MRI safety regulation, Danish/Nordic MR Meeting, 2019Lars G. Hanson
This document discusses regulatory aspects of MR safety in Denmark. It outlines that management has overall responsibility to ensure safe treatment and working conditions, while employees must act according to their training and procedures. New Danish laws will clarify that adverse events due to working conditions are not an employee's responsibility. Safety regulations are based on reviews by organizations like ICNIRP and result in standards like IEC 60601-2-33 which specify safety requirements for MR equipment to aim for zero risk to patients and employees. The EU and Danish authorities implement these standards through directives that are then applied practically in regulations and guidelines.

Product Design Trends in 2024 | Teenage EngineeringsPixeldarts
The realm of product design is a constantly changing environment where technology and style intersect. Every year introduces fresh challenges and exciting trends that mold the future of this captivating art form. In this piece, we delve into the significant trends set to influence the look and functionality of product design in the year 2024.
How Race, Age and Gender Shape Attitudes Towards Mental HealthThinkNow
Mental health has been in the news quite a bit lately. Dozens of U.S. states are currently suing Meta for contributing to the youth mental health crisis by inserting addictive features into their products, while the U.S. Surgeon General is touring the nation to bring awareness to the growing epidemic of loneliness and isolation. The country has endured periods of low national morale, such as in the 1970s when high inflation and the energy crisis worsened public sentiment following the Vietnam War. The current mood, however, feels different. Gallup recently reported that national mental health is at an all-time low, with few bright spots to lift spirits.
To better understand how Americans are feeling and their attitudes towards mental health in general, ThinkNow conducted a nationally representative quantitative survey of 1,500 respondents and found some interesting differences among ethnic, age and gender groups.
Technology
For example, 52% agree that technology and social media have a negative impact on mental health, but when broken out by race, 61% of Whites felt technology had a negative effect, and only 48% of Hispanics thought it did.
While technology has helped us keep in touch with friends and family in faraway places, it appears to have degraded our ability to connect in person. Staying connected online is a double-edged sword since the same news feed that brings us pictures of the grandkids and fluffy kittens also feeds us news about the wars in Israel and Ukraine, the dysfunction in Washington, the latest mass shooting and the climate crisis.
Hispanics may have a built-in defense against the isolation technology breeds, owing to their large, multigenerational households, strong social support systems, and tendency to use social media to stay connected with relatives abroad.
Age and Gender
When asked how individuals rate their mental health, men rate it higher than women by 11 percentage points, and Baby Boomers rank it highest at 83%, saying it’s good or excellent vs. 57% of Gen Z saying the same.
Gen Z spends the most amount of time on social media, so the notion that social media negatively affects mental health appears to be correlated. Unfortunately, Gen Z is also the generation that’s least comfortable discussing mental health concerns with healthcare professionals. Only 40% of them state they’re comfortable discussing their issues with a professional compared to 60% of Millennials and 65% of Boomers.
Race Affects Attitudes
As seen in previous research conducted by ThinkNow, Asian Americans lag other groups when it comes to awareness of mental health issues. Twenty-four percent of Asian Americans believe that having a mental health issue is a sign of weakness compared to the 16% average for all groups. Asians are also considerably less likely to be aware of mental health services in their communities (42% vs. 55%) and most likely to seek out information on social media (51% vs. 35%).
AI Trends in Creative Operations 2024 by Artwork Flow.pdfmarketingartwork
Creative operations teams expect increased AI use in 2024. Currently, over half of tasks are not AI-enabled, but this is expected to decrease in the coming year. ChatGPT is the most popular AI tool currently. Business leaders are more actively exploring AI benefits than individual contributors. Most respondents do not believe AI will impact workforce size in 2024. However, some inhibitions still exist around AI accuracy and lack of understanding. Creatives primarily want to use AI to save time on mundane tasks and boost productivity.
Organizational culture includes values, norms, systems, symbols, language, assumptions, beliefs, and habits that influence employee behaviors and how people interpret those behaviors. It is important because culture can help or hinder a company's success. Some key aspects of Netflix's culture that help it achieve results include hiring smartly so every position has stars, focusing on attitude over just aptitude, and having a strict policy against peacocks, whiners, and jerks.

Google's Just Not That Into You: Understanding Core Updates & Search IntentLily Ray
1. Core updates from Google periodically change how its algorithms assess and rank websites and pages. This can impact rankings through shifts in user intent, site quality issues being caught up to, world events influencing queries, and overhauls to search like the E-A-T framework.
2. There are many possible user intents beyond just transactional, navigational and informational. Identifying intent shifts is important during core updates. Sites may need to optimize for new intents through different content types and sections.
