Having been an RN in a few different countries, namely the Philippines, Jordan, Saudi Arabia, and Canada, impacted how I evolved as a nurse in the 21st century. Starting as an RN in a third world country 13 years ago was challenging. Instruments in the operating room, as well as gloves, were reusable through the process of autoclaving. Patients documents were hand or typewritten and attached in a solid steel frame chart. In far-flung areas where hospitals are out of reach, people die of myocardial infarction and infectious diseases due to the lack of resources. There is available transportation, including an ambulance; however, there is the absence of an AED or life-saving medications and providers to treat a disease. Besides, the travel takes at least 3 hours to arrive in the hospital. That is the reality of my hometown in the Philippines.
As I ventured along as a nurse in the Middle East, I was amazed at a national system of government-funded health care and how technology evolved. I practice how to use computer-based electronic health and utilized advanced technology in my daily patient care encounter. However, the electronic health record didn't have much of the options in transmitting data compared to the electronic health record (EHR) that we have today. Those days, the usage of a computer was to record nurses' notes, input records of vital signs, and check laboratory and imaging results of the patient. In today's world, we can see competitions and evolving EHR in hospitals. Nowadays, several hospitals and health systems with mature EHRs have portals for patients to access and record their health data. (Glassman, 2017).
We are now in the future of involving patients in their care. I got awestruck with telemedicine and hospital bill payment transactions through hospital portals. Globally, the spread of information in several countries in the hospital setting evolved tremendously. Saudi Arabia's government spends thousands to millions of dollars to develop and improve the kingdom's health care through research and technology. King Faisal Specialist Hospital & Research Centre (KFSH&RC) is Saudi Arabia's first integrated digital hospital that successfully utilized superior health information technology upon achieving Stage 7 on the ambulatory Electronic Medical Record Adoption Model (EMRAM). Alswailem(2017).
Before my arrival in the US, It was part of my obligation to do a 16-week online course in Health Stream as part of my transition in the US nursing practice. It is a great advantage to re-learn basic nursing principles using animations and online patient simulations. I also learn about the problems of miscommunications in the hospital setting due to the use of abbreviations. All health care settings should create a plan for impl.

The CEO of the hospital has made a priority for the hospital to enter the 21st century by converting its operations to an Electronic Health Records System. You have been assigned the role of project manager for this effort, and are therefore responsibility for ensuring that the needs of the hospital have been carefully assessed and that the planning process for bringing an EHRS online is effective.
The Organization
At first glance, MacIntosh Manor Hospital looks like any small town hospital, where the quality of care is certainly modern but the staff and patients still come to know each other and expect a down-to-earth pace. The Good Apples Group, a parent corporation maintaining several local clinics in addition to MacIntosh Manor, has worked hard over the years to maintain that balance of customer service and cutting edge patient care.
MacIntosh Manor Hospital is a 500-bed, critical access hospital in Shiminy, Pennsylvania. It is the largest hospital within 100 miles, and schedules between 22,000 to 26,000 visits a year. MMH provides surgical, medical and acute care, 24-hour emergency room services, outpatient services, health education, behavioral services, and home and hospice care to a primarily suburban region of about 124,000 residents. It employs 2200 people, including 275 physicians, and as a hospital is managed by its own CEO.
MMH’s mission is to provide for the residents of Shiminy high quality services that enhance the quality of life and promote healthy lifestyles for patients, clients, employees, organization and communities. In its community MMH seeks to lead by example through compassionate, caring and comprehensive health care services.
The direction of MacIntosh Manor’s strategic vision change greatly in 2010 when Dr. Phillip Kapp was named CEO of the Good Apples Group and made it clear that his own successes with implementing EHRS and related technologies in healthcare facilities in the Philadelphia area would be the kind of challenge he wished to take on again with MacIntosh Manor Hospital. Kapp formed a strategic planning committee to assess the use of technology and what MMH should implement, and to determine and monitor a migration path.
Around the same time that Dr. Kapp took charge of the Good Apples Group, a federal mandate was issued that hospitals nationwide need to be using electronic medical records by 2015, giving Kapp and the strategic planning committee even more incentive to act quickly. The committee recommended beginning the transition to EHRS immediately and implementing both financial and clinical solutions.

learn it is an upper respiratory infection – a very treatable
condition.
The method of her diagnosis is

Campus performed the exam by
videoconference, guiding a team of FSU
doctors and medical students onsite in
Panama on what tests to perform.

