This document provides information and exercises for a student to assess their body mass index (BMI) and develop a personal diet and exercise plan based on their BMI classification. It instructs the student to determine their BMI with help from a responsible adult at home. It then asks the student to record their BMI classification and reflect on what it indicates about their current health. Finally, it has the student list physical activities and develop a personal meal plan to improve or maintain a healthy BMI.