3. Responding effectively to core updates requires analyzing "before and after" data to understand changes, identifying new intents or page types, and ensuring content matches appropriate intents across video, images, knowledge graphs and more.

2. Oversigt
• Generelle regler
• Viden om elektromagnetisme og sundhed
• Direktiv for medicinsk udstyr, 93/42/EC
• Note: Erstattes med nyt fra 2020.
• Normen IEC 60601-2-33 angående krav til
MR-skannere
• EU direktiv for arbejde i elektromagnetiske
felter
• Forbehold:
– Brug info på eget ansvar. Check gyldighed!
4. Patientsikkerhed
• Ansvar
– Overordnet er behandling og
patientsikkerhed et ledelsesansvar.
– Den enkelte ansatte har ansvar for at følge
lokale instrukser og faglige retningslinjer.
– Et lovforslag fra oktober 2018 præciserer
at skader, der skyldes arbejdsforhold ikke
er den enkelte ansattes ansvar.
• Bred opbakning, men er i høring.
• Forventes gældende fra juli 2019
(delvist januar 2019).
http://www.folketingstidende.dk/samling/20181/lovforslag/L63/index.aspx
5. Utilsigtede hændelser
• Utilsigtede hændelser skal indberettes
– begivenheder som forvolder skade, eller
kunne forvolde skade på patienten
– både egne og andres hændelser
– obligatorisk i hele sundhedssektoren
– gøres via Styrelsen for Patientsikkerhed
– Sagsbehandlig foretages lokalt, og
registreres centralt (anonymiseret)
6. Ansvar for arbejdsmiljø
• Arbejdsgivere skal…
– sikre at arbejdsmiljøet er sikkert.
– foretage risikovurdering.
– sikre tilstrækkelig uddannelse for alle
ansatte.
• De enkelte ansatte skal…
– i videst mulig omfang sikre egen og
andres sikkerhed og følge arbejdsgivers
uddannelse og retningslinjer.
7. Arbejdsmiljø
• Regler og vejledninger
– Almindelige arbejdsmiljøregler
– Direktiv for arbejde i EM felter (se senere slides)
– Arbejdstilsynets vejledninger
• f.eks. AT-vejledning vedr. gravide og ammendes arbejdsmiljø
– Branchevejledninger (meget lidt MR)
Arbejdstilsynet: Danish working environment authority
9. ICNIRP
• ”International Commission on
Non-Ionizing Radiation Protection”:
– Lægefaglig organisation (primært), der løbende
vurderer viden om felt-effekter.
• ikke-ioniserende stråling, elektriske/magnetiske felter.
– Udgiver guidelines for begrænsning af effekter
inkl. grænseværdier.
– ICNIRPs overvejelser indgår ved fastsættelse af
normen for MR-ustyr, f.eks. SAR grænser.
– ICNIRPS anbefalinger er udgangspunkt for EU-
direktiv vedrørende arbejdsmiljø i felter
– Andre organisationer foretager også reviews.
• Se f.eks. normen for MR-udstyr, IEC 60601-2-33.
10. Statisk felt
• Statisk felt review af ICNIRP, 2009:
– “In conclusion, current information does not
indicate any serious health effects resulting
from acute exposure to static magnetic fields
up to 8 T. It should be noted, however, that such
exposures can lead to potentially unpleasant
sensory effects such as vertigo during head or
body movement.”
AMENDMENT TO THE ICNIRP “STATEMENT ON MEDICAL MR
PROCEDURES: PROTECTION OF PATIENTS”
HEALTH PHYSICS 97(3):259‐261; 2009
11. Statisk felt
• ICNIRP om MRI, 2017:
• MRI is associated with several adverse
effects...However, all of these effects are reversible
and can be prevented... There is insufficient
evidence…although there is little theoretical reason to
expect any permanent damage if scanners are
operated in line with existing regulations. A priority for
further research is to investigate any potential harmful
effects from exposure to the fetus during pregnancy,
particularly during the first trimester….More
information on any long-term effects in well-defined
MRI worker cohorts would be useful.
ICNIRP STATEMENT ON DIAGNOSTIC DEVICES USING
NON-IONIZING RADIATION. HEALTH PHYSICS 112(3):305‐21; 2017
13. Eksempel ”foot print”, feltfordeling
200 mT
Konsekvens: 400 mT grænsen for gravide fordi fostret
er ”general public” fremfor ansat. Grænsen er ikke baseret
på viden om risiko, men forsigtighed.

isocenter
14. Grænseværdier
• ICNIRP&direktiver har 2 slags grænser:
– ”Basic restrictions”:
induceret strøm inde i vævet.
