The Moffitt Cancer Center Survivorship Program aims to provide consistent, evidence-based care to cancer survivors through health promotion, education, rehabilitation, and treatment of late or long-term side effects. The program currently serves over 900 unique patients, many of whom had breast, genitourinary, or gastrointestinal cancers. Survivors receive a personalized care plan that addresses long-term treatment effects, surveillance for recurrence or new cancers, health living recommendations, and psychosocial support. The program utilizes survivorship care plans that meet national standards to document treatment summaries and coordinate follow-up care between specialists and primary care physicians.
