENT AGREEMENT
This contract entered into this ____ day of May 2011, in Quezon City, by and between:
Prime Movers Review Center (PMRC), a private educational institution that provides review classes for physical therapy (PT) and
occupational therapy (OT) students who are going to take the OT-PT board exam, operated by Therapeutix, Inc., with office address at
Second Floor Suite 204 Kaimo Condominium Building No. 101, Quezon Avenue, Quezon City; hereinafter referred to as the “Review
Center”;
— and —
___________________________ (name of PMRC reviewee), Filipino, of legal age, and with address at ______________________
hereinafter referred to as the “Student”. The Student is not authorized to act in behalf of the Review Center, unless otherwise specified
in this contract.
— WITNESSETH —
WHEREAS, the Review Center is engaged in providing lectures, small group discussions, practice exams, and mentoring for students
enrolled in the Review Center.
WHEREAS, the Student herein represents that he/she has the right to avail the lectures and discussions in anatomy, physiology,
kinesiology, medical and rehabilitiation lectures, physical therapy applications, and occupational therapy applications; and the Review
Center, by virtue of said representation, intends to provide the Student with these services.
NOW, FOR IN CONSIDERATION OF THE FOREGOING, the parties hereby agrees as follows:
I. TERMS AND CONDITIONS / ACCOUNTABILITIES
1. The Student shall be provided with the following materials upon enrollment in the Review Center:
1.1 Official Shirt of the Review Center
1.2 PMRC Reviewer entitled “Q&R Review for the OT and PT Board Examination”
1.3 Official ID (valid for 3 months)
2. The Student shall be provided with a mentor under a Mentoring Program suited to his/her needs to guide him/her throughout the
review season. The Mentoring Program will be scheduled upon the discretion of the Student’s assigned mentor. The mentor shall
make himself/herself available for consultations should the Student have any questions or concerns related to the review process.
3. The Student has the right to avail the lectures, small-group discussion (SGD), rationalization, and exam facilitation in the Review
Center. It is the responsibility of the Student to make up for his/her absences by asking the Review Center for handouts or review
materials provided by the Lecturer (as applicable).
4. The Student shall be given prior notice to any changes in the Academic Calendar:
4.1 Change in lecture, SGD faciliation, rationalization, and exam facilitation schedules;
4.2 Forfeiture of scheduled lecture;
4.3 Change in Lecturer in case of unexpected forfeiture of original Lecturer.
5. The Student must respect all the properties and equipment within the premises of the Review Center. Any form of destruction or
vandalism on the said properties shall be replaced by the Student who is responsible for such violation(s).
6. The Student shall regard with utmost confidentiality and respect the copyright of all the review materials provided by the Review
Center:
6.1 Lecturer’s Handouts
6.2 PMRC Review Book
6.3 Lecture Notes
6.4 Exam Papers
6.5 Academic Calendar
7. All records and information concerning the curriculum, academic calendar, PMRC students, and lecturers shall be the sole
property of the Review Center and shall be kept private. The Student shall not disclose the same, in any manner, to any person or
entity outside PMRC.
8. The Review Center has the right to acknowledge the names of students who has enrolled in the Review Center who eventually
pass or place in the OT-PT board exam by means of website, tarpaulin, social networking sites, brochures, and posters. They will
be acknowledged regardless if he/she takes the board exam one or more seasons after the time she has enrolled in the Review
Center, or if he/she ventures out to other review centers or in-house programs after reviewing at Prime Movers Review Center.
II. BREACH
1. Breach by the Student of any provision of this Contract shall entitle the Review Center to damages, forfeiture of service given and
other fees and renumerations due, and pre-termination of this Contract.

2. 2. Failure on the part of the Review Center to insist upon a stringent performance of any of the provisions of this Contract shall in no
way be regarded as a waiver of any right that the Center may have under the law or this Contract; nor shall it be construed as a
waiver of any subsequent breach or default by the Student of the provisions of this Contract. Any waiver of the Review Center
shall be expressly executed in writing, and signed by the Center (through the Executive Director).
III. DURATION
1. This Contract shall be effective until December 2012 unless renewed, in writing, by the parties concerned.
2. Either party may pre-terminate this Contract by serving at least two (2) months advance written notice to the other party
concerned.
3. Pre-termination of this Contract by the Review Center shall take effect upon service of notice to the Student.
IV. MISCELLANEOUS
1. This Contract supersedes and renders void any and all agreements and undertakings, oral and/or written, previously entered
between parties concerned. This contract may not be modified or altered except by instrument in writing duly signed by the
Student and the Director.
