The country has endured periods of low national morale, such as in the 1970s when high inflation and the energy crisis worsened public sentiment following the Vietnam War. The current mood, however, feels different. Gallup recently reported that national mental health is at an all-time low, with few bright spots to lift spirits.
To better understand how Americans are feeling and their attitudes towards mental health in general, ThinkNow conducted a nationally representative quantitative survey of 1,500 respondents and found some interesting differences among ethnic, age and gender groups.
Technology
For example, 52% agree that technology and social media have a negative impact on mental health, but when broken out by race, 61% of Whites felt technology had a negative effect, and only 48% of Hispanics thought it did.
While technology has helped us keep in touch with friends and family in faraway places, it appears to have degraded our ability to connect in person. Staying connected online is a double-edged sword since the same news feed that brings us pictures of the grandkids and fluffy kittens also feeds us news about the wars in Israel and Ukraine, the dysfunction in Washington, the latest mass shooting and the climate crisis.
Hispanics may have a built-in defense against the isolation technology breeds, owing to their large, multigenerational households, strong social support systems, and tendency to use social media to stay connected with relatives abroad.
Age and Gender
When asked how individuals rate their mental health, men rate it higher than women by 11 percentage points, and Baby Boomers rank it highest at 83%, saying it’s good or excellent vs. 57% of Gen Z saying the same.
Gen Z spends the most amount of time on social media, so the notion that social media negatively affects mental health appears to be correlated. Unfortunately, Gen Z is also the generation that’s least comfortable discussing mental health concerns with healthcare professionals. Only 40% of them state they’re comfortable discussing their issues with a professional compared to 60% of Millennials and 65% of Boomers.
Race Affects Attitudes
As seen in previous research conducted by ThinkNow, Asian Americans lag other groups when it comes to awareness of mental health issues. Twenty-four percent of Asian Americans believe that having a mental health issue is a sign of weakness compared to the 16% average for all groups. Asians are also considerably less likely to be aware of mental health services in their communities (42% vs. 55%) and most likely to seek out information on social media (51% vs. 35%).

Az alternatív beviteli kapuk közül a pulmonális terápia egyre elterjedtebbé válik, a bevitel nem fájdalmas, kevesebb hatóanyaggal is nagy helyi koncentráció érhető el és a tüdő fiziológiás sajátságai rendkívül gyors felszívódást eredményeznek. Hátrány lehet hogy a betegtől kooperációt igényel, de a megfelelő technika könnyen elsajátítható.
Aktuális feladat a különböző tüdőbetegségek lokális kezelése, hiszen incidenciájuk egyre emelkedik. Ezen felül megvalósítható szisztémás terápia is.

