Ms. Carol Ramirez is a new resident in a long-term care facility based in Tucson, Arizona. One of the recreational therapists at the facility, Curt Swinney, is assigned to do her intake evaluation. The first thing Curt does is review her chart to leam about her history and her background. He finds that she is 75 years old, she has been widowed for two years, and in the years since her husband's death, she has become increasingly confused, demonstrating poor shortterm memory but adequate long-term memory. Her thoughts appear more disorganized, her speech is tangential, and her family physician is the one who has recommended that she move into a long- term care facility. She does not have children, but quasi has a lot of support from friends and family. After reviewing the chart, Kurt fills in as much information as he can on the BANDI-RT. He then meets with Ms. Ramirez to fill out the other sections. They meet in the lounge, a noisy room that has TVs on at a loud volume and where many residents are socializing. On this particular day, bingo is being played in the room, so there are frequent distractions and loud shouts from participants. Kurt finds Miss Ramirez it easy to converse with but the information she provides him from section to section is unreliable and contradictory. Ms. Ramirez indicates that her primary le'sure interests have been physical and social in nature; however, when he suggests that they go for a walk toge fier, she refuses, saying. "I hate to walk." He suggests that they go to exercise group, but she states, "I hate to exercise." She then starts to act aggressively toward him. 1. Curt shares, Ms. Ramirez's reaction with an attending physician. The physician states. "Well of course she is: confused. She is 75 years old:" How should Curt respond? 2. Describe the differences between delirium and dementia. Case Vignette: Counseling Older Adults - Sarah
Assessment/Conceptualization
Sarah’s presenting concerns are clear signs of depression and anxiety seemingly stemming from the demands of her caregiving responsibilities for her husband with Alzheimer’s. She describes her feelings as being overwhelmed, anxious, and very sad about the changes her husband has gone through. Sarah talks about getting angry in response to her husband expressing anger as well as getting upset when he is difficult to care for. Other notable aspects of Sarah’s presentation include that she becomes tearful when speaking about her husband, and she explains that she helps him with daily activities, dressing, bathing, on top of her taking care of all their home management and maintenance upkeep. She also describes not sleeping well at night due to her husband wandering around during the night and having her own health problems including being morbidly obese, with high blood pressure, high cholesterol, and peripheral vascular disease. The main sociohistorical contexts of Sarah’s identity that I am privy to now are the fact that she is an older adult, and caregiver to her spouse. I am unaware of her racial, ethnic, religious, or socioeconomic identities, and therefore cannot take these into account before meeting with her.
Current existing research suggests that given Sarah’s particular presenting concern, and the other aspects of Sarah that I know about, the evidence-based practices to implement when working with Sarah will be cognitive behavioral therapy (CBT), as well as relaxation training such as breathing exercises, progressive relaxation of the muscles, and visualization (U.S. Department of Health and Human Services, 2021). Ayers et al. (2007) conducted a review on evidence-based treatments for late-life anxiety and concluded that CBT and relaxation training had the most support for lessening symptoms of anxiety, whereas cognitive therapy and supportive therapy did not show lessened symptoms in older adults. Trevino et al. (2021) carried out a research study to test effectiveness of different anxiety interventions for older adults with cancer and their caregivers and concluded that a seven-session CBT-based psychotherapy intervention was associated with the greatest reduction in anxiety among the pairs of participants. Although Sarah’s husband has Alzheimer’s and not cancer, this study’s focus on an ill older adult and their caregiver further shows support for using CBT as the primary evidence-based practice in treating Sarah.

People compare themselves to one another, their bodies, accomplishments, grades, and even their sex life. Girlfriends and Bros have their conversations, " I wish my girl did that" or " Wow, your man is so romantic, I never get that treatment." My administrative supervisor shared with us one day that the entire 42 years of marriage, she never experienced an orgasm. He did what he had to do, rolled over and went to sleep. She explained that back then (she is 65), that is the way it was. My supervisor never experienced an orgasm from her husband, which caused her to be dissatisfied with her sex life. However, another female could have different reasons for not being satisfied with hers.

