ผู้ป่วยชายไทย อายุ 19 ปี เชื้อชาติไทย สัญชาติไทย
ปฎิเสธการประกอบอาชีพ เข้ารับการรักษาที่ รพ.
มหาราช วันที่ 06/06/60

3. Chief Complaint
อาการเจ็บเท้าซ้ายมา 3 วันก่อนมารพ

4. Present Illness
3 วัน ก่อนมารพ. จากผู้ป่วยประสบอุบัติเหตุทาง
รถจักรยานยนต์ ขับตกถนนเนื่องมาจากหลับใน สลบ จา
เหตุการณ์ไม่ได้ ปวดเท้าซ้ายมาก ไม่สามารถขยับข้อเท้าซ้ายได้
ไม่สามารถลงน้าหนักได้ ไม่ชา รับความรู้สึกปลายนิ้วได้

16. Talus head fracture
Mechanism of injury :
Axially directed loadond and compression of
talar head
Dorsal compression fracture of anterior tibial
plafond

17. Talus neck fracture
30 % of talus fracture
Mechanism of injury
Forced hyperdorsiflexion at the ankle and
impingement of the taller neck on the distal
anterior tibia.
Axial load to plantar foot causes talar neck
fracture.

18. Talus body fracture
Mechanism of injury
Axial compression of the talus between tibial
plafond and the calcaneus
Usually seen in motor vehical accidents and falls
from height

19. Talar neck fracture treatment
Avoid multiple attempt at close reduction
Open reduction
Anteromedial or anterolateral approach
Cannulated screw
NWB cast 6-12 wk

20. Treatment
Operative : ORIF
Surgical approach – anteromedial
approach with medial malleolus
ostotomy