• Også kendt som ”exposure limit values”, ELVs
– ”Reference levels”:
• Praktisk målbare størrelser, som kan give en
indikation af om ”Basic Restrictions” er
overskredet, f.eks. feltændring per sek. (dB/dt).
• Også kendt som aktionsværdier/”action levels”
• Må overskrides, blot ELVs ikke bliver det.
15. Bevægelse i statisk felt
Undgå svimmelhed: ΔB bør ikke overskride 2T over en 3 sek. periode.
16. Krav til MR-udstyr udspringer fra
EU direktiv for medicinsk udstyr 93/42/EC
og aktive implantater 90/385/EC
Erstattes maj 2020 af
Regulativ for Medicinsk udstyr 2017/45
vedtaget maj 2017.
Skiftet har mange konsekvenser,
men vist ingen usædvanlige for MR.
18. Direktivers vej til medlemslande
• EU
– Vedtager (minimums-) direktiver, som
medlemslande skal implementere.
– Udpeger standardiseringsorganisationer til at
opstille tekniske krav som kan sikre opfyldelse
af direktiverne.
• Danske myndigheder
– Arbejdstilsynet, Sundhedsstyrelsen…
– Tilpasser og implementerer bl.a. EU direktiver
• ”Minimumsdirektiver” kan udvides.
– Standarder distribueres via. eksempelvis Dansk
Standard (privat, selvejende virksomhed).
19. Eksempel
• Eksempel:
– EUs Medikodirektiv vedtages (93/42/EC).
CENELEC bemyndiges til at udmønte direktivet.
– CENELEC: Den Europæiske komite for
elektroteknisk standardisering. Ikke-kommerciel
international organisation.
– Udfærdiger for EU sammen med IEC tekniske
standarder som sikrer EU direktivers
overholdelse (IEC: Udarbejder Internationale
elektrotekniske normer)
– Eksempelvis standarden 60601-2-33 for
elektromedicinsk udstyr: ”Særlige bestemmelser
for sikkerheden for MR udstyr til medicinsk
diagnose”.
– Standarden er ”harmoniseret” med
Medikodirektivet og sikrer dermed opfyldelse.
Lyder tungt, men sparer administration i medlemslande.
20. Eksempel, fortsat
• Implementering i medlemslande:
– Indenrigs- og sundhedsministeriet inkorporerer via
bekendtgørelse Medikodirektivet i dansk ret.
Stramninger kan forekomme.
– Derved er IECs/CENELECs tekniske standard for
MR-udstyr gældende i DK (IEC 60601-2-33)
• Godkendelse:
Eksempel: Presafe Denmark (tidligere DGM).
21. EN 60601-2-33, kortfattet
• Vigtigste dokument vedr. MR sikkerhed
• Standarden specificerer krav til MR-udstyr,
eksempelvis tilladelig…
– feltstyrke
– afsat energi (opvarmning af kroppen)
– gradientydelse
– mærkning
– testprocedurer
– …
• Gældende hoveddokument er fra 2010
– Rettelser og tilføjelser:
• 2011, 2013, 2015 (omfattende), 2016
22. • Standarder for test af implantaters sikkerhed:
Tilsvarende for implantater
Gregor Schaefers
23. Andre regler ang. patientsikkerhed
• Sundhedsstyrelsens retningslinjer for brug af
kontrasttoffer.
• Krav til speciallæger via Dansk Radiologisk
Selskabs ”Målbeskrivelse for Radiologi”.
– godkendt af Sundhedsstyrelsen.
• Irrelevant: Strålingsgeneratorbekendtgørelsen
– Bekendtgørelse nr. 86 af 2. februar 2018 om
medicinske røntgenanlæg til undersøgelse af
patienter (nyt ”røntgenregulativ”).
• området hører under
29. Den østrigske model
• Det generelle ansvar for MR sikkerhed
ligger hos ledelsen. Denne skal sikre,
at personer udpeget som ”MR safety
officer” er kompetente og har den
nødvendige myndighed til at udføre
denne opgave effektivt. For hver MR
enhed (som kan have flere skannere)
skal der være mindst en ”MR safety
officer” (ÖNORM S1125-1, 2009)
Forbehold for oversættelse
31. EN 60601-2-33 i detalje
• EN 60601-2-33: ”Særlige bestemmelser for
sikkerheden for MR udstyr til medicinsk
diagnose”.
– Vigtigste dokument på området.
– Foreligger på engelsk/fransk. Delvis teknisk.
– Målsætning: Nul risiko
• Modsat eksempelvis røntgenregulativet.
– Læs den i uddrag! (som minimum via udstyrsmanualer)
• resume af kendt viden på sikkerhedsområdet.