Az inhalált részecskék méretüktől függően eltérően viselkednek a légutakban. Impaktáció során az 5 mikrométernél nagyobb részecskék tehetetlenségük miatt kitapadnak a legelső helyen, ahol a levegőáram érintkezik a légutak falával.
Szedimentáció figyelhető meg az 1-5 mikrométeres mérettartományban, ami az eredményes pulmonális terápiához szükséges. Ennek a méretnek az elérésével érhetünk el depozíciót az alső légutakban. Diffuzót tapasztalunk az egy mikrométernél kisebb részecskéknél, ezek a nem hasznosulnak, kilélegzésre kerülnek.
A terápia során előtérbe kerülnek a száraz porinhalációs készítmények. Alkalmazásuk köszönhető kiemelkedő stabilitásuknak és egyszerű alkalmazásuknak. A hatóanyag porát a belélegzett levegőáram hozza mozgásba és juttatja a légutakba, nem igényelnek hajtógázt, így környezetbarátak. Formulálás szempontjából két nagyobb csoportot, klasszikus hordozó alapú és hordozó mentes rendszereket különböztetünk meg. A hordozó mentes készítményeknél nem egy különálló nagyméretű szemcsére viszik fel a hatóanyagot, hanem a megfelelő additívekkel alakítják ki a hatóanyagot is magába foglaló komplex rendszereket.
Célunk modern előállítási technológiával és korszerű segédanyagok alkalmazásával jó aerodinamikai tulajdonságokkal rendelkező hordozó mentes porinhalációs készítmény előállítása melyben a depozícióhoz szükséges 1-5 mikroméretes részecskék gyors felszabadulást biztosító nanoméretes hatóanyagszemcséket hordoznak. Lehetőséget adva súlyos tüdőbetegségek hatékony lokális kezelésére.
Modell anyagként a gyenge vízoldékonyságú, gyulladásgátló hatású meloxikámot használtuk, amely inhalálva alkalmas lehet nem-kissejtes tüdő karcinóma lokális kezelésére, valamint alkalmazható cisztás fibrózisban és krónikus obstruktív tüdőbetegségben is. Célunk mikrorészecskék eljuttatása az epitel sejtekig, majd azokból felszabaduló meloxikám nanorészecskék esetleges tumorellenes hatásának indukálása.
A minta előállításának első lépésében a kutatócsoport által optimalizált nedves őrléssel nanoszuszpenziót állítottunk elő. Kombinált technikát alkalmaztunk, bolygómalomban őröltük cirkónium gyöngyökkel a hatóanyagot 2,5%-os PVA oldatban. A PVA a Mx felületén adszorbeálódó polimer, mely stabilizálja az inhalációs por szerkezetét és mukoadhezív sajátsága révén a deponálódást biztosítja. A szuszpenzió szemcsemérete, nano mérettartományba esett.
Wet milling requires less energy and time than dry milling. Thanks to the environmentally isolated system, it is a dust free process and the material is less heated up. Disadvantages e.g., increased wear of the grinding medium, corrosion hazards, etc.
Második lépésként három különböző összetételű mintát formuláltunk porlasztva szárítással, ami léptéknövelhető és reprodukálható előállítási technika és kedvező aerodinamikai tulajdonságokkal rendelkező részecskék nyerhetők vele. Segédanyagként leucint alkalmaztunk, ami egy hidrofób oldalláncal rendelkező aminosav, mely csökkenti a részecskék közötti kohéziót, növelve ezzel a por diszperzitásfokát. A leucint növekvő koncentrációban alkalmaztuk, hogy megvizsgalhassuk milyen módon befolyásolja a végtermék fizikai kémiai sajatsagait. Megállapítottuk, hogy a leucin jelenlétében több mint 10 százalékkal emelkedett a kitermelés.
Termékeink kritikus minőségi paraméterek monitorozása során az inhalációs készítmények vizsgálati protokolljának előírása szerint haladtunk.
A minták karakterizálását először méret, sűrűség és morfológiai vizsgálatokkal végzetük. A lézer diffrakcióval végzett szemcseanalízis bebizonyította, hogy mikrokompozitok homodiszperz méreteloszlásúak. Mindhárom minta átlagos szemcsemérete 3-5 mikrométeres, ami a megfelelő tüdődepozícióhoz szükséges. Az elektronmikroszkópos felvételeken megfigyelhetőek hogy a közel szférikus porlasztott szemcsék nanométeres hatóanyag részecskékből épülnek fel. Sikerült kis sűrűségű termékeket produkálni, amely szintén a kedvező aerodinamikát vetít előre.
A termoanalitikai vizsgálatok azt mutatták, hogy az eredetileg kristályos szerkezetű hatóanyag részben amorfizálódott. A kristályossági fok normalizált integrálok alapján való meghatározásával megkaptuk, hogy az amorf frakció mértéke 93 százalék volt. A kis mennyiségű kristályos frakció olvadáspontja a kezelés hatására csökkent. Porröntgen diffrakciós mérésekkel szintén alátámasztottuk az amorfizációt. Ezt bizonyítja hogy a jellemző 2theta értékeknél található karakterisztikus csúcsok intenzitása lecsökkent.
Porkészítmények formulálásánál fontos az interpartikuláris kölcsönhatások tanulmányozása. A részecskék között fennálló kohézió segédanyagokkal befolyásolható valamint a szemcsék morfológiája is hatást gyakorol rá. A lipofil meloxikámhoz képest a készített minták hidrofil sajátságot mutatnak. Megállapítható, hogy a PVA alkalmazása és a fajlagos felület növelése fokozta a minták polaritását. A leucinnak köszönhetően viszont a részecskék közötti kohéziós munka csökkenthető.
Elvégeztük a minták in vitro kioldódási vizsgálatát mesterséges tüdőfolyadékban módosított forgólapátos kioldókészülékben. Jól látható kezeletlen meloxikámhoz képest a nanonizált minták a megnövekedett fajlagos felületüknél fogva már az első 5-10 percben 60 százalékos hatóanyag felszabadulást mutattak.

A minták in vitro türődepozícióját Andersen féle kaszkádimpaktor mérésekkel tanulmányoztuk, ami gyógyszerkönyvi vizsgálóeszköz az inhalációs készítmények hatékonyságának ellenőrzésére. Az impaktor egy légcsőből és 8 szűrő szintből és egy filterből áll, melyek a tüdő adott szegmenseit szemléltetik. A vizsgálat során mintákat kapszuláztuk, inhalátorba tettük és nehézlégzésnek megfelelően lélegeztettük be. Szintenként vizsgáltuk a deponálódott hatóanyag mennyiségét, amit spektrofotométerrel határoztuk meg. A minták eloszlásából KaleidaGraph program alkalmazásával kaptuk meg az eredményeket, a kedvező 2 mikronos MMAD-t és a 40-55%-os FPF értékeket.
A sztöchasztikus tüdőmodell egy számítógépes szimuláció, ami a légutakba belélegzett részecskék transzportját és depozícióját vizsgálja. Egyénre szabott adhatóak meg adatok a kor, nem és betegség függvényében, ezzel növelhető a hatékonyság és mérsékelhetőek mellékhatások. A számításokhoz felhasználtuk a termékeink méreteloszlását, morfológiáját és sűrűségét. Az in vitro méréshez hasonlóan mindegyik rendszernél magas tüdődepozíciót kaptunk eredményül. A leucin alkalmazása növeli a felső légúti kitapadást, de csökkenti a kilélegzett frakció mennyiségét.

Összefoglalásként elmondhatjuk, hogy munkánk során ko-porlasztásos eljárással sikeresen állítottunk elő hordozó mentes amorf sajátságú nanonizált hatóanyagot tartalmazó mikrokompozitokat. Megfigyeltük a leucin morfológiára, kohézióra, és tüdődepozícióra gyakorolt kedvező hatását.
Az optimalizált összetételnek és az új típusú szerkezetnek köszönhetően a termékekkel hatékony pulmonális bevitel valósítható meg, mivel tüdődepozíciója magasabb, mint a forgalomban levő készítményeké, ennek köszönhetően a jövőben több légúti megbetegedés kezelésére is lehetőséget adnak.
Jövőbeni terveink között szerepel kombinációs készítmények előállítása, vizsgálati és előállítási protokollok fejlesztése.

Munkánk során porlasztva szárítást alkalmaztunk, ami az iparban régóta és széles körben alkalmazott eljárás szilárd fázisú gyógyszerkészítmények előállítására, léptéknövelhető és reprodukálható, kedvező aerodinamikai tulajdonságokkal rendelkező részecskék nyerhetők vele.
