Abstract
Background: Co-administration of multiple intravenous (IV) medicines down the
same lumen of an IV catheter is often necessary in the intensive care unit (ICU) while
ensuring medicine compatibility.
Aims and objectives: This study explores ICU nurses' views on the everyday practice
surrounding co-administration of multiple IV medicines down the same lumen.
Design: Qualitative study using focus group interviews.
Methods: Three focus groups were conducted with 20 ICU nurses across two hospi-
tals in the Thames Valley Critical Care Network, England. Participants' experience of
co-administration down the same lumen and means of assessing compatibility were
explored. All focus groups were recorded, transcribed verbatim, and analysed using
thematic analysis. Functional Resonance Analysis Method was used to provide a
visual representation of the co-administration process.
Results: Two key themes were identified as essential during the process of co-admin-
istration, namely, venous access and resources. Most nurses described insufficient
venous access and lack of compatibility data for commonly used medicines (eg, anal-
gesics and antibiotics) as particular challenges. Strategies such as obtaining additional

Background and Aims: Medication errors occur at any point of the medication management
process, and are a major cause of death and harm globally. The objective of this review was to
compare the effectiveness of different interventions in reducing prescribing, dispensing and
administration medication errors in acute medical and surgical settings.
Methods: The protocol for this systematic review was registered in PROSPERO
(CRD42019124587). The library databases, MEDLINE, CINAHL, EMBASE, PsycINFO, Cochrane
Database of Systematic Reviews and the Cochrane Central Register of Controlled Trials were
searched from inception to February 2019. Studies were included if they involved testing of an
intervention aimed at reducing medication errors in adult, acute medical or surgical settings.
Meta-analyses were performed to examine the effectiveness of intervention types.
Results: A total of 34 articles were included with 12 intervention types identified. Meta-analysis
showed that prescribing errors were reduced by pharmacist-led medication reconciliation,
computerised medication reconciliation, pharmacist partnership, prescriber education,
medication reconciliation by trained mentors and computerised physician order entry (CPOE)
as single interventions. Medication administration errors were reduced by CPOE and the use
of an automated drug distribution system as single interventions. Combined interventions
were also found to be effective in reducing prescribing or administration medication errors.
No interventions were found to reduce dispensing error rates. Most studies were conducted at
single-site hospitals, with chart review being the most common method for collecting medication

For a leading medical centre such as Mercy Medical Center, it is essential to have a sound policy and practice standards for the control of drug errors. The nursing staff of the medical centre who will be responsible for implementing the policy on managing medication errors must have a comprehensive understanding of the strategies specified in the policy for it to be appropriately implemented. A staff training program will ensure that the necessary information and skills for implementing the policy guidelines are effectively disseminated. The training curriculum outlined in this study will be administered to a pilot group of 20 pediatric nursing staff members at the medical facility.

According to Ruddy et al. (2016), technological advances are required for a significant change in medical practice but are not sufficient. When practitioners are modified, systemic alterations in practice occur. According to a study by French-Bravo et al., nurses felt that effective communication with nurse supervisors boosted their support for organizational objectives (2020). Utilizing a range of communication and persuasion techniques, nurse supervisors forged close bonds with nurses. In addition to email, huddles, and staff meetings, nurse managers used data in the form of statistics, facts, and patient feedback comments to explain the rationale for change. In addition to serving as role models for nurses, nurse supervisors influence unit culture by demonstrating characteristics such as approachability and attentiveness. In addition, nurse managers contributed to change by fostering employee engagement and management support through tactics such as listening to nurses' challenges and assisting them individually and as a team. Nurse managers support a staff-led decision-making strategy by supporting employees in comprehending unit objectives, empowering them to drive unit work, and enhancing manager-facilitated peer communication (French-Bravo et al., 2020). I decided to select physicians as health care providers and also as medical practitioners since they are always in demand in the medical field, day after another. This is a professional medical practitioner or by simple terms, a doctor who has completed highly advanced training in providing a range of mostly non-surgical health care to patients. A physician should have adverse knowledge in a medical specialty. The main concern is to maintain, restore, and promote health via the study, treatment, and diagnosis of a disease of patients to ensure their wellbeing.

For a leading medical centre such as Mercy Medical Center, it is essential to have a sound policy and practice standards for the control of drug errors. The nursing staff of the medical centre who will be responsible for implementing the policy on managing medication errors must have a comprehensive understanding of the strategies specified in the policy for it to be appropriately implemented. A staff training program will ensure that the necessary information and skills for implementing the policy guidelines are effectively disseminated. The training curriculum outlined in this study will be administered to a pilot group of 20 pediatric nursing staff members at the medical facility.
Promoting the Buy-In of Nurses
According to Ruddy et al. (2016), technological advances are required for a significant change in medical practice but are not sufficient. When practitioners are modified, systemic alterations in practice occur. According to a study by French-Bravo et al., nurses felt that effective communication with nurse supervisors boosted their support for organizational objectives (2020). Utilizing a range of communication and persuasion techniques, nurse supervisors forged close bonds with nurses. In addition to email, huddles, and staff meetings, nurse managers used data in the form of statistics, facts, and patient feedback comments to explain the rationale for change.