Client Information
INITIAL CASE CONCEPTUALIZATION: PTSD !2
Maria is a 25 year old African American female, with four children under the age of six
years old. To ensure her protection of her identity, I will refer to her as Maria. Maria states that
she is a christian and does not attend church often. Maria reports that she would like to become
more active in church again. Maria says that she is currently in an abusive relationship with her
spouse of seven years. While she is not physically disabled, she mentioned that she is facing var-
ious psychological challenges that have made her live in an inpatient psychological hospital.
Maria says that she identifies as a female who is attracted to the opposite sex. She is currently
working two jobs to sustain her life and the lives of her four children aged six, three, one, and
two months (1boy and 3 girls). Therefore, she is financially and economically challenged; but
she has been doing the best she could to make ends meet despite her circumstances.
Maria has managed to secure a 2-bedroom apartment that she shares with her four chil-
dren. Besides her two jobs, she also seeks welfare support to help her with daycare, medical and
food assistance. Maria also mentioned that her older brother sends her checks from time to time
to help her with the children. Additionally, she makes use of food stamps that have been availed
to her to cut the cost of food. Some non-governmental organization has also volunteered to take
care of educational needs of her two older children. While her emotional and psychological state
seems a bit unstable, Maria is physically healthy, and reports that she takes care of her physical
wellbeing, by walking 3 miles per day, to catch a bus to work.
Maria presenting issues is PTSD, she has been sexually assaulted numerous of times
throughout her childhood. She seems to be trying to forget some issues of her past that bring
back bad memories regarding her sexual abuse. She seems traumatized by her sexual past, which

is apparent in how she disregards any questions about her sexual abusive past. The relevant his-
tory behind her traumatic response to sexual-related questions stems from her experiences as a
young girl. At the time of the abuse, she was living with her grandmother, cousins, uncles, broth-
ers, and aunts. From the ages of four to nine, she was sexually abused by her family members.
The trauma from those incidents has seemingly remained with her in her adult life. It is affecting
her relationship with males.
Theoretical Approach
The theoretical approach I used for Maria was Trauma-Focused Cognitive Behavioral
Therapy (TF-CBT). This is an evidence-based theoretical approach to treatment of traumatized .

Afterward, Bob threatened to harm her family if she did not remain silent and proceeded to drive her home. Although Rita did not tell her family what happened, she did call our agency hotline the next day to discuss her options. Background History
The following background information was obtained from an interview with Smith and a review of the demographic information sheet that she completed before the evaluation.
Smith is a forty-one-year-old Caucasian female who was referred for a psychological evaluation by the Division of Vocational Rehabilitation to assist with determining eligibility and to assess whether her emotional problems are interfering with her ability to work. She initially requested assistance from the Division of Vocational Rehabilitation in October 2008 to assist her with maintaining employment. At this time, she is interested in learning new skills to enable her to find full-time work in an office setting.
Smith was born in Jersey City, New Jersey, and raised in a small nearby town, Williamsport,
Pennsylvania. She is the oldest of three children born to her mother and father following an uncomplicated pregnancy and deliver. Her younger sisters relied upon her for their after-school child care once their mother returned to work and when she was twelve years old. She spoke of her mother as having been physically and emotionally abusive in the past, often yelling, hitting her, and pushing her around. While her mother took her frustration out on Smith, her father would drink alcohol in excess. Social workers must understand this complex relationship so they can educate and empower their clients. Empowerment is the cornerstone of social work practice.
In the book
Black Empowerment
, the author,Barbara Solomon, writes, "Empowerment refers to a process whereby persons who belong to a stigmatized social category throughout their lives can be assisted to develop and increase skills in the exercise of interpersonal influence and the performance of valued social roles" (p. 6).
Empowerment practice is based on a collaborative relationship between worker and client, initiated to promote the client's power through self-actualization, self-determination, and the fulfillment of personal goals (Gutierrez, Parsons, & Cox, 1998). Further, this process includes increasing your clients' awareness of the structural oppression that exists and its impact on them. It is the role of social workers to empower clients and to bring about awareness of the inequalities that exist in society.
This is done two fold—through work with clients on the micro and mezzo levels and on a macro level through work in organizations and communities.
Social workers cannot effectively empower clients without first understanding the mechanisms of oppression and how they impact their day-to-day work. Then social workers can recognize the impact on their clients' lives and apply the appropriate skills.