2. Bringing Care to the
Underserved
FSUCares is an organization of
medical students devoted to helping
underserved and underprivileged
populations both locally and
internationally. The group delivers
health services to those who would
otherwise not have ready access to
care, allowing students to experience
an interactive, clinical learning
environment.
Since 2002, a team of doctors and
medical students from the FSUCares
program has traveled to Panama on
annual medical missions. The group
identified a particular need in the
community of Filipina, a town about
two hours from Panama City.
“The entire community of Filipina
would have to travel to another town
to get healthcare once a month, or
would have to travel to a medium-size
city, which was very cost prohibitive,”
said Mark Stavros, M.D., who
coordinates the emergency medicine
curriculum and training of third- and
fourth-year medical students at the
Pensacola Regional Medical School
Campus. Stavros has traveled to
Panama with FSUCares nine out of
the last 11 years.
Until recently, Filipina lacked a health
clinic of its own. FSUCares worked to
establish a permanent clinic that is
recognized, supported and staffed by
the government.
During FSU’s annual spring trip,
faculty doctors and students join with
the Filipina clinic staff to run
temporary clinics at schools
throughout the area. Their efforts
create an opportunity for locals to
receive care that would otherwise be
difficult to obtain, especially for those
needing specialized medicine.
Portable, High-
Definition Telemedicine
This past year, the FSU team
expanded its reach by connecting
virtually with doctors back at the
FSU campus in Pensacola.
GlobalMed loaned the team the

and connect with colleagues. It’s an
extension of our own hands,” Stavros
said.

To enable videoconferencing, Stavros
and the team used Scopia Desktop, a
web browser plug-in, on the TES™
laptop and on a computer at FSU.
Stavros purchased an air card in
Panama to connect via cellular
service, which was available in most
of the clinic locations. He also had the
option to use Scopia Mobile on his
Android tablet.
When the connection temporarily did
not enable live streaming , the team
took photos and recorded exam
notes, and later sent them to the
secure cloud where colleagues in
Florida could access and review them.
Patient data was protected by Scopia
with an advanced encryption that
meets HIPAA regulations.

very clear images with colleagues.
“We did a head-to-toe exam on the
woman with shortness of breath,”
Stavros said. “My colleague at FSU,
Dr. Kim Landry, directed us, saying, ‘I
want to listen to her heart. I’d like an
EKG.’ ”
It was particularly rewarding to
see one young woman receive
confirmation that she was pregnant
and to witness her seeing her baby for
the first time on ultrasound, made
possible by the TES™.
For medical students, first-hand
telemedicine experience adds
dimension to their education. For
patients, the technology provides rare
access to medical specialists. For that
kind of care, patients would otherwise
have to go to a more populated area –
travel that’s expensive and difficult,
especially for those who are ill.
“Typically if someone needs more
specialized care, you would have to
travel 25 miles and it would cost
you one to two weeks’ salary to get
there. Imagine how life-changing
telemedicine can be for a community
like this,” Stavros said.
In the future, Stavros sees an
opportunity to apply telemedicine on
a more regular basis, by equipping
the permanent clinic in Filipina with
the tools that will enable connectivity
with students and faculty at FSU, and
by sending students on their own to
Filipina to conduct examswith virtual
support from faculty.
“I’d like to set up a program where
part of the students’ experience was
going to Panama and interfacing with
the faculty here to render care,”
Stavros said.
“The TES is a compact case on wheels that I put
in the overhead bin like any carry-on. It comes
with all the peripherals we need to do the exam
and connect with colleagues. It’s an extension
of our own hands.”