• grænseværdier og lignende som MR udstyr skal
overholde for at måtte sælges.
• vigtige definitioner af brugstilstande (”operating modes”).
• krav til dokumentation, skiltning m.m.
32. Ansvar for sikkerhed
• Ansvar for sikkerhed lægges delvis på
normen/fabrikanten/certificeringen….
– EN 60601-2-33 sikrer at der ikke sker skade på
patienten, forudsat at udstyret anvendes iflg.
medfølgende brugsanvisninger.
• Brugeren har også ansvar. Specifikt skal…
– udstyr anvendes og serviceres som angivet i
manual.
– brugstilstande anvendes iflg. forskrifter.
– Patientoplysninger indtastes korrekt.
• Forstå farerne!
33. Brugstilstande
• Standarden opererer med 3 vigtige
brugstilstande (”operating modes”):
– Normal-tilstand: Alle kan skannes i denne under
hensyntagen til kontraindikationer.
– 1. kontrollerede tilstand: Let øgede
grænseværdier. Skanning i denne tilstand kræver
aktiv godkendelse, lægelig vurdering og
supervision.
– 2. anden kontrollerede tilstand: Overskridelse af
fastsatte grænser. Kræver etisk godkendelse og
password.
• Tilstande defineres separat for hovedfelt,
gradienter og radiobølger.
34. • SAR (”specific absorption rate”):
– Sekvens- og person-specifik opvarmning.
– Skyldes brug af radiobølger.
• Grænser jf. standarden:
• Værdier er i W/kg midlet over 6 minutter.
• Midlertidigt (<10s) må værdier øges faktor 3
• Værdier indgår i skannerens SAR beregning.
• Personvægt SKAL tastes korrekt (indenfor få procent).
Eksempel: SAR-grænser
35. SAR-grænser, fortsat
• Brugstilstande i praksis:
– Skanneren starter for hver patient i normal-tilstand
standarden.
– Eksempel: Skift af SAR tilstand.
• Kan patienten styre egen temperatur?
(overvej eksempelvis feber, hjerteproblemer, diabetes,
36. Nyt i normen for MR udstyr
Felter mellem 4 og 8 tesla er siden 2015
”first-level controlled mode” (tidligere 2T-4T)
– Resultat: Kun lav-SAR us. kan foretages klinisk uden
ekstra godkendelse (indenfor CE mærkning)
37. Opsummering, MR-sikkerhed
via skannernes manualer).
• Designet mhp. nul risiko for patienter og
ansatte
– så begrænset yderligere lovgivning er berettiget.
• Forstå brugstilstande (”controlled modes”)
• Gældende hoveddokument er fra 2010
– Rettelser og tilføjelser:
• 2011, 2013, 2015 (omfattende), 2016
38. EU arbejdsmiljødirektiv for arbejde
i elektromagnetiske felter
National implementering fra 2016
er i kraft, og har konsekvenser.
39. Direktivet
• EU arbejdsmiljødirektivet for arbejde i
elektromagnetiske felter
– Håndtering uddybes i EU dokument
• ”Ikke-bindende vejledning i god praksis for
gennemførelse af direktiv 2013/35/EU”. NYTTIGT!
40. EU arbejdsmiljødirektiv
• Delvis MR-undtagelse:
• Begrundet i de meget specielle forhold vedr. MR,
direkte felteffekter.
41. Vejen til opfyldelse af krav
Fra ”Ikke-bindende vejledning
i god praksis for gennemførelse
af direktiv 2013/35/EU”
42. Betinget MR undtagelse. Direktiv-uddrag:
– Kun her kræves sikkerhedsvurdering.
god praksis for gennemførelse
af direktiv 2013/35/EU”
44. EU’s ”ikke-bindende guide i god praksis…”
• procedurer i nødstilfælde.
45. Afsluttende bemærkninger
• Direktivet for arbejde i EM-felter
– kom i søgelyset i 2002 (planlægningsfase)
– var indledende katastrofalt for MR
• gav ingen ekstra reel sikkerhed
• forhindrede standardprocedurer, som ingen mente var
problematiske.
46. Sammenfatning: Ansvar
• Ansvarsfordeling generelt:
– Ledelser har generelt ansvar for sikkerhed
• skal sikre tilstrækkeligt ansatte, uddannelse,
tid, hjælpemidler…
– Den enkelte ansatte har personligt ansvar:
• skal optræde fagligt forsvarligt, følge
instruktioner, bruge værnemidler,…
– Vurdering:
• For MR ses begge typer ansvar brudt. Eksempel:
– Kørestol med jern medbringes i skannerrum. Kan være
ledelsesansvar (procedure) eller personligt ansvar.