"All happy families are alike, every unhappy family is unhappy in its way." Whoa, that is quite a statement, insightful, and makes you ponder. I believe this quote can be related to sexuality. People compare themselves to one another, their bodies, accomplishments, grades, and even their sex life. Girlfriends and Bros have their conversations, " I wish my girl did that" or " Wow, your man is so romantic, I never get that treatment." My administrative supervisor shared with us one day that the entire 42 years of marriage, she never experienced an orgasm. He did what he had to do, rolled over and went to sleep. She explained that back then (she is 65), that is the way it was. My supervisor never experienced an orgasm from her husband, which caused her to be dissatisfied with her sex life. However, another female could have different reasons for not being satisfied with hers.

INITIAL CASE CONCEPTUALIZATION: PTSD !2
Maria is a 25 year old African American female, with four children under the age of six
years old. To ensure her protection of her identity, I will refer to her as Maria. Maria states that
she is a christian and does not attend church often. Maria reports that she would like to become
more active in church again. Maria says that she is currently in an abusive relationship with her
spouse of seven years. While she is not physically disabled, she mentioned that she is facing var-
ious psychological challenges that have made her live in an inpatient psychological hospital.
Maria says that she identifies as a female who is attracted to the opposite sex. She is currently
working two jobs to sustain her life and the lives of her four children aged six, three, one, and
two months (1boy and 3 girls). Therefore, she is financially and economically challenged; but
she has been doing the best she could to make ends meet despite her circumstances.
Maria has managed to secure a 2-bedroom apartment that she shares with her four chil-
dren. Besides her two jobs, she also seeks welfare support to help her with daycare, medical and
food assistance. Maria also mentioned that her older brother sends her checks from time to time
to help her with the children. Additionally, she makes use of food stamps that have been availed
to her to cut the cost of food. Some non-governmental organization has also volunteered to take
care of educational needs of her two older children. While her emotional and psychological state
seems a bit unstable, Maria is physically healthy, and reports that she takes care of her physical
wellbeing, by walking 3 miles per day, to catch a bus to work.
Maria presenting issues is PTSD, she has been sexually assaulted numerous of times
throughout her childhood. She seems to be trying to forget some issues of her past that bring
back bad memories regarding her sexual abuse. She seems traumatized by her sexual past, which

is apparent in how she disregards any questions about her sexual abusive past. The relevant his-
tory behind her traumatic response to sexual-related questions stems from her experiences as a
young girl. At the time of the abuse, she was living with her grandmother, cousins, uncles, broth-
ers, and aunts. From the ages of four to nine, she was sexually abused by her family members.
The trauma from those incidents has seemingly remained with her in her adult life. It is affecting
her relationship with males.
Theoretical Approach
The theoretical approach I used for Maria was Trauma-Focused Cognitive Behavioral
Therapy (TF-CBT). This is an evidence-based theoretical approach to treatment of traumatized .

Racism is the set of institutional, cultural, and interpersonal patterns and practices that create advantages for people legally defined and socially constructed as white and corollary disadvantaged for belonging to racial groups that were considered essential for the formation of the United States. Therefore, the relationship between racism and privilege is the ideology that some persons deserve better, while others should be considered last because of their appearance. Charo is a 34-year-old, heterosexual, Hispanic female. She is unemployed and currently lives in an apartment with her ve chil- dren, ages 2, 3, 6, 7, and 8. She came to this country 8 years ago from Mexico with her husband, Paulo. During intake, Charo reported that she suffered severe abuse and neglect in the home as a child and rape as a young adult. Charo does not speak English and currently does not have a visa to work.
Charo initially came for services at our domestic violence agency because Child Protective Services (CPS) and the court ordered her to attend a domestic violence support group after allegations of domestic violence were made by one of her chil- dren to a teacher at their school. Her husband was ordered to attend a batterer’s intervention program (BIP). Charo attended the domestic violence support group but seldom said a word. Although she rarely shared during group, she also rarely missed a session. While she attended the group, she also met with me weekly for individual sessions. During these sessions I informed her of the dynamics of domestic violence and helped her create a safety plan. She often said that she was only attending the group because it was mandated and that she just wanted CPS to close her case. One week, Charo suddenly stopped attending group. When I called her, she said that she had been busy and unable to attend. That same day her husband called me to verify that I was who his wife said I was, as he often accused Charo of having affairs.

