Ms. Carol Ramirez is a new resident in a long-term care facility based in Tucson, Arizona. One of the recreational therapists at the facility, Curt Swinney, is assigned to do her intake evaluation. The first thing Curt does is review her chart to leam about her history and her background. He finds that she is 75 years old, she has been widowed for two years, and in the years since her husband's death, she has become increasingly confused, demonstrating poor shortterm memory but adequate long-term memory. Her thoughts appear more disorganized, her speech is tangential, and her family physician is the one who has recommended that she move into a long- term care facility. She does not have children, but quasi has a lot of support from friends and family. After reviewing the chart, Kurt fills in as much information as he can on the BANDI-RT. He then meets with Ms. Ramirez to fill out the other sections. They meet in the lounge, a noisy room that has TVs on at a loud volume and where many residents are socializing. On this particular day, bingo is being played in the room, so there are frequent distractions and loud shouts from participants. Kurt finds Miss Ramirez it easy to converse with but the information she provides him from section to section is unreliable and contradictory. Ms. Ramirez indicates that her primary le'sure interests have been physical and social in nature; however, when he suggests that they go for a walk toge fier, she refuses, saying. "I hate to walk." He suggests that they go to exercise group, but she states, "I hate to exercise." She then starts to act aggressively toward him. 1. Curt shares, Ms. Ramirez's reaction with an attending physician.