Domestic violence is any repeated attempt to control an intimate partner using physical, emotional and/or sexual tactics.
In 1987 Ellen Pence, a long-time domestic violence advocate with the Domestic Abuse Intervention Project in Duluth, Minnesota, published a workbook entitled
In Our Best Interest: A Process For Personal and Social Change
(1987) (I've found this an excellent resource when conducting support groups for women). In this work Pence includes a diagram that has commonly come to be known as the
Power and Control Wheel
. This diagram is based on the feedback of hundreds of battered women and outlines the various controlling and abusive behaviors that perpetrators use to control their partners. The "Wheel" has become the signature diagram used to explain survivors' experiences in abusive relationships. The tactics batterers use to control their partners are without limit. While some are certainly more dangerous than others, all forms of abuse have serious short and/or long-term consequences for the victim, and for those around them.
In reviewing the "Wheel" please note that the tactics within the spokes of the wheel actually work to support and reinforce the physical and sexual violence occurring in the relationship. In some abusive relationships, over time, the abusing partner may decrease the amount of physical violence and rely more and more on the non-physical tactics to control the survivor. This is not true for all abusive relationships, however.

Charo is a 34-year-old, heterosexual, Hispanic female. She is unemployed and currently lives in an apartment with her ve chil- dren, ages 2, 3, 6, 7, and 8. She came to this country 8 years ago from Mexico with her husband, Paulo. During intake, Charo reported that she suffered severe abuse and neglect in the home as a child and rape as a young adult. Charo does not speak English and currently does not have a visa to work.
Charo initially came for services at our domestic violence agency because Child Protective Services (CPS) and the court ordered her to attend a domestic violence support group after allegations of domestic violence were made by one of her chil- dren to a teacher at their school. Her husband was ordered to attend a batterer’s intervention program (BIP). Charo attended the domestic violence support group but seldom said a word. Although she rarely shared during group, she also rarely missed a session. While she attended the group, she also met with me weekly for individual sessions. During these sessions I informed her of the dynamics of domestic violence and helped her create a safety plan. She often said that she was only attending the group because it was mandated and that she just wanted CPS to close her case. One week, Charo suddenly stopped attending group. When I called her, she said that she had been busy and unable to attend. That same day her husband called me to verify that I was who his wife said I was, as he often accused Charo of having affairs.
Charo showed up to group again one day.

Domestic violence is any repeated attempt to control an intimate partner using physical, emotional and/or sexual tactics.
In 1987 Ellen Pence, a long-time domestic violence advocate with the Domestic Abuse Intervention Project in Duluth, Minnesota, published a workbook entitled
In Our Best Interest: A Process For Personal and Social Change
(1987) (I've found this an excellent resource when conducting support groups for women). In this work Pence includes a diagram that has commonly come to be known as the
Power and Control Wheel
. This diagram is based on the feedback of hundreds of battered women and outlines the various controlling and abusive behaviors that perpetrators use to control their partners. The "Wheel" has become the signature diagram used to explain survivors' experiences in abusive relationships. The tactics batterers use to control their partners are without limit. While some are certainly more dangerous than others, all forms of abuse have serious short and/or long-term consequences for the victim, and for those around them.
In reviewing the "Wheel" please note that the tactics within the spokes of the wheel actually work to support and reinforce the physical and sexual violence occurring in the relationship. In some abusive relationships, over time, the abusing partner may decrease the amount of physical violence and rely more and more on the non-physical tactics to control the survivor. This is not true for all abusive relationships, however.

- 1. Ms. Carol Ramirez is a new resident in a long-term care facility based in Tucson, Arizona. One of the recreational therapists at the facility, Curt Swinney, is assigned to do her intake evaluation. The first thing Curt does is review her chart to leam about her history and her background. He finds that she is 75 years old, she has been widowed for two years, and in the years since her husband's death, she has become increasingly confused, demonstrating poor shortterm memory but adequate long-term memory. Her thoughts appear more disorganized, her speech is tangential, and her family physician is the one who has recommended that she move into a long- term care facility. She does not have children, but quasi has a lot of support from friends and family. After reviewing the chart, Kurt fills in as much information as he can on the BANDI-RT. He then meets with Ms. Ramirez to fill out the other sections. They meet in the lounge, a noisy room that has TVs on at a loud volume and where many residents are socializing. On this particular day, bingo is being played in the room, so there are frequent distractions and loud shouts from participants. Kurt finds Miss Ramirez it easy to converse with but the information she provides him from section to section is unreliable and contradictory. Ms. Ramirez indicates that her primary le'sure interests have been physical and social in nature; however, when he suggests that they go for a walk toge fier, she refuses, saying. "I hate to walk."
